Robotic Puboprostatic Fistula Repair with Holmium Laser Pubic Debridement.

INTRODUCTION AND OBJECTIVE: Urosymphyseal fistula (UF) with osteomyelitis most commonly occurs as a result of prostate cancer and benign pro … of the pelvis to characterize the pubis. Specific indications for holmium laser debridement of the pubic bone include: 1) history of sacral insufficiency fractures which eliminate management with partial pubectomy due to risk of pelvic ring instability and 2) mild osteomyelitis which can be managed with debridement. The patient is placed in dorsal lithotomy …

Should discontinuity of the osteotomy site 1 year after periacetabular osteotomy be diagnosed as delayed union and not non-union?

BACKGROUND: Discontinuity at the site of osteotomy is a complication after periacetabular osteotomy. The objectives of this study were to assess whether discontinuity is owed to delayed union or non-union and to clarify the risk factors associated with discontinuity >1 year after periacetabular osteotomy. METHODS: We performed a retrospective review of 104 hips in 95 consecutive patients who underwent periacetabular osteotomy between 2017 and 2021. Pre- and post-periacetabular osteotomy radiographs of 97 hips in 89 patients were finally evaluated. Radiographic evaluations included the occurre

[Congenital prepubic sinus: blind epispadial duplication of the urethra].

Presentation of one case of congenital epispadial urethral duplication in a 1-year old child. The malformation consisted on one normal urethra and the other one a prepubic epispadial accessory urethra, with the proximal end finishing blind into the anterior side of the bladder. The mictional cystography performed showed normal bladder and urethra, while the retrograde urethrography through the dorsal sinus showed a 3 cm long tract with no communication to the urethra or the bladder. Treatment consisted in total exeresis of the accessory urethra by way of suprapubic approach, with no symphysiot

Pubic Osteomyelitis Identified on Follow-Up MRI Following Adductor Myositis in a Patient With Escherichia coli Bacteremia.

A previously healthy 54-year-old man presented with a high-grade fever and bilateral groin pain. Initial non-contrast imaging failed to identify an infectious focus, while blood cultures revealed the presence of Escherichia coli. Pelvic MRI on hospital day 5 demonstrated bilateral adductor brevis myositis without osseous involvement. Despite antimicrobial therapy, symptoms persisted. Follow-up MRI on hospital day 13 revealed new pubic bone marrow edema, confirming a diagnosis of pubic osteomyelitis. The patient was successfully treated with a 12-week course of antimicrobial therapy, resulting …

Evaluating maternal recovery from labor and delivery: bone and levator ani injuries.

OBJECTIVE: We sought to describe occurrence, recovery, and consequences of musculoskeletal (MSK) injuries in women at risk for childbirth-re … weeks and 8 months' postpartum. We categorized magnitude of injury by extent of bone marrow edema, pubic bone fracture, LA muscle edema, and LA muscle tear. We also measured the force of LA muscle contraction, urethral pressure, pelvic organ prolapse, and incontinence. RESULTS: In this higher-risk sample, 66% (39/59) had pubic bone marrow edema, 29% (17/59 …

[Pubic osteitis caused by Salmonella indiana. Apropos of a case].

INTRODUCTION: The authors report the case of a pubic osteomyelitis caused by a salmonella indiana bacterium in a 12 year old child. CLINICAL CASE: A post traumatic left side pubic pain in a 12 year old child was associated to feverish and inflammatory syndrome, without any predisposing factor. Salmonella Indiana bacterium was found in fecal samples. Radiographs showed an asymetry of the iliopubic branches and an irregularity of the upper left border of the pubic branch. Bone scan showed a public hyperfixation and CT revealed a left-sided pubic osteolysis. The bacteriological examination of th

Dynamic ultrasound guided assessment of pelvic ring injury measurement (PRIME) for traumatic symphysis diastasis.

BACKGROUND: Assessment of pelvic ring injuries in the pre-clinical and trauma bay setting represents a challenge for the treating trauma team. The objective of the present project was to conduct a pre-clinical trial to investigate the feasibility and accuracy of ultrasound (US) guided assessment of symphyseal diastasis, of cadavers with pelvic ring injuries. METHODS: This is a prospective, anatomical, interventional and radiological cadaveric laboratory investigation. Cadavers were prepared with a pelvic ring injury (symphyseal diastasis). Eleven trauma surgeons performed an ultrasound-guided

Chronic Anterior Pelvic Instability: Diagnosis and Management.

Chronic anterior pelvic ring instability can cause pain and disability. Pain typically is localized to the suprapubic area or inner thigh; often is associated with lower back or buttock pain; and may be exacerbated by activity, direct impact, or pelvic ring compression. Known etiologies of chronic anterior pelvic ring instability include pregnancy, parturition, trauma, insufficiency fractures, athletics, prior surgery, and osteitis pubis. Diagnosis often is delayed. Physical examination may reveal an antalgic or waddling gait, tenderness over the pubic bones or symphysis pubis, and pain with p…

Penile anatomy under the pubic arch: reconstructive implications.

PURPOSE: We have previously defined the anatomy of the neurovascular bundle in the normal and hypospadiac penis. These studies were based on analysis of the fetal penis distal to the pubic arch without total inclusion of the crural bodies. To our knowledge the neuroanatomy beneath the pubic arch has not been well described. We defined the nerve distribution under the pubic arch and the relationship of the nerves to the crural bodies, corporeal bodies and urethra of the penis. MATERIALS AND METHODS: Eight normal human fetal penile specimens (at 17.5 to 29 weeks of gestation and 1 hypospadiac s

Safety Precautions for the Corona Mortis using Minimally Invasive Ilioinguinal Approach in Treatment of Anterior Pelvic Ring Fracture.

OBJECTIVE: To explore the safety of the corona mortis of the minimally invasive plate insertion in treatment of the anterior pelvic ring fra … els near the obturator. Toothless tweezers were used to peel and lift up the corona mortis from the pubic bone. The maximum vertical distance between the corona mortis and the pubis ranged between 8.8 and 18.3 mm (average of 12.6 ± 3.0 mm). CONCLUSION: The corona mortis have a high rate of incidence, with a large number of differences in the type and shape …

Spontaneous healing of aneurysmal bone cysts. A report of two cases.

Two cases of aneurysmal bone cyst in the pelvis which healed spontaneously are described. Spontaneous regression of aneurysmal bone cysts has not previously been well documented. Our experience with these two cases suggests that radical intervention is not always necessary.

[A case of prostatic carcinoma with osteolytic bone metastases].

A 55-year-old man consulted us on December 3, 1986 with the chief complaints of left leg pain, disturbance in gait and dysuria. On digital rectal examination his prostate was found enlarged to a hen's egg size and increased in consistency. He was admitted to the hospital under the suspicion of prostatic cancer on December 8, 1986. Scout kidney-ureter-bladder X-ray revealed extensive osteolytic lesions in the left iliac and pubic bones as well as in the sacrum. Needle biopsy of the prostate demonstrated moderately differentiated adenocarcinoma, leading to a diagnosis of prostatic cancer with os…

Anatomy of the proximal musculotendinous junction of the adductor longus muscle.

Injuries to the adductor longus commonly occur in the proximal part of the muscle tendon unit, close to the insertion site on the pubic bone. Ultrasonography, magnetic resonance imaging (MRI) and surgery have been helpful in localising the lesions, but the exact anatomy of the musculotendinous junction (MTJ) and insertion of the muscle remain unclear. We studied the anatomical features of the MTJ and measured the dimensions of the tendinous insertion into the pubic bone on 37 cadavers: 18 men and 19 women. The medial boundaries were the longest part of the tendon bilaterally in women, while th…

Total endoscopic anterior pelvic approach (TAPA) - A new approach to the internal fixation of the symphysis.

This study presents a detailed documentation of a total endoscopic anterior pelvic approach (TAPA) for plate fixation of a symphyseal disruption. The purpose of this work is to describe a minimally invasive technique as a possible method for reducing complications and hospitalization. Other goals included giving technical recommendations and assessing potential pitfalls and problems of this new surgical approach. Surgery was performed in an interdisciplinary setting by an experienced orthopaedic and general surgeon. The first endoscopic approach used to visualize the injury was the same as is

Plexiform Neurofibromatosis in a Woman With a Pubic Ramus Stress Fracture.

A 22-year-old woman in Army Basic Combat Training was evaluated in a physical therapy clinic for insidious-onset groin pain. The referring primary care physician assistant ordered initial radiographs, which were noncontributory, followed by a bone scan that indicated a left inferior pubic ramus stress fracture. She was prescribed a 30-day convalescent leave. Due to the palpable mass and severe pain upon return, the physical therapist ordered magnetic resonance imaging, which showed plexiform neurofibromas, with a left buttock mass and left inferior pubic ramus stress fracture.
