Progestin and breast cancer. The missing pieces of a puzzle.

The previous assumption that progestin does not promote breast cancer development needs to be re-examined since a growing body of evidence indicates the opposite. Data from recent experimental trials and results from clinical and epidemiological studies on hormonal contraceptives and hormone replacement therapy (HRT) have been confronted with breast cancer cases known from the German database of adverse drug reactions (ADR), reported in association with the use of progestin only contraceptives (POC) and combined oral contraceptives (COC). Also cases reported in association with HRT have been a

Cyclic progestin therapy for the management of mastopathy and mastodynia.

The management of benign diseases of the breast aims to halt the progression of fibrocystic transformation and to eliminate the symptoms of pain and breast tenderness. Progestins can be used for this purpose. In a controlled, randomized, double-blind, parallel-group study we treated 31 women with mastopathy/mastodynia with the progestins medrogestone (10 mg/day) or dydrogesterone (10 mg/day) from day 14 to day 25 for six cycles. Before, during and at the end of therapy the following parameters were evaluated: subjective symptoms (pain, tenderness, impairment of daily activities), palpatory fin…

The pharmacological profile of a novel norpregnance progestin (trimegestone).

Trimegestone is a novel norpregnane progestin that is being developed, in combination with estradiol, for the treatment of menopausal symptoms. The pharmacological characteristics of trimegestone have been evaluated in both in vitro and in vivo test systems, and compared with reference progestins. Interaction with hormonal steroid receptors from animal tissues and with human recombinant receptors in vitro has demonstrated that trimegestone has a high specificity and potency for the progesterone receptor, no affinity for the estrogen receptor, and weak affinity for androgen, glucocorticoid and …

[The effects of progestin on the bone metabolism in postmenopausal women].

Thirty postmenopausal women, including 26 with history of hysterectomy and bilateral salpingooophorectomy due to uterine myoma and 4 with natural menopause without any diseases were randomly allocated into two groups. Fifteen subjects received medroxyprogesterone acetate (MPA) 10 mg twice daily for three months, the other 15 received both MPA 10 mg twice daily and cyclopentylethinyl estriol (CEE3) 5 mg once a month for three months. Fasting urinary calcium/creatinine (Ca/Cr) and hydroxyproline/creatinine (OHpr/Cr) ratio, serum calcitonon (CT) and alkaline phosphatase (AKP) concentrations were

Progestin and antiprogestin effects on progesterone receptor transformation.

Transformation of the rabbit uterine progesterone receptor following binding to several synthetic steroids was studied. Cytosolic receptors … ion was faster in the presence of molybdate than in its absence. These data demonstrate that potent progestins (MPA and R5020) promote a greater amount of receptor transformation than does progesterone, and that steroids with little progestin bioactivity (DOC and 20 alpha OH-P) promote very little transformation. In addition, the antiprogestin activity of U- …

[Hepato-biliary complications of estro-progestin therapy].

A brief account of the most frequently observed side-effects of the protracted administration of oestroprogestinic substances is followed by a detailed explanation of the damage they may cause to the hepatobiliary apparatus: cholestatic icterus; cholesterolic biliary lithiasis; benign and malignant liver neoplasias. Reference is made to the liver diagnosis problems the practitioner may encounter when dealing with patients treated with these substances. Stress is laid on the need for close monitoring of tolerance and the possible effects of oestroprogestinic management on liver and bile functio

The progestin revolution 2: progestins are now a dominant player in the tight interlink between contraceptive protection and bleeding control-plus more.

The interlink between bleeding control and contraceptive development has always been an important factor. But after many years of advances i … .But even better, there is a new PLUS. The distinct benefit and risk profiles of the many different progestins now available are intentionally being used either in combination contraceptive pills [COCPs] or alone, to add additional benefits, to decrease side effects and risks, and increase effectiveness and bleeding control.

Progesterone, but not progesterone-independent activation of progestin receptors by a mating stimulus, rapidly decreases progestin receptor immunoreactivity in female rat brain.

Recent studies suggest that progestin receptors may be activated in vivo by neurotransmitters in the absence of ligand. More specifically, vaginal-cervical stimulation (VCS) can influence sexual behavior by activating progestin receptors in the absence of progesterone. Another way to test if progestin receptors are influenced by particular stimuli is to examine progestin receptor immunostaining. We report that progestin receptor immunoreactivity is decreased in the forebrain of estradiol-primed ovariectomized (OVX) rats within 1 h after a subcutaneous injection of progesterone, a time by which

Oral contraceptive progestins and angiotensin-dependent control of the renal circulation in humans.

Oral contraceptive (OC) use is associated with increased intrarenal renin-angiotensin-aldosterone system (RAA System) activity and risk of nephropathy, though the contribution of progestins contained in the OC in the regulation of angiotensin-dependent control of the renal circulation has not been elucidated. A total of 18 OC users (8 non-diabetic, 10 Type 1 diabetic) were studied in high salt balance, a state of maximal RAA System suppression. Progestational and androgenic activity of the progestin in each OC was standardized to that of the reference progestin norethindrone. Renal plasma flow…

GnRH mediates estrous behavior induced by ring A reduced progestins and vaginocervical stimulation.

The present study was designed to assess the participation of gonadotropin-releasing hormone (GnRH) in the display of estrous behavior induced by application of vaginal-cervical stimulation (VCS) and by the intracerebroventricular (icv) administration of progesterone and its ring A-reduced metabolites to ovariectomized (ovx), estradiol benzoate (E2B) primed rats. Icv injection of Antide, a GnRH-1 receptor antagonist, significantly depressed lordosis behavior in ovx, E2B-primed rats treated with icv GnRH. Application of VCS to ovx, E2B-primed rats facilitated both lordosis and proceptivity. The

Replacement of estrogens and progestins to prevent morbidity and mortality in preterm infants.

BACKGROUND: A potential therapeutic effect of sex steroids on premature infants has been proposed from animal data and observational studies in humans. Purported benefits include reduction in chronic lung disease, improved bone density and improved neurodevelopmental outcome. OBJECTIVES: To determine if estrogens or progestins, either alone or in combination, when compared to placebo or no treatment, reduce morbidity and/or mortality in preterm infants. SEARCH STRATEGY: The standard search strategy of the Cochrane Neonatal Review Group as outlined in the Cochrane Library (Issue 2, 2004) was …

Relative binding affinity of various progestins and antiprogestins to a rabbit myometrium receptor.

Relative binding affinity (RBA) of various progestins and antiprogestins to a cytoplasmic receptor prepared from the myometrium of estrogenized immature female rabbits was investigated. The cytosol was incubated with tritiated promegestone (3H-R5020) in the presence of various concentrations of non-radioactive promegestone (R5020) as standard on the one hand, and several progestins and antiprogestins on the other hand. The incubate was subjected to isoelectric focusing in slabs of polyacrylamide gel and the bound radioactivity in the peak of the receptor was measured, RBA was expressed as a pe…

Progestins as endocrine disrupters in aquatic ecosystems: Concentrations, effects and risk assessment.

In aquatic ecosystems, progesterone (P4) and synthetic progestins (gestagens) originate from excretion by humans and livestock. Synthetic progestins are used for contraception and as P4 for medical treatments as well. Despite significant use, their ecotoxicological implications are poorly understood. Only about 50% of the progestins in use have been analyzed for their environmental occurrence and effects in aquatic organisms. Here we critically summarize concentrations and effects of progestins in aquatic systems. P4 and progestins were mostly detected when analyzed for, and they occurred in t…

Histologic alterations in endometrial hyperplasia and well-differentiated carcinoma treated with progestins.

The treatment of complex atypical hyperplasia (CAH) and well-differentiated endometrioid carcinoma (WDC) by progestin therapy has been shown to be a safe alternative to hysterectomy. Accurate assessment for regressive changes induced by the progestins is critical to successful treatment. However, there are few studies detailing the histopathologic changes associated with progestin therapy. A total of 44 patients with CAH or WDC, treated with oral progestins or a progesterone or levonorgestrel-releasing intrauterine device, were followed by endometrial biopsy and/or curettage at 3 to 6 month in…

Androgenic progestins in oral contraceptives and the risk of epithelial ovarian cancer.

OBJECTIVE: Oral contraceptives (OCs) have been consistently linked to reduced risk of ovarian cancer. Oral contraceptive formulations display varying degrees of androgenicity. Data linking androgens to ovarian cancer suggest that OC androgenicity may impact efficacy in preventing ovarian cancer. The authors investigated whether OC efficacy might differ according to androgenicity by using data from a large, population-based, case-control study (the Steroid Hormones and Reproductions [SHARE] Study). METHODS: Detailed data on OC formulation was obtained by an in-person interview for 568 cases an
