Extremely Elevated Prostate-Specific Antigen in Acute Prostatitis: A Case Report.

Elevated prostate-specific antigen (PSA) levels are mostly suggestive of prostate cancer, but they are elevated in non-cancerous prostatic conditions as well. However, extreme levels of PSA as reported here have not been observed in cases other than prostatic cancer so far. Our patient had a significantly elevated PSA of 1,398 ng/mL in acute prostatitis. The purpose of this case report is to review the patient's atypical and rare presentation of extremely high PSA in acute prostatitis in the background of benign prostatic hyperplasia (BPH) and chronic prostatitis.

Management of elevated prostate-specific antigen in men with nonbacterial chronic prostatitis.

Elevated serum prostate-specific antigen (PSA) in the setting of acute or chronic bacterial prostatitis is common. Serum PSA, however, is much more variable in the setting of chronic nonbacterial prostatitis. Because elevated serum PSA is associated with prostate cancer and is used in screening programs for prostate cancer, patients with benign causes for elevation of serum PSA present a challenge. This article reviews the management of patients with elevated serum PSA and a diagnosis of chronic nonbacterial prostatitis.

High Detection Rates for Prostate-specific Membrane Antigen-avid Prostate Cancer Recurrence at Low Prostate-specific Antigen levels on Extended Axial Field-of-view Positron Emission Tomography/Computed Tomography.

BACKGROUND AND OBJECTIVE: Although prostate-specific membrane antigen (PSMA) positron emission tomography/computed tomography (PET/CT) has impacted the investigation and management of biochemical recurrence (BCR) of prostate cancer, negative scans are common at low rising prostate-specific antigen (PSA) levels. PET/CT devices with an extended axial field-of-view, such as the Siemens Biograph Vision Quadra (Quadra) scanner, have substantially higher sensitivity than conventional field-of-view scanners. Our aim was to assess whether the enhanced signal-to-noise ratios achieved on the Quadra scan…

Reduction of prostate-specific antigen after tamsulosin treatment in patients with elevated prostate-specific antigen and lower urinary tract symptoms associated with low incidence of prostate cancer at biopsy.

OBJECTIVES: To evaluate the effect of tamsulosin on reducing the serum levels of prostate-specific antigen (PSA) in patients with lower urinary tract symptoms and an elevated PSA level. METHODS: From June 2004 to July 2006, patients with lower urinary tract symptoms, a PSA level of >or=4 ng/mL, and a maximal flow rate of <15 mL/s received tamsulosin 0.4 mg daily for 2 months. They were then scheduled for 12-core prostate biopsy. PSA determination and a uroflow study were performed before biopsy. RESULTS: A total of 80 patients completed the present study. The mean patient age was 66.3 years,…

Clinical value of prostate specific antigen based parameters for the detection of prostate cancer on repeat biopsy: the usefulness of complexed prostate specific antigen adjusted for transition zone volume.

PURPOSE: To our knowledge the indications for repeat prostate needle biopsy in men whose previous transrectal ultrasound guided biopsy results revealed no evidence of cancer have not yet been defined. We identified the most effective method for detecting prostate cancer on repeat biopsy. MATERIALS AND METHODS: One or more systematic repeat prostate biopsies were performed in 144 consecutive patients, including 86 with prostate specific antigen (PSA) levels between 4 and 10 ng./ml. at repeat biopsy. Men in whom cancer was detected on repeat biopsies were compared with their counterparts in ter

Fluorometric nanoprobes for simultaneous aptamer-based detection of carcinoembryonic antigen and prostate specific antigen.

A "turn-on" fluorometric assay based on the combined effects of fluorescence resonance energy transfer (FRET) and internal filter effect (IFE) is described for the rapid and ultrasensitive detection of both carcinoembryonic antigen (CEA) and prostate specific antigen (PSA). Their unique porous structures and high specific surface enable mesoporous silica nanoparticles (MSNs) to load a large number of CdTe quantum dots (QDs). These amplify the fluorescence signal and provide a platform to fabricate more distinctly fluorescent MSNs (QD-MSNs). Two kinds of QD-MSNs with the maximum emission wavele

High-performance affinity peptide sensor for prostate specific antigen detection.

Prostate-specific antigen (PSA) is the best serum biomarker for prostate cancer (PCa), and the detection of PSA concentration can be used to assess the risk of malignancy. Given the current lack of effective treatment options for PCa, early detection and intervention are particularly important. To address this, we developed a novel electronic biosensor aimed at highly sensitive detection of PSA. The core materials of this sensor consist of the receptor material, PSA-affinity peptides, and the support material, single-walled carbon nanotubes. These materials are cost-effective, can operate at r…

Prostate-Specific Membrane Antigen PET/Computed Tomography: Pearls and Pitfalls.

Prostate-specific membrane antigen PET (PSMA-PET) has emerged as a powerful imaging tool for prostate cancer primary staging, biochemical recurrence, and advanced disease assessment. This article offers a concise overview of the benefits and challenges associated with PSMA-PET for prostate cancer evaluation. The article highlights the advantages of PSMA-PET over conventional imaging, such as its higher sensitivity and specificity for detecting metastases, and the potential for guiding personalized treatment decisions. However, it also explores the limitations and potential pitfalls for interpr

Increased Prostate-Specific Membrane Antigen Uptake in a Gallbladder Stone.

An Al 18F-prostate-specific membrane antigen (PSMA) Q PET/CT scan was performed in a 67-year-old man to identify any potential recurrent prostate cancer lesions, which revealed no recurrent or metastatic lesions. However, a large gallbladder stone with increased PSMA uptake was incidentally detected, which could be a potential pitfall in the interpretation of PSMA PET imaging.

Serum levels of prostate specific antigen are elevated after colonoscopy.

The risks of prostate cancer and colorectal carcinoma increase with age. So, colonoscopy and measurement of serum prostate specific antigen (PSA) may be performed during a short term in a given patient. We aimed to evaluate whether colonoscopy affects serum PSA levels and to evaluate the relationship between prostate volume and elevation in serum PSA levels after colonoscopy. This study included 44 consecutive male patients, who underwent colonoscopy. The mean age of the patients was 56.05±9.27 years. The mean time required for colonoscopy was 30 min. Serum PSA levels were measured 48-72 hours

Prostate specific antigen--a new constituent of breast cyst fluid.

We demonstrate for the first time that prostate specific antigen (PSA) is a component of breast cyst fluid. At the cutoff level of 0.01 or 0.03 micrograms/L of PSA, 64% or 43% of cyst fluids are positive for PSA, respectively. PSA in cyst fluid, as characterized by high performance liquid chromatography, exists in almost equal concentrations as free PSA, with a molecular weight of 33 KDa, and as PSA bound to alpha 1-antichymotrypsin, with a molecular weight of 100 KDa. PSA presence was also characterized in cyst fluid by Western blot analysis. These data suggest that PSA is a frequent componen

Assay for prostate specific antigen (PSA): problems and possible solutions.

The absolute tissue specificity of prostate specific antigen (PSA) allows the use of PSA test not only for detecting recurrence or metastasis at an early stage after radical prostatectomy but also for screening prostate cancer if combined with digital rectal examination. There is also a need to improve the current PSA test to better differentiate between prostate cancer and benign prostate hyperplasia (BPH). Because of these clinical applications, a much greater demand was placed on PSA test for extra sensitivity, accuracy, and precision even within the normal PSA concentration range. However,

Characterization and measurement of prostate-specific antigen using monoclonal antibodies.

Monoclonal and polyclonal antibodies were produced using a pure preparation of prostate-specific antigen (PSA) as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). By SDS-PAGE, the apparent molecular weight of PSA was about 33 kD. Eighty-four monoclonal antibodies were produced, 77 of which bind PSA with an affinity higher than 10(9) M-1. Use of these monoclonal antibodies to study the immunological characteristics of PSA revealed the presence of 4 epitopes. Injection of PSA into goats resulted in a production of polyclonal antibodies with high affinity. These…

Digital rectal examination-associated alterations in serum prostate-specific antigen.

The effect of digital rectal examination (DRE) on measurement of serum prostate-specific antigen was investigated during a prostate cancer screening program of 2,736 ambulatory men. Serum samples were collected before and after DRE and values compared using the nonparametric Wilcoxon signed rank test. Small, yet statistically significant, increases were found associated with DRE. The magnitude of these increases, however, was of minor clinical importance. Patients who exhibited the largest increases with initial values greater than the upper limit of normal (4 micrograms/L) were found to have …

Medically significant concentrations of prostate-specific antigen in serum assessed.

We used the method of Rudolph et al. (Clin Chem 1988; 34:2031-8) to find information in the data from correlated determinations of acid phosphatase (PAP, EC 3.1.3.2; DuPont aca) and prostate-specific antigen (PSA, Hybritech). We described there how we assign medical decision limits for two or more correlated variables and convert the database to a binary coded message, allowing separation of a selected disease class with minimum error. The decision point, analogous to a percentile upper limit on the ordered values of each variable in the reference group, satisfies the maximum entropy constrain…

[Effects of digital rectal examination on serum prostate specific antigen].

OBJECTIVE: Prostate specific antigen (PSA) level is determined under precise conditions for the detection and follow-up of cancer of the prostate. The question is raised as to whether digital rectal examination has an effect on serum levels and whether an interval of time is required before PSA assay can give reliable results. METHODS: A prospective protocol was conducted for 6 months. PSA was assayed in 56 patients hospitalized in an internal medicine ward whose clinical status justified digital rectal examination. Blood samples were taken 2 to 3 hours before the digital rectal examination,
