Venous blood gas and lactate values in adult giraffe trained for voluntary jugular phlebotomy.

Blood gas analyses have previously been performed for giraffe under anesthesia and with physical restraint in a chute; however, reference values for giraffe that are trained to participate in venipuncture without chemical or physical restraint have not previously been described. Using a point-of-care blood gas and lactate analyzer and 20 giraffe trained for voluntary phlebotomy, values were determined for venous blood gasses and plasma lactate concentration. Ninety-five percent confidence in the mean values for venous pH (7.34-7.46), partial pressure of carbon dioxide (PCO2; 28.5-37.8 mmHg), p

Use of the least intrusive coercion at Danish psychiatric wards: A register-based cohort study of 131,632 first and subsequent coercive episodes within 35,812 admissions.

OBJECTIVE: Psychiatric legislation in Denmark implies a principle of using the least intrusive types of coercion first. The intrusiveness is not universally agreed upon. We examined the order in which coercive measures during admission were used, implying that the first used should be less intrusive than the following types. METHODS: For coercive episodes reported to the national administrative register for the period 2011-16, the order of 12 legal coercive interventions during each admission was examined. Comparing with mechanical restraint, the odds ratio (OR) and confidence interval (95%CI

[Pharmacological Effects of Yokukansankachimpihange on Urinary Catecholamine in Restraint-stressed Mice].

Herbal medicines, acupuncture and moxibustion are often used for unidentified complaints. It is well known that catecholamine secreted by the sympatho-adrenal medullary system primarily functions to increase cardiac output and raise glucose levels in the blood during acute stress. In the present study, the effects of yokukansankachimpihange (YKSKCH, a Kampo medicine) on urinary catecholamine in mice that were repeatedly stressed by restraining were examined. Restraint stress (240 min/d×3 d×3 cycles, daytime: 12:00-16:00) induced a marked increase in noradrenaline (NA) and adrenaline (A) levels

Reducing Inequities in Restraint Use in the Pediatric Emergency Department.

BACKGROUND AND OBJECTIVES: Restraints are used in emergency departments to treat acute agitation and prevent harm but can cause injury. Local data demonstrated racial and ethnic disparities in restraint use. Of non-Hispanic Black and Hispanic patients, 2.7% were physically restrained, compared with 1.1% of non-Hispanic White patients. Intramuscular chemical restraint rates were highest among non-Hispanic Black patients at 6.0%, compared with 3.7% in Hispanic patients and 2.6% in non-Hispanic White patients. We sought to reduce these differences to less than 1% within 12 months. METHODS: We id

Measurement of hyper- and hypotension during repeated dose toxicity studies in either freely moving or physically restrained cynomolgus monkeys.

INTRODUCTION: The measurement of cardiovascular endpoints in standard toxicology studies remains a challenge as the routinely used non-invasive methods require physical restraint, causing an increase of sympathetic neural activity, leading to excitement and potentially hypertension in the experimental animals. In this study, a miniature telemetry blood pressure transmitter was used to evaluate if the acute hyper- and hypotension could be detected in free moving cynomolgus monkeys as well as physically restrained animals using positive control drugs. Furthermore, as a comparator, routine high d

Punishment or therapy. Patients, doctors, and somatic remedies in the early twentieth century.

Although it has been argued that psychiatrists entered the modern era with the introduction of shock therapies and lobotomy in the 1930s and antipsychotic drugs in the 1950s, practicing psychiatrists of the 1910s and 1920s did not feel they were in the dark ages of therapeutics. These early twentieth-century psychiatrists had a variety of somatic remedies at their disposal. For example, they had a plethora of sedatives and hypnotic agents from which to choose, although these drugs too often produced troublesome side effects, and, from the point of view of these physicians, too closely resemble

Heterogeneity, equifinality and risk: Profiles of restrained patients presenting to a general medical hospital.

BACKGROUND: Physical restraints refer to the immobilization of patient movement to address safety concerns or manage treatment interference. Interventions to reduce restraints have been effective in long-term care and psychiatric facilities. In contrast, rates of restraint have increased in general medical hospitals, possibly because of high patient volume and heterogenous patient populations. This high level of heterogeneity makes it difficult to develop preventative interventions. The current study aims to address this heterogeneity by generating profiles of restrained patients. METHODS: La

Prehospital chemical restraint of a noncommunicative autistic minor by law enforcement.

When responders are dealing with an agitated patient in the field, safety for all involved may sometimes only be accomplished with physical or chemical restraints. While experiences using chemical restraint in the prehospital setting are found in the medical literature, the use of this by law enforcement as a first-response restraint has not previously been described. We report a case of successful law enforcement-administered sedation of a noncommunicative, autistic, and violent minor using intramuscular droperidol and diphenhydramine. Although this case has some unique characteristics that a

[Development and evaluation of a dementia and delirium nursing education program using workshops and regular case conferences in acute-care hospitals].

AIM: To develop a dementia and delirium nursing education program for nurses in acute care hospitals with the aim of enhancing their practical competencies. The program, based on person-centered care, included a 60-minute workshop and regular case conferences held two to three times per week. This study aimed to evaluate the effects of the program by examining changes in nurses' knowledge and self-efficacy regarding dementia and delirium nursing, as well as patient outcomes, including falls, self-removal of tubes or catheters, and the use of physical restraints. METHODS: The program was condu

Forebrain overexpression of type 1 adenylyl cyclase promotes molecular stability and behavioral resilience to physical stress.

The ability to cope with stress is essential for emotional stability and mental health. It is also hypothesized that factors promoting resilience to stress may offer treatment strategies for maladaptive disorders such as anxiety and depression. Here, we find that physical restraint reduces the expression of type 1 adenylyl cyclase (Adcy1), a neurospecific synaptic enzyme that positively regulates the cAMP signaling cascade. Conversely, an increase of forebrain Adcy1 expression in transgenic mouse (i.e., Adcy1 tg mouse) predisposes individuals to molecular stability and behavioral resilience. T

Characteristics of pediatric behavioral health emergencies in the prehospital setting.

OBJECTIVE: Approximately 10% of emergency medical services (EMS) encounters in the United States are behavioral health related, but pediatric behavioral health EMS encounters have not been well characterized. We sought to describe demographic, clinical, and EMS system characteristics of pediatric behavioral health EMS encounters across the United States and to evaluate factors associated with sedative medication administration and physical restraint use during these encounters. METHODS: We conducted a retrospective cross-sectional study of pediatric (<18 years old) behavioral health EMS encou

Intranasal sedation with diazepam-butorphanol in yellow-legged gulls (Larus michahellis): a preliminary clinical evaluation.

BACKGROUND: Yellow-legged gulls (Larus michahellis) are frequently presented to wildlife clinics, often requiring safe and effective sedation and analgesia for diagnostic procedures. Minimising stress and restraint during handling are critical for both animal welfare and staff safety. To address this need, this study evaluates the first intranasal protocol in gulls, aiming to facilitate routine examination and diagnostic imaging without physical restraint. A total of 16 gulls were included in this prospective, preliminary clinical evaluation. A combination of diazepam (8 mg/kg) and butorphanol

Restraint use for psychiatric patients in the pediatric emergency department.

BACKGROUND: Restraint of children and adolescents with psychiatric problems has generated controversy. Restraint may be defined as methods used to restrict movement or normal access to one's body. Published data to guide restraint practice are limited. OBJECTIVE: To examine frequency of restraint use and patient demographics and chief complaints associated with restraint use in our pediatric emergency department. METHODS: We retrospectively reviewed the pediatric emergency department records of children (age <18 years) who underwent emergency psychiatric evaluation in an urban general hospit

The shoulder: taking the strain during restraint.

The decision to use or utilize physical intervention techniques is a contentious one. There has been much discussion on the complex legal and ethical dimensions framing the use of force. The risk of injury has also been considered in detail, but almost all of the published work has focused on restraint asphyxia, the prone restraint hold and the use of particular pain compliance techniques such as the wrist flexion hold. This paper focuses on the structure and function of the shoulder and examines how physical interventions and a variety of risk factors can threaten its physical integrity. This

Incidence and type of restrictive practice use in nursing homes in Ireland.

BACKGROUND: Use of restrictive practices (RP) in care settings may sometimes be warranted but can also conflict with human rights. Research to date has focused primarily on physical and chemical RP, however other forms are also used. Better understanding of practice can inform RP reduction. This study describes the incidence of all types of RP use reported from nursing homes in Ireland. METHODS: RP notifications from nursing homes reported in 2020 were extracted from the Database of Statutory Notifications from Social Care in Ireland. The primary outcome measurement was the national incidence

[Contain psychomotor agitation in somatic units : implementation of a protocol at the HUG].

Psychomotor agitation in somatic units is a medical and psychiatric emergency requiring rapid intervention by the team in charge of the patient and the liaison psychiatrist. Verbal de-escalation is attempted throughout the treatment. Securing the surroundings is needed to protect both the patient and the team. The etiology of the agitation, somatic and/or psychiatric, should be investigated to allow the administration of an adequate treatment. Antipsychotics and benzodiazepines are often recommended. Physical restraint measures can be applied as a last resort if necessary. The continuation of

'Excited Delirium', acute behavioural disturbance, death and diagnosis.

In the 1980s the traditional Hippocratic term excited delirium was transplanted from the bedsides of febrile, agitated and disoriented patients to the streets of Miami. Deaths in custody of young men who were intoxicated with cocaine and who were restrained by the police because of their erratic or violent behaviour were attributed to excited delirium. The blood concentrations of cocaine in these subjects were approximately ten times lower than the lethal level and other factors which might have contributed to the fatal outcome, such as the police use of neck-holds, choke-holds or 'hog-tying',

Commentary: Interventions based on learning principles can supplant seclusion and restraint.

Regulatory and administrative imperatives, when paired with staff training in humanistic, verbally mediated interventions can reduce the frequency of seclusion and restraint but can be associated with increases in the frequency and severity of staff injury, with concomitant tension and apprehension in the treatment setting. Even when educational programs for staff are made available for patient-centered, therapeutic, and persuasion-based modes of de-escalation, aggression, destructiveness and self-injury may continue to occur or even increase. Administrative contingencies can lead to less repo
