[Combined pulmonary fibrosis and emphysema - case report and literature review].

We describe the case of a 61-year-old male patient, in which the search for the cause of chronic respiratory failure, severe pulmonary hypertension and secondary erythrocytosis resulted in a diagnosis of combined pulmonary fibrosis and emphysema (CPFE). This is a unique, recently characterised syndrome with upper-lobe emphysema and pulmonary fibrosis of the lower lungs. The cause is unknown, but one of the main risk factor remains smoking. The patient was a heavy smoker (over 40 pack-years). He complained of dyspnoea on exertion and cough. Physical examination revealed basal crackles and cyano

Pulmonary interstitial emphysema during piston-type high-frequency oscillatory ventilation.

We retrospectively reviewed data on high frequency oscillatory ventilation (HFOV) settings and pulmonary function in 14 extremely low birth weight infants who were treated with piston-type HFOV immediately after birth for severe respiratory distress to investigate the cause of pulmonary interstitial emphysema (PIE) associated with HFOV. PIE developed a mean of 23 hr after the initiation of HFOV in 6 infants. Eight infants did not develop PIE and served as the control group. The lowest mean airway pressure (MAP) in the PIE group, which occurred immediately before disease onset was 5.2 cm H2O. T

Severe localised pulmonary interstitial emphysema--decompression by selective bronchial intubation.

We report three infants (26-28 weeks gestational age) in whom selective bronchial intubation was associated with successful decompression of severe localised pulmonary interstitial emphysema, uniquely in two cases this involved intubation of the left main bronchus. Pulmonary interstitial emphysema did not recur despite maintenance of selective intubation for only 48 hours or less in all three cases.

Air embolus following pulmonary interstitial emphysema in hyaline membrane disease.

Two cases of hyaline membrane disease are presented complicated by severe, interstitial emphysema and resulting in death from air embolus. This is consistent with the experimental evidence of other investigators that shows that such an air embolus may be more common complication of hyaline membrane disease than previously expected.

Bilateral lung transplantation for pulmonary emphysema associated with cutis laxa.

Cutis laxa is a rare elastic tissue disorder that mainly affects the skin and results in loss of elasticity. Occasionally, pulmonary emphysema complicates this condition. Herein, we report the first case of successful lung transplantation for severe juvenile emphysema associated with cutis laxa. The patient underwent bilateral deceased-donor lung transplantation. The patient's lungs had very thin visceral pleura due to a deficiency of elastic fibers along with bronchomalacia and fragile bronchial cartilage. The right upper bronchus and truncus intermedius were individually anastomosed to impro…

Unilateral neonatal pulmonary interstitial emphysema managed conservatively: A case report.

BACKGROUND: Pulmonary interstitial emphysema (PIE) is a pathological state when air escapes from ruptured alveoli and is trapped along the sheaths surrounding the bronchovascular bundle. PIE is not uncommon in infants who require mechanical ventilation and even less common in infants on noninvasive ventilatory support; however, it is extremely unusual in infants in room air. CASE PRESENTATION: A 2-week-old male infant developed worsening tachypnea in the special-care nursery. The patient was born at 33 weeks' gestation by induced vaginal delivery due to pre-eclampsia. He required positive pre

Computed tomography-based centrilobular emphysema subtypes relate with pulmonary function.

INTRODUCTION: Centrilobular emphysema (CLE) is recognized as low attenuation areas (LAA) with centrilobular pattern on high-resolution computed tomography (CT). However, several shapes of LAA are observed. Our preliminary study showed three types of LAA in CLE by CT-pathologic correlations. This study was performed to investigate whether the morphological features of LAA affect pulmonary functions. MATERIALS AND METHODS: A total of 73 Japanese patients with stable CLE (63 males, 10 females) were evaluated visually by CT and classified into three subtypes based on the morphology of LAA includi

Pulmonary emphysema subtypes on computed tomography: the MESA COPD study.

BACKGROUND: Pulmonary emphysema is divided into 3 major subtypes at autopsy: centrilobular, paraseptal, and panlobular emphysema. These subtypes can be defined by visual assessment on computed tomography (CT); however, clinical characteristics of emphysema subtypes on CT are not well defined. We developed a reliable approach to visual assessment of emphysema subtypes on CT and examined if emphysema subtypes have distinct characteristics. METHODS: The Multi-Ethnic Study of Atherosclerosis COPD Study recruited smokers with chronic obstructive pulmonary disease (COPD) and controls ages 50-79 yea…

Combined pulmonary fibrosis and emphysema syndrome in connective tissue disease.

OBJECTIVE: Connective tissue diseases (CTDs) are associated with several interstitial lung diseases. The aim of this study was to describe the recently individualized syndrome of combined pulmonary fibrosis and emphysema (CPFE) in a population of patients with CTD. METHODS: In this multicenter study, we retrospectively investigated data from patients with CTD who also have CPFE. The demographic characteristics of the patients, the results of pulmonary function testing, high-resolution computed tomography, lung biopsy, and treatment, and the outcomes of the patients were analyzed. RESULTS: Da

Centrilobular emphysema combined with pulmonary fibrosis results in improved survival.

BACKGROUND: We hypothesized that, in patients with pulmonary fibrosis combined with emphysema, clinical characteristics and outcomes may differ from patients with pulmonary fibrosis without emphysema. We identified 102 patients who met established criteria for pulmonary fibrosis. The amount of emphysema (numerical score) and type of emphysema (centrilobular, paraseptal, or mixed) were characterized in each patient. Clinical characteristics, pulmonary function tests and patient survival were analysed. RESULTS: Based on the numerical emphysema score, patients were classified into those having n

Selective intubation in pulmonary interstitial emphysema: experience in five patients.

Selective bronchial intubation can be useful in the management of severe localized pulmonary interstitial emphysema (PIE). Five infants between 9 and 20 days old with severe left sided PIE had the right main bronchus intubated for between 4 to 30 h. Four patients showed permanent and one patient temporary improvement and the experience from these cases suggests that prolonging the procedure after obtaining radiological evidence of improvement may not be necessary. There were no serious complications although right upper lobe atelectasis occurred in three cases and could not be prevented by usi

Persistent localized pulmonary interstitial emphysema and lymphangiectasia: a causal relationship?

The mechanism responsible for persistent regional pulmonary interstitial emphysema under tension, which occasionally complicates assisted ventilation, has not been explained. It is difficult to comprehend how interstitial air may cause lobar hyperinflation and not escape to the mediastinum or pleura. Histologic examination of resected emphysematous lung in two neonates with this type of interstitial emphysema revealed air dissecting from the interstitial connective tissue into markedly dilated lymphatics. It is proposed that intralymphatic air dissection causing emphysematous lymphangiectasia
