Implant-retained auricular prostheses: a clinical challenge.

Microtia, malformation, deformity, and partial or complete loss of the pinna may be due to various congenital or acquired factors. In adult patients, surgical reconstruction of the missing pinna is difficult and the results are often far from satisfactory. An implant-retained auricular prosthesis is a suitable alternative. A retrospective study of eight patients treated with implant-retained auricular prostheses was performed. For each missing pinna, three titanium implants were placed in the temporal bone. After 6 months of osseointegration, the implants were loaded. Four cases were rehabilit

Rehabilitation of a mid-facial defect using maxillary obturator with a maxillary expansion device and orbital prosthesis.

Midfacial defects are defined as congenital or acquired defects in the horizontal plane at the middle third of the face and communicate with intraoral maxillary defects. These defects lead to speaking difficulty, difficulty in saliva control and deglutition, mastication, and esthetics. Prosthetic rehabilitation of such defects with maxillofacial prosthesis is a challenging task. Maxillary defects with bilateral undercuts present are common. This case report explains to achieve retention by engaging the bilateral undercuts with the desired path of insertion and obtaining adequate retention of t

Fabrication of an overdenture covering a torus palatinus using a combination of denture base materials: a case report.

A maxillary torus provides a challenge when restoring an edentulous arch. Tori tend to have very thin mucosa and can be intolerant of normal pressures from a denture base. The tori tend to have large undercuts and at times extend to the junction with the soft palate, preventing the creation of a good palatal seal. Triple lamination involves combining 3 materials to fabricate a denture base that utilizes the retentive aspects of the torus. The thermoplastic flange provides a measure of flexibility, allowing the denture to seat over the torus. The resilient liner provides a cushion to diffuse th

Obturators: A proposed classification and its associated techniques.

STATEMENT OF PROBLEM: Multiple classifications exist for maxillectomy defects. However, none of the existing classifications describes the defects as favorable or unfavorable from the prosthodontist's standpoint. The most common problem with prosthetic treatment in such patients is in getting adequate retention, stability, and support. The size and location of the defect usually influence the amount of impairment and difficulty in prosthetic rehabilitation. PROPOSED CLASSIFICATION: A series of cases has been studied, and a newer type of maxillary defect is seen with a better presurgical invol

Prosthetic rehabilitation of 2 missing digits using a customized ring attachment-supported device: A detailed case report.

Traumatic finger amputations are common upper-limb injuries, often resulting from accidents involving machinery, workplace incidents, motor vehicle collisions, or sports-related injuries. Epidemiological studies suggest that traumatic finger amputations constitute 3% of emergencies treated in hand clinics, with 40% of cases resulting from workplace accidents. These injuries can range in severity, sometimes requiring partial or full amputation of the affected digits. The loss of 1 or more fingers has profound functional and psychological impacts on patients, affecting hand dexterity, grip stren

A single unit digital flexor tendon prosthesis.

Historically, the surgical repair of serious injuries to the flexor tendons of the hand have met with less than satisfactory clinical results. In an attempt to solve this problem, studies have been made on the design and testing of a Dacron polyester tendon prosthesis for complete replacement of the digital flexor tendon. The prosthesis attaches proximally at the anatomical musculotendon junction via collagen ingrowth into a fabric structure and in similar manner distally via bone ingrowth at the anatomical insertion site. These fabrics are continuous with the tendon body of the prosthesis con

Fabrication of a functional palatal saliva reservoir by using a resilient liner during processing of a complete denture.

One of the major problems associated with xerostomic patients is the poor tolerance and retention of removable dental prostheses because of thin dry atrophic mucosa and lack of a saliva film. The use of salivary substitutes can improve lubrication, provide irrigation for dry mucosa, provide significant relief from symptoms, and also improve the retention of removable prostheses. These substitutes can be delivered over prolonged periods by using saliva reservoirs in the prosthesis itself. This report describes a novel technique for the fabrication of a functional palatal saliva reservoir in ede

Clinical and Patient Satisfaction Outcomes from Southern Osseointegrated Fixtures for Craniofacial Reconstruction: A Retrospective Study.

This study reports the clinical outcomes from using Southern osseointegrated fixtures (SOFs) for retention of auricular, nasal, and orbital silicone prosthetics. Osseointegrated implants are widely used in the craniofacial skeleton, but data specific to the use of SOFs are more limited. A retrospective review was conducted of 16 patients who underwent implantation of SOFs between 2013 and 2024 at a large London teaching hospital. Data were collected from Electronic Patient Records and post-implantation surveys to assess patient satisfaction, prosthesis use, and complication rates. Descriptive

Split-Skin-Graft Wrapped Conformer to Treat Severe Contracted Sockets.

This case series present the surgical management of cases with severe contracted sockets using a split-skin-graft wrapped conformer. Eight patients with severely contracted anophthalmic sockets underwent surgery. The surgical technique is described and a chart review was performed to collect patient data pre- and postoperatively. Preoperatively, all patients had grade 3 or 4 contracted sockets with shallow or obliterated fornices and patients were unable to retain an external prosthesis in place. Donor material was harvested from the supraclavicular area. The split-skin-graft was successfully

Quality of life and psychological evaluation of patients after anaplastology.

A comprehensive questionnaire with 43 questions was designed to evaluate quality of life, based on rehabilitation with a facial prosthesis. Each patient's psychological situation was assessed using the validated questionnaire and associated scales. Different patient groups were compared with each other in terms of questionnaire scores and general data. In total, 76 patients with a prosthesis of the orbit, nose, or ear, or a combination thereof, were included. There was a highly significant difference in overall satisfaction with defect reconstruction via a prosthesis of the ear compared with t

Tooth-Supported O-Ball Retained Maxillary Overdenture in a Patient Contraindicated for Dental Implants Due to Bisphosphonate Therapy.

Tooth-supported overdentures offer an effective and conservative option for patients with partial edentulism and systemic contraindications to implant therapy. Retaining natural roots under the denture base helps preserve alveolar bone, enhances proprioception, and improves prosthesis stability. This case report describes the rehabilitation of a 76-year-old man with osteoporosis on long-term bisphosphonate therapy, type 2 diabetes, and hypertension. Due to the high risk of medication-related osteonecrosis of the jaw (MRONJ), implant placement was contraindicated. A maxillary overdenture retain

Implant-supported double-bar overdenture to reestablish lip support in an atrophic maxilla: a case report.

Prosthodontic treatment for edentulous maxillae with severe bone loss is a challenge to clinicians. Although implant-supported, fixed maxillary complete dentures are increasingly common, they are not always able to provide adequate lip support without a buccal overextension in patients with bone loss. The overextended buccal flange makes it difficult for patients to clean under the prosthesis. This case report describes rehabilitation of an atrophic maxilla through an implant-supported double-bar overdenture designed to resolve difficulties in oral hygiene maintenance that were caused by the b

Displacement of Zygomatic Implant in Infratemporal Region in a Case of COVID-19-Associated Mucormycosis.

Reconstruction of maxillary defects can be carried out using a variety of treatment modalities. The choice of modality depends on numerous factors, including the size and extent of the defect, patient characteristics, and existing comorbidities. One of the various options is reconstruction and rehabilitation using a fixed prosthesis supported over a zygomatic implant. In maxillectomy cases, a zygomatic implant can be used to improve the retention of the obturator prosthesis, as the loss of alveolar bone makes it impossible to use conventional endosseous implants. Although zygomatic implants ar

Prosthetic rehabilitation of acquired maxillary defects secondary to mucormycosis: clinical cases.

Maxillary necrosis can occur due to bacterial infections such as osteomyelitis, viral infections, such as herpes zoster or fungal infections, such as mucormycosis, aspergillosis etc. Mucormycosis is an opportunistic fungal infection, which mainly infects immunocompromised patients. Once the maxilla is involved, surgical resection and debridement of the necrosed areas can result in extensive maxillary defects. The clinician is to face many a challenge in order to replace not only the missing teeth, but also the lost soft tissues and bone, including hard palate and alveolar ridges. The prosthesi
