Characteristics of smokers with a psychotic disorder and implications for smoking interventions.

Despite high rates of smoking among people with psychotic disorders, and the associated health and financial burden, few studies have investigated the characteristics of this group of smokers. This paper reports data from 298 smokers with an ICD-10 psychotic disorder residing in the community (56.7% with schizophrenia or schizoaffective disorder), including an examination of their demographic and clinical characteristics, smoking behaviours, severity of nicotine dependence, stage of change, and reasons for smoking and for quitting. Standardized self-report instruments were used, in conjunction

Analysis of TBX1 variation in patients with psychotic and affective disorders.

A significant portion of patients with 22q11 deletion syndrome (22q11DS) develop psychiatric disorders, including schizophrenia and other psychotic and affective symptoms, and the responsible gene/s are assumed to also play a significant role in the etiology of nonsyndromic psychiatric disease. The most common psychiatric diagnosis among patients with 22q11DS is schizophrenia, thought to result from neurotransmitter imbalances and also from disturbed brain development. Several genes in the 22q11 region with known or suspected roles in neurotransmitter metabolism have been analyzed in patients

Psychotic symptoms in combat-related posttraumatic stress disorder.

BACKGROUND: Posttraumatic stress disorder (PTSD) is known often to be comorbid with other anxiety, mood, and substance use disorders. Psychotic symptoms have also been noted in PTSD and have been reported to be more common in Hispanic veterans. However, the occurrence of psychotic symptoms, including the degree to which they are accounted for by comorbid disorders, have received limited systematic investigation. Our study objectives were to assess psychotic symptoms according to DSM-III-R criteria in patients with a primary diagnosis of combat-related PTSD and determine the associations of tho

[Value of clozapine in treatment of psychotic disorder in Parkinson disease].

Psychosis secondary to dopaminergic therapy can limit the ability to manage motor symptoms of advanced Parkinson's disease (PD). Scholz and Dichgans (1985) were the first to report the use of clozapine in drug-induced psychosis in PD. The rationale for use of clozapine in parkinsonian patients is supported by his original pharmacological profile with weak extra-pyramidal side effects. A Medline search was performed of literature from 1985 to 1994. The literature search was not limited to the English language. Numerous authors (23 articles) using case reports or open trials among more than 100

Obsessive-compulsive disorder with psychotic features: a phenomenologic analysis.

The authors review the literature on obsessive-compulsive disorder and present clinical vignettes to illustrate that delusions can arise in the course of this illness. These delusions do not signify a schizophrenic diagnosis but represent reactive affective or paranoid psychoses, which are generally transient. Using a phenomenologic analysis of 23 patients, the authors further argue that obsessive-compulsive disorder represents a psychopathological spectrum varying along a continuum of insight. Patients at the severe end of this spectrum are best described as having an "obsessive-compulsive ps

[Cognitive functioning in psychotic spectrum disorders: current status and clinical implications].

BACKGROUND: Although cognitive impairments occur in approximately 70% of individuals with a psychotic spectrum disorder (PSS) and have a substantial impact on daily functioning, they are often underrecognized in clinical practice. Due to their limited specificity, they are not included in the DSM classification criteria. Historically, cognitive deficits were already central to the concept of dementia praecox. AIM: To provide an overview of the literature on cognition in psychosis. METHOD: This article summarizes the existing literature on cognition in PSS. RESULTS: Patients with PSS general

Investigating neural impairments in psychotic disorders using electroencephalography and cortical spheroids.

Synaptic dysfunction is a candidate mechanism in psychotic disorders, yet the precise underlying substrates remain elusive. We investigated how combining in vivo electroencephalography (EEG) and in vitro human cortical spheroid (hCS)-based methods can further our understanding of psychosis pathophysiology during fetal stages of neurodevelopment. Ten individuals with schizophrenia (SZ) or bipolar disorder (BD; 5 males and 5 females) and five controls (CTRL; 3 males and 2 females) underwent EEG assessments, including long-term potentiation (LTP)-like cortical plasticity and mismatch negativity (

Cognitive decline in psychotic disorders: A longitudinal matched case-control study.

BACKGROUND: Patients with psychotic disorders often exhibit premorbid cognitive deficits and future cognitive changes following the onset of illness. This study examined cognitive trajectories in schizophrenia (SCZ), schizoaffective disorder (SZA) and bipolar disorder (BD) compared with healthy matched controls. METHOD: Cases included individuals with SCZ (n = 24), SZA (n = 15), or BD (n = 11), and 50 controls matched for age, sex, and socioeconomic status (SES). Participants underwent IQ testing at age 17 as part of mandatory military screening (T1); the same tests were administered after il

Dopamine and Mood in Psychotic Disorders: An 18F-DOPA PET Study.

IMPORTANCE: There is limited neurobiological or trial evidence guiding treatment of comorbid affective syndromes in psychotic disorders. Given the use of dopamine-blocking antipsychotics, understanding dopamine function across these mood states is warranted. OBJECTIVE: To test for differences in dopamine synthesis capacity (Kicer) between affective syndromes across psychotic disorders and for association with psychotic symptom severity. DESIGN, SETTING, AND PARTICIPANTS: In this cross-sectional study using fluorine F 18-labeled fluorodopa (18F-DOPA) positron emission tomography (PET), indivi

Childhood emotional and physical bullying in affective and nonaffective psychotic disorders.

OBJECTIVE: Bullying is common in childhood. Identifying developmental periods most sensitive to bullying may improve understanding of the link between bullying and psychotic disorders. METHOD: Forty-six individuals with schizophrenia spectrum disorders (schizophrenia or schizoaffective disorder; SZ), 53 with psychotic bipolar disorder (BP), and 51 healthy control (HC) participants completed the Maltreatment and Abuse Chronology of Exposures questionnaire. We tested for differences in the rates and severity of emotional and physical bullying across diagnostic groups, and also calculated odds r

Second Opinions for Diagnoses of Psychotic Disorders: Delivering Stage-Specific Recommendations.

OBJECTIVE: The impact of obtaining second-opinion consultations on diagnoses of schizophrenia spectrum disorders was evaluated. METHODS: A retrospective chart review was conducted for 177 patients referred to a psychosis consultation service at an academic medical center from January 1, 2017, to October 1, 2023; these consultations aimed to clarify a diagnosis of psychosis. Diagnoses made before and after consultations were compared, and treatment recommendations resulting from the consultation visit were summarized. RESULTS: Among patients without a preconsultation diagnosis of schizophreni

Viewer and object mental rotation in young adults with psychotic disorders.

Schizophrenia patients have difficulty with processing visuo-spatial information, which may explain their deficits with considering other people's point-of-view. Processing visuo-spatial information operates on egocentric and allocentric frames of reference. Here, we tested the ability of individuals at different stages of psychotic disorders, specifically ultra-high-risk for psychosis individuals, as well as first-episode psychosis, and chronic schizophrenia patients, to perform a viewer mental rotation task and an object mental rotation task. The two tasks were differentiated only by the ins

Prevalence of hepatitis C in patients with non-affective psychotic disorders.

INTRODUCTION: the goal of this study was to determine the prevalence of hepatitis C virus (HCV) infection in patients with non-affective psychotic disorders and to compare it with population-based data. MATERIAL AND METHODS: an observational study was performed that measured anti-HCV antibodies (HCV-RNA in case of seropositivity) in 425 serum samples from patients with non-affective psychosis. Eight patients were positive for anti-HCV (1.9 %) and five had detectable HCV-RNA (1.2 %). The prevalence of viremia was significantly higher than in the general population (OR: 5.4; 95 % CI: 1.9-14.6).

Exploring and assessing demoralization in patients with non-psychotic affective disorders.

BACKGROUND: Demoralization, as assessed through the Diagnostic Criteria for Psychosomatic Research-Demoralization (DCPR/D) interview or the Demoralization Scale (DS), has been found to affect about 30% of patients with medical disorders, while few studies have been done in patients with psychiatric disorders. METHODS: A convenience sample of 377 patients with ICD-10 diagnoses of mood, anxiety, stress-related disorders or other non-psychotic disorders was recruited from two Italian university psychiatry centers. The DCPR/D interview and the Italian version of the DS (DS-IT) were used to assess

Correlates of vitamin D in psychotic disorders: A comprehensive systematic review.

People with psychosis have high prevalence of low vitamin D levels but the correlates and relevance of this deficiency are unclear. A systematic search of major databases from inception to 03/2016 was undertaken investigating correlates of vitamin D in people with psychosis. Data was summarised with a best evidence synthesis. Across 23 included studies (n=1770 psychosis, n=8171 controls) a mean difference in vitamin D levels between both groups of -11.14ng/ml±0.59 was found. 53 unique correlations between vitamin D and outcomes in people with psychosis were identified. The evidence base was br

[Changes to Schizophrenia Spectrum and other psychotic disorders in DSM-5].

This article provides an overview of the main changes in the chapter "Schizophrenia Spectrum and Other Psychotic Disorders" from DSM-IV-TR to DSM-5, which, once again, does not make allowance for potential characteristics of children and adolescents. Changes in the main text include abandoning the classical subtypes of Schizophrenia as well as of the special significance of Schneider's first-rank symptoms, resulting in the general requirement of two key features (one having to be a positive symptom) in the definition of Schizophrenia and the allowance for bizarre contents in Delusional Disorde

Use of synthetic cannabinoids in patients with psychotic disorders: case series.

An increasing number of synthetic cannabinoids have become available on the black market in recent years, and health professionals have seen a corresponding increase in use of these compounds among patients with psychiatric disorders. Unfortunately, there is almost no research available in the literature on this topic, and what little exists is based on case reports of individuals without psychiatric disorders. Synthetic cannabinoids are functionally similar to, but structurally different from, delta-9-tetrahydrocannabinol, the active principle in cannabis, and are problematic for many reasons
