We present a rare clinical case of a 90-year-old female who sustained a proximal femoral neck fracture following long-standing hip arthrodesis. Since the fracture occurred relatively proximally and involved the pelvis, double-plate fixation was chosen to achieve rigid fixation. The reconstruction plate was placed at the posterior and anterior columns individually through single vertical incision. She was treated successfully, and she attained preinjury activity level. Proximal femoral fractures in arthrodesed hips need to be recognized as a fracture between the pelvis and femur. Rotational str…

BACKGROUND: Whether reducing time-to-surgery for elderly patients suffering from hip fracture results in better outcomes remains subject to controversial debates.
METHODS: As part of a prospective observational study conducted between January 2002 and September 2003 on hip-fracture patients from 268 acute-care hospitals all over Germany, we investigated the relationship of time-to-surgery with frequency of post-operative complications and one-year mortality in elderly patients (age > or =65) with isolated proximal femoral fracture (femoral neck fracture or pertrochanteric femoral fracture). INTRODUCTION: Dynamic hip screw (DHS) and proximal femoral nail (PFN) are both used to fix extracapsular proximal femoral fractures, with implant selection in routine practice typically guided by fracture morphology. This study describes perioperative outcomes and the pattern of implant selection in older adults treated at a single UK trauma centre.
MATERIALS AND METHODS: This single-centre retrospective descriptive study included all eligible patients aged 65 years or older undergoing fixation of extracapsular proximal femoral fractures during the 12-month study period (1 January to 31 Decem…

INTRODUCTION: Intertrochanteric (IT) fractures require stable fixation and early mobilization. This study evaluated the functional and radiological outcomes of proximal femoral nailing (PFN) in adults with IT fractures.
MATERIALS AND METHODS: A prospective observational study was conducted over 18 months in 48 patients aged 18-80 years with radiologically confirmed IT fractures treated with PFN. Functional outcome was assessed using the Modified Harris Hip Score (HHS) at 3 and 6 months. Radiological outcome was assessed using the radiographic union score for hip (RUSH), fracture alignment, an

INTRODUCTION: A high mortality rate is associated with hip fractures in the elderly. This is because their bones are osteoporotic with implants having less hold and there are more co-morbidities associated with the elderly. Osteosynthesis with the proximal femoral nail (PFN) features the advantages of high rotational stability of the head-neck fragment. However, the use of the nail is technically ambitious and is accompanied by some risks of error, which can lead to failure. This study aims to understand the technical difficulties related to PFN and methods to mitigate them and radiological in

BACKGROUND AND AIMS: Cardiovascular complications are common in hip fracture patients but the role of fracture type and operative method in these is unclear. This prospective cohort study aimed to evaluate the impact of fracture- and operative characteristics on perioperative cardiovascular complications and prognosis in unselected hip fracture patients.
MATERIAL AND METHODS: During a 7-month trial registration period, a population of 197 consecutive hip fracture patients (median age 84 years) diagnosed with femoral neck or pertrochanteric fracture was formed. The exclusion criteria were pati

BACKGROUND: Proximal femoral fracture (PFF), such as intertrochanteric femoral fracture or femur neck fracture, and its management are crucial issues to surgeons. PFF has been dramatically is becoming exponentially prevalent, and it is at high risk of complication and mortality because it is frequently associated with serious trauma and advanced age, especially in patients treated with anticoagulants or antiplatelet agents. Surgical management is essential for the treatment of PFF. Unfortunately, current surgical procedures have been related to accompanied by vascular complications, including

Peritrochanteric fractures are the most common fragility fractures for which patient are admitted in hospital and often require surgical interventions. With increasing life expectancy and early age of presentation, revision surgeries are increasing due to re-trauma, implant failure, infections etc. Here we present the case of a 65 years female, with inter trochanteric fracture femur right side with ipsilateral malunited proximal femur fracture, which was managed with customised proximal femoral nail. This case exemplifies the need for novel techniques and implants in our armamentarium to deal

CASE: We report on a 22-year-old patient with a comminuted femoral neck fracture because of a low-velocity gunshot wound. Treatment consisted of reduction and internal fixation with a fixed angled blade plate and immediate valgus osteotomy. He went on to uneventful healing and an excellent clinical result.
CONCLUSIONS: A comminuted gunshot femoral neck fracture in a young patient is a rare and potentially devastating injury. If stable, and there are no other associated emergent injuries, reduction, internal fixation, and immediate valgus osteotomy should be considered.

INTRODUCTION: Trochanteric fractures represent a substantial proportion of hip fractures in the elderly and are associated with significant morbidity and mortality. Dynamic hip screw fixation has shown higher failure rates in unstable fractures, leading to the use of intramedullary implants such as the proximal femoral nail (PFN) and the Halifax (Gamma) nail.
METHODS: A cross-sectional study was conducted on 134 patients aged 60-80 years with unstable trochanteric fractures admitted to the Trauma Center of the Institute of Medical Sciences, Banaras Hindu University, Varanasi, Uttar Pradesh, I

BACKGROUND: Proximal femoral nails (PFNs) are the most common method for the treatment of unstable intertrochanteric femoral fractures (IFFs), but postoperative bed rest is required. There is a large amount of blood loss during the operation. Osteoporosis in elderly patients may cause nonunion of fractures and other complications. Arthroplasty can give patients early weight bearing and reduce financial burden, but whether it can replace PFNs remains controversial.
AIM: To compare the clinical outcomes of arthroplasty and PFNs in the treatment of unstable IFFs in elderly patients.
METHODS: A

The unreamed femoral nail system (UFN) features highly versatile proximal interlocking options for the treatment of a wide range of femoral fracture patterns and combinations. Besides standard interlocking modes for diaphyseal fractures, special interlocking options for subtrochanteric fractures and ipsilateral neck/shaft fractures are available. Two aiming arms can be attached to the insertion handle of the nail in a modular fashion. The standard aiming arm is designed for transverse static or dynamic locking. The special aiming arm permits spiral blade, 130 degrees, and miss-a-nail interlock

Periprosthetic proximal femoral fractures (PPFFs) represent an increasingly prevalent complication following hip arthroplasty, driven by global demographic shifts and rising arthroplasty rates. These fractures pose significant clinical and surgical challenges due to their association with implant instability, compromised bone quality, and increased patient morbidity and mortality. The Vancouver classification system remains the cornerstone for fracture classification and management decision-making, distinguishing fractures by location, implant stability, and bone stock quality. This review out…
