Diffuse Xanthogranulomatous pyelonephritis: A Rare Disease With A Common Presentation.

Xanthogranulomatous pyelonephritis (XGP) is a distinct entity characterized by chronic granulomatous changes in the renal parenchyma associated with renal destruction and urinary tract abnormalities, most often from obstruction or infection in the urinary tract. We have presented the case report of a girl with fever, abdominal pain, vomiting, anorexia, and weight loss. Computed tomography of the abdomen showed multiloculated cystic lesions with calcifications and a psoas muscle abscess, which tested positive for Escherichia (E.) coli. Histopathology revealed lipid-laden macrophages, multinucle

Pyeloduodenal Fistula in Xanthogranulomatous Pyelonephritis: A Series of Two Cases.

Xanthogranulomatous inflammation, characterized by destruction and replacement of tissues with chronic inflammatory cells, including foamy histiocytes and hemosiderin-laden macrophages, is uncommon. In patients with xanthogranulomatous pyelonephritis, inflammation may extend from the kidney to the overlying duodenum, creating a pyeloduodenal fistula that further complicates medical and surgical management. We present two cases with recurrent kidney infections who each ultimately received a nephrectomy and repair of their duodenal fistula.

Endometriosis in a kidney with focal xanthogranulomatous pyelonephritis and a perinephric abscess.

BACKGROUND: The presence of endometriosis in the kidney is extremely rare. We report a case of endometriosis in renal parenchyma incidentally found in a malfunctioning kidney removed because of xanthogranulomatous pyelonephritis. CASE PRESENTATION: A 53-year-old Chinese premenopausal woman presented with intermittent right flank pain for many years. Imaging studies revealed a contracted non-functioning right kidney and a perinephric abscess. The contracted kidney was considered to have resulted from chronic pyelonephritis. The abscess was drained. The patient subsequently underwent a right ne

[Xanthogranulomatous pyelonephritis with pyonephrosis in a 4-year-old child].

Xanthogranulomatous pyelonephritis is a rare form of chronic pyelonephritis observed in only a few cases in children. Symptoms are mild, which explains the delay in diagnosis. Diagnosis is based on histology but can be suspected on CT. The treatment is medical and often surgical, with an uncertain renal prognosis. It is therefore imperative to diagnose early. We report the case of a 4-year-old child who presented with xanthogranulomatous pyelonephritis caused by Pseudomonas aeruginosa, which evolved into pyonephrosis, due to inadequate antibiotic therapy. This highlights the importance of unde

Laparoscopic heminephrectomy in a horseshoe kidney affected by xanthogranulomatous pyelonephritis.

BACKGROUND AND OBJECTIVES: Horseshoe kidney is a relatively common renal fusion anomaly. There are increasing reports of successful laparoscopic management of surgical disease in this entity. Xanthogranulomatous pyelonephritis (XGP) is an inflammatory condition of chronically obstructed and infected kidneys that is typically refractory to medical management. XGP has been regarded as a contraindication to laparoscopic surgery. Successful laparoscopic nephrectomies have been reported, albeit with high conversion and complication rates. In this report, we describe successful laparoscopic hemineph

[A case of xanthogranulomatous pyelonephritis presenting with the flank subcutaneous mass].

A 51-year-old female exhibited fever, left flank pain and left flank mass in March, 1993. Drip infusion pyelography (DIP) revealed a non-functioning left kidney with shadows of calculi, and abdominal computerized tomography (CT) showed renal calculi and multilocular cystic lesions in the left kidney extending through the perinephric space into the mass on the left flank. Percutaneous nephrostomy and percutaneous drainage were performed, followed by left nephrectomy. Histopathological findings revealed xanthogranulomatous pyelonephritis. There have been a few case reports of xanthogranulomatous

Xanthogranulomatous pyelonephritis: MRI findings in the diffuse and the focal type.

Two cases of xanthogranulomatous pyelonephritis (XGP) are presented with emphasis on MR appearances. One case is the diffuse type of XGP secondary to chronic obstruction caused by transitional cell carcinoma of the renal pelvis. The other case is the focal or "tumefactive" type of XGP which mimics renal cell carcinoma.

[Abscess of the abdominal wall: a rare presentation of xanthogranulomatous pyelonephritis].

A young psychiatric patient presented with a purulent collection in the lower abdominal wall. Simple drainage failed to heal the abdominal wall abscess, and the presence of a primary septic focus was suspected. Full investigation showed a right renal pelvis calculus with xanthogranulomatous pyelonephritis complicated by a retroperitoneal abscess. Recovery was complete after a nephrectomy. A review of the published literature demonstrated the rarity of this presentation, although even more curious cases have been described, including a urinobronchial fistula, as well as catastrophic cases prese

Emphysematous pyelonephritis in a xanthogranulomatous kidney. An unusual cause of pneumoperitoneum.

Emphysematous pyelonephritis is a rare, life-threatening suppurative infection of the renal parenchyma and perinephric tissues. The disease is encountered primarily in patients with diabetes mellitus or ureteral obstruction associated with perinephric and intrarenal gas. Causative organisms are those normally found in the urinary and gastrointestinal tracts; however, anaerobic bacteria have been demonstrated in only 1% of cases. We describe a case of emphysematous pyelonephritis, which presented as an acute abdomen with pneumoperitoneum in a nondiabetic patient. No visceral injury was found at

Xanthogranulomatous pyelonephritis: a review and meta-analysis with a focus on management.

PURPOSE: Xanthogranulomatous pyelonephritis (XGP) is a rare form of pyelonephritis more commonly seen in females and diabetics. Frequently associated with renal tract calculi, it is often difficult to diagnose, as it can clinically and radiologically mimic other disorders. Most cases are treated with antibiotics and nephrectomy. The aim of our review is to summarise and analyse the current evidence focusing on management. METHODS: A literature search was conducted to identify papers relating to xanthogranulomatous pyelonephritis in adults. Studies containing ten or more patients with XGP were

Xanthogranulomatous pyelonephritis: a focus on microbiological and antibiotic resistance profiles.

BACKGROUND: Xanthogranulomatous pyelonephritis (XGP) is an inflammatory condition of the kidney and its treatment most often involves a combination of antibiotics and nephrectomy. This study aimed to define the clinical features and management of XGP, focusing on microbiological aspects and antibiotic therapy. METHODS: We performed a retrospective study of 27 cases of XGP diagnosed between January 2001 and January 2020 to analyse their clinical and management characteristics. In addition, a literature review was conducted of XGP case series covering the period from 2000-2020. We searched PubM

Malakoplakia and xanthogranulomatous pyelonephritis treated with nephrectomy: A case report.

RATIONALE: Malakoplakia and xanthogranulomatous pyelonephritis are chronic inflammatory conditions of the kidney characterized by the infiltration of inflammatory cells. PATIENT CONCERNS: An 82-year-old female patient had a history of hypertension, type 2 diabetes mellitus, dyslipidemia, and end-stage renal disease under hemodialysis. She was admitted repeatedly 4 times within 4 months due to urosepsis. DIAGNOSIS: The enlarged right kidney with a low-density lesion at the right middle calyx, and a well-enhanced ureter were noted on the computed tomography scan. Therefore, xanthogranulomatous

[Xanthogranulomatous pyelonephritis in a child with severe malnutrition and recurrent fever].

Xanthogranulomatous pyelonephritis is a rare inflammatory disease, characterized by replacement of renal parenchyma with granulomatous tissue. Initial clinical presentation includes abdominal pain and constitutional symptoms related to recurrent urinary infections. The microorganisms most commonly involved are Escherichia coli and Proteus mirabilis. Final diagnosis is made by histopathology, and the only curative treatment is total or partial nephrectomy. A recently diagnosed case in our unit is presented, as well as an update on the knowledge of this disease.

Mucinous adenocarcinoma of the renal pelvis masquerading as xanthogranulomatous pyelonephritis.

Primary adenocarcinoma of the renal pelvis accounts for less than 1% of renal pelvis tumors. We encountered a patient who presented with long-term nephrolithiasis and hydronephrosis. The initial impression was xanthogranulomatous pyelonephritis. The postoperative histopathologic report showed mucinous adenocarcinoma of the renal pelvis with signet ring cell formation. Signet ring cell phenomenon is extremely rare in this kind of tumor. We describe the patient's clinical presentation, imaging findings, histopathologic examination, and immunohistochemistry results.

Laparoscopic nephrectomy for xanthogranulomatous pyelonephritis--are there predictive factors for success?

OBJECTIVES: Laparoscopic nephrectomy for xanthogranulomatous pyelonephritis is currently associated with great operative difficulty and surgical complications. Herein, we report on our single-center experience and describe predictive factors for successfully accomplishing this procedure. METHOD: Between March 1998 and April 2010, 66 patients (27 men and 39 women) underwent laparoscopic nephrectomy for the treatment of a unilateral nonfunctioning kidney. These patients had previous diagnoses of renal chronic inflammation associated with calculi and previous pyonephrosis. All of the nephrectomi

Laparoscopic nephrectomy for xanthogranulomatous pyelonephritis in childhood: the way forward.

OBJECTIVE: Xanthogranulomatous pyelonephritis (XGP) is a severe, chronic renal parenchymal infection. Nephrectomy is curative. Open nephrectomy is a well-known operative challenge. We describe successful laparoscopic nephrectomy in three patients with XGP. MATERIALS AND METHODS: Case notes and radiology of three patients who underwent laparoscopic nephrectomy for XGP were reviewed. The preoperative presentation, operative details, postoperative recovery and complications were recorded. RESULTS: The ages of the three children were 9, 5 and 1.3 years. Retroperitoneoscopy was performed via 3 x

Diffuse xanthogranulomatous pyelonephritis with psoas abscess in a pregnant woman.

We report the first case, to our knowledge, of xanthogranulomatous pyelonephritis (XGP) with psoas abscess occurring during pregnancy. A 37-year-old woman in the third trimester of pregnancy presented with low back pain and a fever. From sonographic features, a multidisciplinary team decided to perform computed tomography of the abdomen with contrast agent injection, which strongly suggested diffuse XGP of the left kidney with a psoas abscess. Cesarean section at 32 weeks was followed by extended nephrectomy. Pathological examination of the operative specimen confirmed the diagnosis. Outcomes
