Differentiation of radicular cysts and radicular granulomas via texture analysis of multi-slice computed tomography images.

OBJECTIVES: This study aimed to establish a method for differentiating radicular cysts from granulomas via texture analysis (TA) of multi-slice computed tomography (CT) images. METHODS: A total of 222 lesions with multi-slice computed tomography images acquired at our hospital between 2013 and 2022 that were pathologically diagnosed were included in this study. Cases of contrast-enhanced images, severe metallic artefacts, and lesions that were not sufficiently large to be analysed were excluded. The images were chronologically divided into a training group and a validation group. The radiolog…

Immunohistochemical expression of vascular endothelial growth factor and matrix metalloproteinase-9 in radicular and residual radicular cysts.

OBJECTIVE: This study assessed and compared the immunoexpression of vascular endothelial growth factor (VEGF) and matrix metalloproteinase-9 (MMP-9) in radicular cysts (RCs) and residual radicular cysts (RRCs), relating them to the angiogenic index and the intensity of the inflammatory infiltrate. MATERIAL AND METHODS: Twenty RCs and 10 RRCs were evaluated by immunohistochemistry using anti-VEGF and anti-MMP-9 antibodies. The angiogenic index was determined by microvessel count (MVC) using anti-von Willebrand factor antibody. RESULTS: The expression of both VEGF and MMP-9 was higher in RCs t…

Paradental cyst mimicking a radicular cyst on the adjacent tooth: case report and review of terminology.

A distinctive form of paradental cyst can occur on the buccal and apical aspects of erupted mandibular molars. This cyst has peculiar clinical and radiographic features, although the microscopic findings are the same as those of odontogenic inflammatory cysts. Diagnostic and therapeutic problems can occur when this lesion is misinterpreted as a radicular cyst. The purpose of this paper is to present an additional case of a paradental cyst in the buccal and mesial aspects of a mandibular second molar involving the apical area of a mandibular first molar. The difficulty of diagnosis, treatment, …

Radicular cysts of primary teeth mimicking premolar dentigerous cysts: report of three cases.

In three cases, the clinical diagnosis of dentigerous cyst was disproved by surgical exploration. In all other cases reviewed from a thirteen-year period, the clinical diagnosis of radicular cyst from an infected primary tooth was verified by surgery and histological examination.

Differences in Inflammation and Bone Resorption between Apical Granulomas, Radicular Cysts, and Dentigerous Cysts.

INTRODUCTION: Dental cysts can be of inflammatory (radicular cysts) or noninflammatory (dentigerous cysts) origin. Apical periodontitis is a necrosis of the pulp and infection of the root canal causing the development of apical granulomas or radicular cysts. The immunology of granuloma and cyst formation is important because modern root filling materials are immunologically active and can contribute to the resolution of apical granulomas. In contrast, radicular cysts often require apicectomy. A better understanding of the pathophysiology of inflammation and bone resorption in apical periodonti…

A clinical report demonstrating the significance of distinguishing a nasopalatine duct cyst from a radicular cyst.

Endodontic diagnosis is challenging and depends on the organisation of information from the patient history, clinical examination and analysis of the pulp, radiographic and histopathological assessment. A 35-year-old man was endodontically treated for radiolucency in relation to the roots of maxillary central incisors as it was a provisionally diagnosed case of radicular cyst. Since the palatal swelling persisted, the lesion was re-evaluated using relevant diagnostic aids and a diagnosis of nasopalatine duct cyst (NPDC) was made, which was missed during the initial assessment. An erroneous int…

Development and Treatment of Radicular Cyst in Pediatric Patient-Case Report.

Background: Inflammation in the periapical area of primary teeth can affect the development and eruption of permanent teeth. In an asymptomatic course, they are detected accidentally during routine examinations. In such cases, they often reach significant dimensions and cause irreversible changes. Methods: This report presents the case of a 9-year-old boy with a radicular cyst in the mandible, resulting in the displacement of both premolar tooth buds. To address the inflammation, facilitate the proper eruption of the impacted teeth, and prevent adjacent teeth from shifting, the primary molars …

Successful Endo-Surgical Management of Radicular Cyst - A Clinical Case Report.

The most frequent cystic lesions affecting the jaws are radicular cysts, which present radiographically as an oval or pear-shaped unilocular radiolucency in the periapical region. Radicular cysts are usually asymptomatic. This case study describes the surgical treatment of a radicular cyst involving two maxillary anterior teeth. The diagnosis of radicular cyst was confirmed through a clinical and radiographic evaluation. An apicoectomy was performed, and Mineral Trioxide Aggregate (MTA) was placed as the apical barrier. This case report illustrates the effective surgical management of an exten…

Surgical enucleation of radicular cyst in endodontically treated primary mandibular molar.

Radicular cysts are most common odontogenic cysts seen in the orofacial region, but not commonly detected in paediatric cases. This case report describes the accidental detection of radicular cyst and its surgical management in an 8-year-old boy in lower left back region of the jaw. The article also gives an insight about radicular cyst from its aetiology to the various other treatment modalities.

Radicular cyst associated with primary molar: Surgical intervention and space management.

Radicular cysts associated with primary teeth are rare. This case report describes radicular cyst associated with carious primary mandibular second molar in a 5-year-old patient and discusses its surgical intervention, space management, and follow-up for 3½ years.

Radicular cyst associated with endodontically treated deciduous tooth: a case report.

This article presents a case report of radicular cyst associated with an endodontically treated deciduous second molar causing displacement of the permanent successor, with accompanying buccal expansion. Cystic sac was removed surgically along with the involved tooth under general anesthesia. Healing was uneventful. Histopathologically the cystic sac was consistent with the features of radicular cyst. Unusual amorphous, eosinophilic, atubular material incorporated within the cystic epithelium was observed.

Case report: a large radicular cyst involving the entire maxillary sinus.

Cysts of the maxillary sinus of odontogenic origin have been well-documented in the literature. Most of these lesions involve the apex of the offending tooth and appear as a well-defined periapical radiolucency. Presented here is a case of an unusually large lesion, which involved the entire maxillary sinus and extended into the floor of the nose. The lesion also caused paresthesia on the affected side. The lesion was removed with conservative treatment without any postoperative complications.

Adenomatoid odontogenic tumor mimicking a periapical (radicular) cyst: a case report.

The adenomatoid odontogenic tumor (AOT) is a benign (hamartomatous), noninvasive lesion with slow but progressive growth. A rare subvariant of the extrafollicular type of AOT may mimic periapical disease radiographically. We report on a 15-year-old girl with a periapical radiolucent lesion affecting teeth 7 and 8 initially suspicious of periapical disease, although clinical findings seemed to indicate a nonendodontic lesion. An exploratory surgical approach was chosen, and the final diagnosis of the removed noncystic tissue mass was microscopically confirmed to be an AOT.

[Histological changes of radicular cyst epithelial lining due to chronic inflammation].

The paper deals with the investigation of the relationship between chronic inflammation in the subepithelial layer and histological changes of the epithelium of radicular cysts. Histological examination of 60 radicular cyst specimens was carried out with the aim to define this relationship. The stratified squamous epithelium with a variety of histological changes was found in 58 specimens. A high degrees of chronic inflammation in the subepithelial layer was also recorded. Histological changes of the epithelium correlated to the degree of inflammation in the subepithelial layer of radicular cy…
