Central sleep related breathing disorders - diagnostic and therapeutic features.

THREE CLASSES OF CENTRAL SRBD ARE DISTINGUISHED: 1. Central sleep apnea (CSA), 2. Cheyne-Stokes Respiration as a subgroup of CSA and 3. central hypoventilation syndromes. Reduced or completely absent central respiratory drive without upper airway obstruction is the common feature of central SRBD. Hypoventilation syndromes most often occur secondary in patients with neuromuscular, pulmonary or sceletal diseases or in patients with massive obesity. In patients with hypoventilation during sleep nocturnal and exertional dyspnea and headaches are frequently reported symptoms. Excessive daytime slee

Looking both ways before crossing the street: Assessing the benefits and risk of opioids in treating patients at risk of sleep -disordered breathing for pain and dyspnea.

Opioids adversely influence respiration in five distinct ways. Opioids reduce the respiratory rate, tidal volume, amplitude, reflex responses to hypercapnia and hypoxia, and arousability related necessary for respiratory adaptive responses. Opioids cause impairment of upper pharyngeal dilator muscles leading to obstructive apnea. Opioids cause complex sleep disordered breathing (SDB) consisting of central sleep apnea and obstructive sleep apnea. Clinically opioids worsen pre-existing SDB. Recent studies have shown increased morbidity and mortality in patients receiving opioids for chronic nonc

Death due to positional asphyxia related to underlying seizure disorder.

Positional asphyxia occurs when a victim fails to remove themself from a compromising position, leading to the restriction of respiration with subsequent death. Many cases involve an inverted body position with or without abnormal kinking of the neck. Victims are typically unable to recover from dangerous situations due to impaired consciousness. Furthermore, positional asphyxia is often accidental, stemming from conditions such as intoxication and varying neurologic disorders. We report a unique case of a death of a college student due to positional asphyxia related to an underlying seizure d

[Clinical judgement and decision making in practice. A patient with hemoptysis].

A 43-year-old man was admitted because of severe and recurring haemoptysis, which was eventually ascribed to Goodpasture's syndrome. In case of an aspecific sign such as haemoptysis experienced clinicians will try to restrict the differential diagnosis by concentrating on other signs that are relevant to the main problem. In haemoptysis these other signs are general illness, fever, shortness of breath, painful respiration, chronic productive cough, and signs or a history of cardiac or pulmonary disease. If the differential diagnosis consists mainly of rare disorders or rare manifestations of c

[The role of hyperventilation in symptom formation in patients with autonomic paroxysms].

Analysis of the psychophysiological characteristics was made in patients suffering from vegetative paroxysms of the psychogenic nature. The patients were distributed into 2 groups: group I included patients (n = 35) with a well-defined hyperventilation syndrome (HVS) in the structure of vegetative disorders, group II was made up of patients (n = 23) with the same vegetative disorders without the HVS. 20 healthy subjects served as control. The clinical and psychological assays and analysis of the vegetative characteristics including respiration revealed common mechanisms in both the groups. Bes

Ondine's curse: therapeutic trial of dimefline.

Ondine's curse is a rare disorder characterized by primary failure of central regulation of breathing, resulting in severe hypoventilation during sleep. This report describes a 14-year-old girl with both this disorder and hypergonadotropic hypogonadism. As a therapeutic trial, dimefline was administered orally, which was effective in deepening respiration for certain periods of sleep, during which time blood gas levels improved significantly.

Chondroectodernal dysplasia in an African child: a case report.

Chondroectodermal dysplasia is a rare autosomal recessive disorder characterized by short extremities, short stature, polydactyly, and nail hypoplasia. Dental abnormalities, congenital heart defects, and thoracic abnormalities impending respiration are seen in many patients. Virtually all previous reports of the classic disorder have been in Caucasians. In the following case report, chondroectodermal dysplasia is described in a black Cameroonian infant.

Prolonged periodic breathing: significance in sleep studies.

Periodic breathing (PB) is a pattern of breathing that is frequently recognized in infants being studied for possible sleep apnea. Infants presenting to a sleep laboratory over a 3 1/2-year period who had evidence on their initial study of prolonged (greater than 15% of total sleep time) PB were prospectively studied in an effort to determine the significance of this pattern of breathing. Of the 331 infants studied, 40 demonstrated prolonged PB. Sixteen of these infants, who were of 37 weeks' gestation or greater at birth and did not receive pharmacologic therapy, were studied on at least two

Glutathione Metabolism and the Novel Role of Mitochondrial GSH in Retinal Degeneration.

Glutathione (GSH) is present ubiquitously, and its role as a crucial cellular antioxidant in tissues, including the retina, is well established. GSH's antioxidant function arises from its ability to scavenge reactive oxygen species or to serve as an essential cofactor for GSH S-transferases and peroxidases. This review summarizes the general functions, retinal distribution, disorders linked to GSH deficiency, and the emerging role for mitochondrial GSH (mGSH) in retinal function. Though synthesized only in the cytosol, the presence of GSH in multiple cell organelles suggests the requirement fo

The interaction between breathing and swallowing in amyotrophic lateral sclerosis.

The aim of the study is to determine the association between respiratory swallow patterns in amyotrophic lateral sclerosis (ALS) patients. Furthermore, it aims to clarify the role of the dysphagia limit in defining the relationship between swallowing disorders and respiratory disorders. Functional rating scales were used to describe swallowing and respiratory function. Swallowing was observed using the dysphagia limit. Dysphagia limit is the volume at which a second or more swallows are required to swallow the whole bolus. Laryngeal and chest movement sensors, pulmonary function tests, subment

A Systematic Literature Review of the Natural History of Respiratory, Swallowing, Feeding, and Speech Functions in Spinal Muscular Atrophy (SMA).

BACKGROUND: Respiratory and bulbar dysfunctions (including swallowing, feeding, and speech functions) are key symptoms of spinal muscular atrophy (SMA), especially in its most severe forms. Demonstrating the long-term efficacy of disease-modifying therapies (DMTs) necessitates an understanding of SMA natural history. OBJECTIVE: This study summarizes published natural history data on respiratory, swallowing, feeding, and speech functions in patients with SMA not receiving DMTs. METHODS: Electronic databases (Embase, MEDLINE, and Evidence-Based Medicine Reviews) were searched from database inc

Maternal pre-eclampsia serum increases neurite growth and mitochondrial function through a potential IL-6-dependent mechanism in differentiated SH-SY5Y cells.

Introduction: Pre-eclampsia (PE) is a common and serious hypertensive disorder of pregnancy, which affects 3%-5% of first-time pregnancies and is a leading cause of maternal and neonatal morbidity and mortality. Prenatal exposure to PE is associated with an increased risk of neurodevelopmental disorders in affected offspring, although the cellular and molecular basis of this increased risk is largely unknown. Methods: Here, we examined the effects of exposure to maternal serum from women with PE or a healthy uncomplicated pregnancy on the survival, neurite growth and mitochondrial function of

Lower respiratory rate during sleep in children with angelman syndrome compared to age-matched controls.

BACKGROUND: Angelman syndrome (AS) is a rare genetic neurodevelopmental disorder caused by the absence of a functional UBE3A gene, leading to developmental, behavioral, and medical challenges. Sleep disturbances, including sleep-disordered breathing, are common in AS. This study, for the first time, investigates nocturnal respiration in individuals with AS and healthy controls at home in a long term setting. METHODS: A non-invasive ballistocardiography-based (BCG) sleep monitoring device ("sleep mat") placed under the participants' mattresses, was used to remotely monitor children with AS age

[Effect of naloxone and thyroliberin on respiration in acute hypoxia].

Experiments on 48 adult rats anesthetized with urethane were made to study the effects of naloxone and thyrotropin-releasing hormone on respiration after acute hemorrhage. The drugs were injected intravenously in doses 0.25-0.30 mg/kg. The changes found in the electromyogram of the diaphragm, blood pressure and ECG demonstrated naloxone and thyrotropin-releasing hormone to stimulate the respiratory center at different stages of respiratory disorders (dyspnea, apnea, gasping) induced by hemorrhage.

Mitochondrial respiratory chain Complex I defects in Fanconi anemia complementation group A.

Fanconi anemia (FA) is a rare and complex inherited blood disorder of the child. At least 15 genes are associated with the disease. The highest frequency of mutations belongs to groups A, C and G. Genetic instability and cytokine hypersensitivity support the selection of leukemic over non-leukemic stem cells. FA cellular phenotype is characterized by alterations in red-ox state, mitochondrial functionality and energy metabolism as reported in the past however a clear picture of the altered biochemical phenotype in FA is still elusive and the final biochemical defect(s) still unknown. Here we r

Whole-mount Staining of Mouse Diaphragm Neuromuscular Junctions.

The neuromuscular junction (NMJ) is a specialized synapse that connects the terminal end of a motor neuron and a skeletal muscle fiber. Defects in NMJ cause abnormalities of neuromuscular transmission, leading to NMJ disorders. The mammalian diaphragm muscle is essential for respiration and has been widely used to study NMJ formation. Here, we provide a simple and straightforward protocol for preparing diaphragms from embryonic, neonatal, and adult mice and for subsequent NMJ staining.

Robust Estimation of Respiratory Variability Uncovers Correlates of Limbic Brain Activity and Transcutaneous Cervical Vagus Nerve Stimulation in the Context of Traumatic Stress.

OBJECTIVE: Variations in respiration patterns are a characteristic response to distress due to underlying neurorespiratory couplings. Yet, no work to date has quantified respiration pattern variability (RPV) in the context of traumatic stress and studied its functional neural correlates - this analysis aims to address this gap. METHODS: Fifty human subjects with prior traumatic experiences (24 with posttraumatic stress disorder (PTSD)) completed a ∼3-hr protocol involving personalized traumatic scripts and active/sham (double-blind) transcutaneous cervical vagus nerve stimulation (tcVNS).
