FERTILITY CARE IN LOW- AND MIDDLE-INCOME COUNTRIES: Public sector access to medically assisted reproduction in South Africa: a case study.

ABSTRACT: In South Africa, approximately 10% of the calculated need for medically assisted reproduction is being met due to limited access and unequal availability of these services. To facilitate understanding of challenges associated with access to assisted reproduction, a retrospective case study spanning 6 years was performed at one public sector hospital in South Africa offering these services. Demographic profiles, including income, region of residency and access to medical insurance, of patients seeking assistance to become pregnant were investigated. Patients were categorised as those

Indian medical students in public and private sector medical schools: are motivations and career aspirations different? - studies from Madhya Pradesh, India.

BACKGROUND: In recent years, there has been a massive growth in the private medical education sector in South Asia. India's large private medical education sector reflects the market driven growth in private medical education. Admission criteria to public medical schools are based on qualifying examination scores, while admission into private institutions is often dependent on relative academic merit, but also very much on the ability of the student to afford the education. This paper from Madhya Pradesh province in India aims to study and compare between first year medical students in public

Cost-utility analysis of robotic and conventional total knee arthroplasty: A 200-patient micro-costing study in a public sector institution.

Robotic-arm assisted total knee arthroplasty (RO TKA) has been developed to improve surgical precision and reproducibility compared with conventional techniques. However, the economic implications of adopting robotic technology remain uncertain, particularly within publicly funded healthcare systems. This study aimed to evaluate the in-hospital costs and health-related quality of life outcomes associated with RO TKA compared with conventional TKA (CO TKA) using a cost-utility framework within a UK National Health Service (NHS) institution. A propensity-matched cohort cost-utility analysis was

Implementation and Operational Research: Evaluation of a Public-Sector, Provider-Initiated Cryptococcal Antigen Screening and Treatment Program, Western Cape, South Africa.

BACKGROUND: Screening for serum cryptococcal antigen (CrAg) may identify those at risk for disseminated cryptococcal disease (DCD), and preemptive fluconazole treatment may prevent progression to DCD. In August 2012, the Western Cape Province (WC), South Africa, adopted provider-initiated CrAg screening. We evaluated the implementation and effectiveness of this large-scale public-sector program during its first year, September 1, 2012-August 31, 2013. METHODS: We used data from the South African National Health Laboratory Service, WC provincial HIV program, and nationwide surveillance data fo

High public demand and acceptance of newer vaccines in private sector immunisation in Vellore, South India: Implications for India's national immunisation program.

OBJECTIVES: New vaccines licensed, but not included in Expanded Programme on Immunisation (EPI) in India are used exclusively in the private sector. Quantitative data on their public acceptance and demand are lacking. The Well Baby Immunisation Clinic of our private not-for-profit tertiary care teaching institution provides both EPI and new (licensed non-EPI) vaccines giving us the opportunity to document their uptake trends. METHODS: The volumes of EPI and non-EPI vaccines delivered during the last two decades (2003-2023) were analysed for relative comparison of their time trends and trendli

Evaluating policies and regulations used to control corruption among accounting officers in the public sector of South Africa: a systematic literature review.

Corruption, a global phenomenon, affects countries worldwide, transcending geographic and political boundaries, and continues to escalate, partly due to the inefficiency of financial and legal institutions and lack of enforcement. This has significant implications for Africa, hindering democracy, development, and poverty alleviation efforts. Defined as the intentional misuse of official authority for personal benefit, corruption is often fueled by public sector financial managers or accounting officers embezzling funds. The increasing pressure on these officers to justify public fund usage, co

Case studies of health economic analyses informing pharmaceutical health technology assessments for essential medicine selection and public-sector guidelines in South Africa.

BACKGROUND: Constrained resources under universal health coverage (UHC) necessitate a balance between medication costs and essential health system requirements. Policymakers practice priority-setting, as either implicit or explicit rationing, embedded in evidence-informed decision-making processes to guide funding decisions. Health technology assessment (HTA) is a method that may assist explicit evidence-informed priority setting. South Africa developed an official HTA methods guide in 2022, however before this, commissioning and performing economic evaluations was not standardized. METHODS:

An audit of the polytrauma fracture detection rate of clinicians evaluating lodox statscan bodygrams in two South African public sector trauma units.

BACKGROUND: Increasing global demand for specialized radiological investigations has resulted in delayed or non-reporting of plain trauma radiographs by radiologists. This is particularly true in resource-limited environments, where referring clinicians rely largely on their own radiographic interpretation. A wide accuracy range has been documented for non-radiologist reporting of conventional trauma radiographs. The Lodox Statscan whole-body digital X-ray machine is a relatively new technology that poses unique interpretive challenges. The fracture detection rate of trauma clinicians utilizin

Self-reported knowledge and skills related to diagnosis and management of keratoconus among public sector optometrists in the Limpopo province, South Africa.

BACKGROUND: Keratoconus (KC) has been regarded as a rare condition, although recent studies, including those in South Africa, suggest it is common and is increasing in prevalence. Furthermore, South African-based studies have shown that KC is normally detected at advanced or severe stages. Knowledge and skills for examination, diagnosis and management of KC by optometrists are important for the early detection and management of the disease. AIM: This study aimed to evaluate self-reported knowledge and skills for examination, diagnosis and management of KC patients among public sector optometr

Associations Between Adverse Childhood Experiences and Tobacco, Alcohol, and Drug Use Among Individuals with Serious Mental Illnesses in Public-Sector Treatment Settings.

Adverse childhood experiences (ACEs) increase risk of substance use disorders (SUDs). Little research has focused on individuals with serious mental illnesses (SMI), despite their high prevalence of both ACEs and SUDs. We combined two datasets from prior studies (n = 299 and n = 240, total n = 539) that measured ACEs and made research diagnoses for SUDs. When controlling for other variables-age, gender, race, diagnostic category (psychotic disorder versus mood disorder), and study site (Washington, DC-area versus southeast Georgia)- in logistic regression models, ACE score was associated with

Willingness to pay and preferences for mindfulness-based interventions among patients with chronic low back pain in the Hong Kong public healthcare sector.

INTRODUCTION: Low back pain (LBP) is a leading cause of disability worldwide. Mindfulness-based interventions (MBIs) are effective for LBP management when combined with medication and physical therapy. An understanding of patients' willingness to pay (WTP) and preferences is needed to integrate MBIs into standard LBP care. We examined WTP and preferences for MBIs, as well as associated factors, among patients with chronic LBP in the Hong Kong public healthcare sector. METHODS: A cross-sectional survey was conducted in two Hong Kong public hospitals. We used the payment card method to assess p
