Patient and Family Perspectives on Statutory Duty of Candour Following Serious Adverse Patient Safety Events.

BACKGROUND: The Statutory Duty of Candour (SDC) legally requires healthcare organisations to notify patients, families, and carers who experience a serious adverse patient safety event (SAPSE), provide an apology, and explain steps taken to prevent recurrence. However, despite growing international adoption of SDC, evidence on its implementation and impact on patients and families ('consumers') remains limited. This study examined consumer awareness, understanding, and lived experiences of SDC in Victoria, Australia, two years after its introduction. METHODS: A mixed-methods study integrated

Exploring the "shared" in shared decision-making in the care for children with chronic diseases or disabilities: what are the roles of parents and professionals?

There is a growing body of literature that recognizes the importance of shared decision-making (SDM) in the care for children with chronic conditions and/or disabilities. Although participation in SDM can be more or less active, the tuning between parents and professionals about the way they want to participate in SDM is often an implicit process, limiting parents' optimal involvement. Role definitions may support both partners in the process of SDM. We conducted a scoping review to investigate the available knowledge on the interpretation and variability of different roles of parents and prof

New and Evolving Dimensions in the Pediatrician-Family-Patient Relationship: Maintaining and Managing Boundaries: Clinical Report.

Maintaining and managing boundaries in relationships with patients and their family members remains extremely important for the practicing pediatrician. Technological innovations that facilitate communication and information sharing, such as social media, have created new dimensions within the pediatrician-family-patient relationship. Shifts in patient and family expectations of the medical profession that have occurred over time, such as a greater desire among patients and families to have increased access to their pediatrician, also present novel challenges to traditional professional bounda

Establishing an optimal working relationship with patients with an antisocial personality disorder. Aspects and processes in the therapeutic alliance.

OBJECTIVE: Developing good interpersonal relationships is one of the main impediments for people with an antisocial personality disorder (ASPD). However, in treatment of psychiatric disorders, establishing a strong therapeutic alliance (TA) is important for effective treatment. Nevertheless, there is little knowledge on how to establish this TA with this challenging patient group. This study investigates which factors are important in TA development. METHOD: For this study, a qualitative research methodology is applied. In-depth interviews with therapists experienced in treating ASPD were con

Insights Into Working Alliance: Predictors of Patient-Perceived Bond With Genetic Counselors Among a Diverse, Multispecialty Sample.

The Reciprocal Engagement Model of genetic counseling practice centers the patient-provider relationship as an essential component of practice, but few quantitative studies have assessed what influences this relationship in actual genetic counseling practice. Our study explored which factors are most strongly associated with the patient-perceived bond subscale of the Working Alliance Inventory in a variety of genetic counseling sessions using a diverse patient sample across multiple clinical specialties. There were 416 participants (50.4% non-Hispanic white) seen by 41 genetic counselors (95.1

Communication Strategies to Improve Child Participation in Triadic Consultations Within Healthcare Settings: A Scoping Review.

INTRODUCTION: Children under 18 are recognised as key stakeholders in healthcare and possess the right to actively participate in decisions regarding their care. Despite this, they are frequently relegated to passive roles during triadic healthcare consultations involving healthcare providers (HCPs), caregivers, and children. Enhancing child participation requires effective communication strategies; however, numerous barriers persist, including healthcare provider dominance and parental interference. This review aims to explore and identify effective communication strategies employed by health

An interactive view of countertransference: Differentiation of self and client presentation.

Using family systems theory (Kerr & Bowen, 1988), interpersonal theory (Kiesler, 1996; Leary, 1957), and the interactional theory of countertransference (CT; Gelso & Hayes, 2007) as a framework, we investigated the influences of therapist personal factors (differentiation of self in relation to the family of origin; DoS) and client presentation as predictors of CT feelings and behaviors in response to a video analog client. Therapist-participants viewed either an interpersonally hostile-submissive or interpersonally hostile-dominant (HD) client. Overall, therapists lower in DoS reported signif

The role of emotion in patient safety: Are we brave enough to scratch beneath the surface?

Healthcare professionals work in emotionally charged settings; yet, little is known about the role of emotion in ensuring safe patient care. This article presents current knowledge in this field, drawing upon psychological approaches and evidence from clinical settings. We explore the emotions that health professionals experience in relation to making a medical error and describe the impact on healthcare professionals and on their professional and patient relationships. We also explore how positive and negative emotions can contribute to clinical decision making and affect responses to clinica

The curious physician: exploring the role of curiosity in professionalism, patient care, and well-being.

INTRODUCTION: Curiosity is a fundamental human trait that drives learning and exploration. However, research on curiosity has received little attention in the medical field, despite its potential to enhance knowledge acquisition, work performance, and psychosocial well-being. This study aimed to address part of this gap by investigating physicians' perspectives on their personal experiences with curiosity and its role in their professional practice and medical training. MATERIALS AND METHODS: This qualitative study was conducted with 12 physicians from the University of Heidelberg Medical Hos

Characteristics of therapeutic alliance in musculoskeletal physiotherapy and occupational therapy practice: a scoping review of the literature.

BACKGROUND: Most conventional treatment for musculoskeletal conditions continue to show moderate effects, prompting calls for ways to increase effectiveness, including drawing from strategies used across other health conditions. Therapeutic alliance refers to the relational processes at play in treatment which can act in combination or independently of specific interventions. Current evidence guiding the use of therapeutic alliance in health care arises largely from psychotherapy and medicine literature. The objective of this review was to map out the available literature on therapeutic allian

From isolation to connection: A nursing model for AI-enhanced mental health crisis management in rural Chinese adolescent communities.

This paper presents a conceptual nursing model that integrates artificial intelligence (AI) technologies to enhance mental health crisis management for rural Chinese adolescent communities. Addressing the critical gap between isolation and therapeutic connection, this nursing-led framework responds to escalating mental health crises among rural Chinese adolescents, characterized by elevated suicide rates and severely limited access to professional psychiatric nursing support. The model positions psychiatric nurses as central coordinators who utilize AI-enhanced tools for early crisis detection

Shared Vulnerability: Psychodynamic Reflections on Personal History and Clinical Practice in a Therapeutic Community.

Reflecting on early-career experiences working with patients with severe personality disorders in a therapeutic community, this article explores the counter-transference challenges faced by novice clinicians. I discuss how intense patient projections can activate unresolved conflicts within therapists, often influencing their clinical responses. Countertransference is examined as a co-created experience shaped by both therapist and patient dynamics. Emphasizing the importance of self-awareness, personal analysis, and supportive environments, this perspective offers insights to help early-caree

"What shall we focus on?" - A thematic analysis of what characterizes cognitive-behavior therapy sessions with high or low quality of working alliance.

OBJECTIVE: Several studies have shown that the quality of the working alliance predicts symptomatic improvement session-by-session, including in cognitive-behavioral therapy (CBT). We wanted to explore what characterizes CBT sessions with high and low alliances further using qualitative analysis. METHOD: Ten CBT-sessions were selected from eight patients' therapies in a larger research project on psychotherapy for patients with major depression. Five sessions were chosen from high- and five from low-alliance sessions, based on therapist- and patient-reported Working Alliance Inventory scores.

Countertransference responses mediate the relationship between patients' overall defense functioning and therapists' interventions.

Objective: The aim of this work was to investigate the relationship between therapists' techniques (supportive/expressive intervention level - ESIL), therapists' countertransference (CT) and patients' defense level of functioning (ODF). We hypothesized that CT could play a mediating role in the relationship between patients' ODF and therapists' ESIL. Method: 65 psychotherapy sessions were rated by two different raters each with the Psychodynamic Interventions Rating Scale and the Defense Mechanisms Rating Scale. Clinicians used the Therapist Response Questionnaire to provide information on the

Explanation as treatment for functional neurologic disorders.

There is widespread agreement that the way health professionals communicate the diagnosis of functional neurologic disorders (FND) has a central role in treatment, as it does arguably for most conditions. In this chapter we discuss barriers to effective diagnosis, different models of explanation and evidence regarding the importance of effective communication of the diagnosis in FND, especially movement disorders, and dissociative (nonepileptic) seizures. Debates and disagreements about how to go about this task often reflect different theoretic models held by health professionals rather than
