Prisoners with all types of psychiatric disorders are numerous in prisons. As a result, access to care is a priority in order to detect, orient and treat these patients. The psychiatric care system in prisons is based on a triptych composed of local care teams, regional medical- psychological services and inter-regional structures, such as specially equipped hospital units. Although policies have now worked to develop access to psychiatric care in prisons, there are still constraints, linked to security, demographics and institutional requirements.

Since the beginning of the 21st century, legislation has been in place framing the rights of women prisoners. Pregnancy, motherhood and family life can be organised despite the complications and the difficulties. It is however preferable to favour alternative sentences, outside prison, in order to offer the child and the mother more suitable solutions.

The clarification of deaths from electric current can be difficult when electric skin marks are missing or overlooked. In the following, the fatal electric accident of a 21-year-old man in prison is reported with a scene which primarily seemed to be inconspicuous. The autopsy disclosed typical electric marks in the skin of the left thumb and the corresponding part of the left index finger. A second inspection of the jail room, which was large enough for three men, but occupied by the deceased alone, led to the finding of widely spread utensils (two forks with signs of corrosion and deformation

An investigation of three suspected cases of pulmonary histoplasmosis among 600 employees at a New York prison in a nonendemic area resulted in identification of 12 additional previously unrecognized cases. The 15 cases presented with diagnostic findings between February 1978 and October 1981; 10 were in prison employees, three in inmates and two in community persons. Unlike other outbreaks, cases presented over several years with chronic pulmonary lesions in the absence of an episode marked by acute respiratory illnesses. Of the 13 prison-associated cases, seven had undergone diagnostic thora

The aging of society is also leading to a growing number of elderly prisoners in the prison system. The challenges associated with this relate not only to the treatment and medical care of prisoners in accordance with legal requirements with the prospect of release from prison. The prison system must also find an appropriate way of dealing with prisoners who are spending the end of their lives in a correctional facility. So far, the prison system has only begun to address this issue. This primarily concerns the material and personnel requirements for a prison system that is suitable for the el…

Since the publication of the law of 18 January 1994, health care for detainees has been entrusted to public health establishments, and no longer to the prison administration. This is a real step forward insofar as individuals placed in the hands of justice now benefit from care provided by health professionals, who officiate within health units in prisons. Today, these professionals work in particular to promote preventive and health promotion actions, thereby helping to restore the detainee's place as a subject with a view to reintegrating him or her into the community.

Correctional mental health clinicians are sometimes asked to assess disciplinary responsibility, that is, to ascertain whether an inmate is culpable for violating prison rules. This assessment of disciplinary responsibility is akin to insanity determinations in criminal proceedings. In this article, I review the moral, legal, and practical aspects of disciplinary responsibility. I use California's test of responsibility for prison misconduct, which is similar to the Durham rule, to illustrate some of the dilemmas involved in creating and implementing a test of disciplinary responsibility.

What a good end of life means is a particularly relevant question in the context of confinement and prison. Most of the questions and issues raised by end of life for those living in liberty also apply to the correctional setting. However, the institutional particularities and logics of the prison create unique barriers and make it difficult in practice to reconcile concerns in regard to end of life-like care and comfort-with the mandate of corrections-confinement and punishment. At present, the literature on end of life in prison is dominated by U.S. contributions. We have therefore invited r

In France, nurses practising in the prison environment work in a health care unit, for somatic care, or in a regional medical-psychological unit for large facilities and psychological care. These units belong to the regional hospitals. Located at the heart of the prison, they cater for prisoner-patients. On the frontline, the nurse has specific autonomy and responsibility in this unique context.

Due to the growing number of ageing prisoners in the American correctional system, penal institutions are increasingly caring for patients with chronic and potentially terminal medical conditions. To address this problem states have attempted sentencing reform initiatives and adopted compassionate release programmes; however, these efforts have failed to significantly reduce the number of elderly or seriously ill inmates. Correctional mental health services are now called upon to aid in the care of prisoners at the end of life. This article presents the common elements of prison hospice progra…
