An Ear Mystery: Conductive Hearing Loss with Normal Acoustic Reflexes and VEMPs.

The cause of sudden conductive loss in the absences of other vestibulo-audiologic abnormalities can be isolated to specific subsites of the ossicular chain using audiometric testing. In the absence of such abnormalities, a rare etiology may be the cause after an inciting trauma. Laryngoscope, 134:4344-4346, 2024.

Acoustic Reflexes in Aural Atresia Patients: Evidence of an Intact Efferent System?

OBJECTIVE: To record crossed acoustic reflex thresholds (xART's) postoperatively from patients after surgical repair of unilateral congenital aural atresia (CAA). To seek explanations for when xARTs can and cannot be recorded. We hope to understand the implications for this central auditory reflex despite early afferent deprivation. METHODS: Patients who underwent surgery to correct unilateral CAA at a tertiary academic medical were prospectively enrolled to evaluate for the presence of xART. Preoperative ARTs in the normal (non-atretic) ear, and postoperative ipsilateral ARTs (stimulus in th

Wideband Acoustic Immittance in Cochlear Implant Recipients: Reflectance and Stapedial Reflexes.

OBJECTIVES: to characterize differences in wideband power reflectance for ears with and without cochlear implants (CIs), to describe electrically evoked stapedial reflex (eSR)-induced changes in reflectance, and to evaluate the benefit of a broadband probe for reflex threshold determination for CI recipients. It was hypothesized that reflectance patterns in ears with CIs would be consistent with increased middle ear stiffness and that reflex thresholds measured with a broadband probe would be lower compared with thresholds obtained with a single-frequency probe. DESIGN: Eleven CI recipients p

Hyperactive auditory efferent system and lack of acoustic reflexes in Williams syndrome.

The auditory efferent system and acoustic reflexes have been investigated in patients with Williams syndrome (WS). Twenty-one patients aged 6-26 years with a genetically confirmed diagnosis of WS and with reported hyperacusis were compared with 21 normally developing age-matched subjects. The medial olivocochlear (MOC) efferent system was tested by stimulation of the contralateral ear with increasing levels of white noise, while recording transient evoked otoacoustic emissions (TEOAE) in the ipsilateral ear. The suppression effect on the amplitudes of the TEOAE was computed for each contralate

Predicting vulnerability to acoustic injury with a noninvasive assay of olivocochlear reflex strength.

Permanent noise-induced damage to the inner ear is a major cause of hearing impairment, arising from exposures occurring during both work- and pleasure-related activities. Vulnerability to noise-induced hearing loss is highly variable: some have tough, whereas others have tender ears. This report documents, in an animal model, the efficacy of a simple nontraumatic assay of normal ear function in predicting vulnerability to acoustic injury. The assay measures the strength of a sound-evoked neuronal feedback pathway to the inner ear, the olivocochlear efferents, by examining otoacoustic emission

The acoustic reflex threshold: not predictive for loudness perception in normally-hearing listeners.

The working hypothesis of an ongoing study is that the quick and reliable procedure of acoustic reflex threshold (ART) determination in conjunction with measurements of HTL may yield accurate estimates of loudness. The aim of this study was to investigate whether differences in loudness in normally-hearing subjects are reflected in the ARTs and to collect normal material with respect to pure-tone elicited ART and loudness categories. Categorical loudness scaling (CLS) and ART measurements were performed at frequencies of 0.5, 1, 2 and 4 kHz in 60 normally-hearing subjects (HTL<20 dB HL, 26 mal

The acoustic reflex at a 1000 Hz probe frequency: phasor and vector analysis.

Phasor plots of reflex growth functions have been inconclusive concerning the effect of the reflex for mass dominated ears. The present study aimed to establish whether vector plots clarified the effects of the reflex for phasors not showing a clear circular shape. Measured admittance data (ipsilateral reflexes across a wide intensity range) was represented as both phasor diagrams and converted to impedance quantities, represented as vectors. The results analysed for 34 ears showed few unclassifiable phasor diagrams. In addition, all growth functions showed increased stiffness on vector analys

Comparative threshold studies of the acoustic pinna, jaw and startle reflex in the rat.

Electromyograms of M. Levator auris and M. Temporalis and movement produced by whole body startle were recorded simultaneously in awake, freely moving rats. Thresholds were 78 db SPL for the L. auris, 80 dB SPL for the ballistic and 81 dB SPL for the Temporalis. The rank ordering of the three thresholds was extremely strict, 188 suprathreshold M. L. auris responses could be observed without M. Temporalis responses, but only once was a M. Temporalis response observed without a M. L. auris response. Thresholds as well as amplitudes and latencies measured by the different methods show correlated

Exaggerated acoustic reflex response in a patient with transient facial palsy and hyperacusis.

An unusual acoustic reflex response was documented by immittance recording in a patient with acute facial palsy. The amplitude of the stapedial response to sustained stimulation was reduced when recorded from the affected side, suggesting the facial nerve was affected proximal to the stapedius muscle. In addition, the impedance change at onset of intense acoustic stimulation of the affected side was unusually large, regardless of which stapedius response was recorded. The large change was coincident with the reported hyperacusis in the stimulated ear. This suggested the cochlear nerve as site

No change in the acoustic reflex threshold and auditory brainstem response following short-term acoustic stimulation in normal hearing adults.

Unilateral auditory deprivation or stimulation can induce changes in loudness and modify the sound level required to elicit the acoustic reflex. This has been explained in terms of a change in neural response, or gain, for a given sound level. However, it is unclear if these changes are driven by the asymmetry in auditory input or if they will also occur following bilateral changes in auditory input. The present study used a cross-over trial of unilateral and bilateral amplification to investigate changes in the acoustic reflex thresholds (ARTs) and the auditory brainstem response (ABR) in nor

KCNQ4 potassium channel subunit deletion leads to exaggerated acoustic startle reflex in mice.

The potassium voltage-gated channel subfamily Q member 4 (KCNQ4) subunit forms channels responsible for M-current, a muscarine-sensitive potassium current regulating neuronal excitability. In contrast to other KCNQ subunits, its expression is restricted to the cochlear outer hair cells, the auditory brainstem and other brainstem nuclei in a great overlap with structures involved in startle reflex. We aimed to show whether startle reflexis affected by the loss of KCNQ4 subunit and whether these alterations are similar to the ones caused by brainstem hyperexcitability. Young adult KCNQ4 knockout

Hyperexcitability of inferior colliculus and acoustic startle reflex with age-related hearing loss.

Chronic tinnitus and hyperacusis often develop with age-related hearing loss presumably due to aberrant neural activity in the central auditory system (CAS) induced by cochlear pathologies. However, the full spectrum of physiological changes that occur in the CAS as a result age-related hearing loss are still poorly understood. To address this issue, neurophysiological measures were obtained from the cochlea and the inferior colliculus (IC) of 2, 6 and 12 month old C57BL/6J mice, a mouse model for early age-related hearing loss. Thresholds of the compound action potentials (CAP) in 6 and 12 mo

Wideband acoustic-reflex test in a test battery to predict middle-ear dysfunction.

A wideband (WB) aural acoustical test battery of middle-ear status, including acoustic-reflex thresholds (ARTs) and acoustic-transfer functions (ATFs, i.e., absorbance and admittance) was hypothesized to be more accurate than 1-kHz tympanometry in classifying ears that pass or refer on a newborn hearing screening (NHS) protocol based on otoacoustic emissions. Assessment of middle-ear status may improve NHS programs by identifying conductive dysfunction and cases in which auditory neuropathy exists. Ipsilateral ARTs were assessed with a stimulus including four broadband-noise or tonal activator

Temporal integration of the contralateral acoustic-reflex threshold and its age-related changes.

Although numerous studies have investigated temporal integration of the acoustic-reflex threshold (ART), research is lacking on the effect of age on temporal integration of the ART. Therefore the effect of age on temporal integration of the ART was investigated for a broad-band noise (BBN) activator. Subjects consisted of two groups of adults with normal-hearing sensitivity: one group of 20 young adults (ten males and ten females, ages 18-29 years, with a mean age of 24 years) and one group of 20 older adults (ten males and ten females, ages 59-75 years, with a mean age of 67.5 years). Estimating the acoustic reflex threshold from wideband measures of reflectance, admittance, and power.

OBJECTIVE: A method was developed to estimate the contralateral acoustic reflex threshold using shifts in wideband energy reflectance, admittance magnitude and power. DESIGN: In the first experiment contralateral reflex thresholds for a noise activator were estimated on three adult participants using reflectance, admittance and power measurements at frequencies from 250 to 8000 Hz. The reflex threshold was defined using a magnitude and a correlation technique, both having the property of examining the pattern of the reflex-induced shift across a fairly broad frequency range (250 to 2000 Hz).

Acoustic reflex monitoring during the presentation of 1000 clicks at high repetition rates.

This study was designed to monitor acoustic reflexes during the type of stimulation used in the auditory brainstem measures. Twenty normal female subjects within the age range 20 to 26 years participated in the study. The probe tone frequency was 226 Hz and the intensity was 85 dB SPL. Approximately 1000 clicks were presented to the right ear through an insert earphone and acoustic reflexes were monitored in the left ear. The clicks were presented at 95 and 105 dB peSPL and at 50 and 100/sec rates. At the rate of 50/sec, the acoustic reflex was present in 70% of the subjects at 95 dB peSPL and

Acoustic-reflex adaptation: morphology and half-life data for subjects with normal hearing.

Acoustic-reflex adaptation was studied in 35 subjects with normal hearing using nine pure-tone activators (250-6000 Hz) and a broadband-noise activator. The individual subject data generated by the 31-s activators presented 10 dB above the reflex threshold were digitized, corrected for baseline drift and ear canal volume, and analyzed in terms of the acoustic-admittance change in acoustic mmhos and in terms of the percentage of maximum-reflex magnitude. Reflex adaptation increased as a function of frequency. The adaptation functions for the lower frequencies (less than or equal to 1500 Hz) wer

A comparison of three methods for predicting hearing loss from acoustic reflex thresholds.

Previously reported acoustic reflex threshold data from normal and hearing-impaired subjects indicate that the effect of stimulus bandwidth on reflex thresholds is altered by sensorineural hearing loss. It is this change in the "bandwidth effect" that forms the basis for predicting hearing loss from reflex threshold data. Three predictive procedures were compared for 17 normal and 60 hearing-impaired ears. All methods correctly identified most hearing losses but none of the methods accurately estimated magnitude of hearing loss. Two methods were characterized by a high rate of false positives.

Measurement of the Electrically Evoked Stapedial Reflex Response with Wideband Acoustic Reflectance Measurement.

BACKGROUND: The electrically evoked stapedial reflex threshold (ESRT) has been shown to be a good predictor of upper stimulation level for cochlear implant recipients. Previous research has shown that the ESRT may be recorded at lower stimulation levels and with a higher incidence of success with the use of higher frequency probe tones (e.g., 678 and 1000 Hz) relative to the use of the conventional 226-Hz probe tone. Research has also shown that the acoustic reflex may be recorded at lower stimulus levels with the use of wideband reflectance when compared to the acoustic reflex threshold recor
