Ocular blood flow and retinal oxygen saturation measurements in central retinal artery occlusion.

OBJECTIVES: To provide numerical quantitative measurements of blood flow velocity in the optic nerve head (ONH) and choroid and retinal vascular oxygen saturation in patients with central retinal artery occlusion (CRAO). METHODS: Twelve consecutive patients (12 eyes) underwent laser speckle flowgraphy (LSFG) and retinal oximetry measurements at presentation. The normal fellow eyes were evaluated and used as controls. RESULTS: In eyes with CRAO, the ONH mean blur rate (MBR)-vessel (25 ± 15 AU), representing blood flow velocity in the large vessels originating from the central retinal artery, …

Prevalence of Retinal Emboli and Acute Retinal Artery Occlusion in Acute Ischemic Stroke.

OBJECTIVE: In population-based studies asymptomatic retinal emboli occur in .32%-2.9% of people. Retinal artery occlusion (RAO) may occur concurrently with cerebral stroke but the frequency is unknown. No study has examined how commonly retinal emboli occur in the acute stroke population. We aimed to assess the prevalence of retinal emboli and RAO at the time of carotid territory ischemic stroke. METHODS: Patients were enrolled prospectively after onset of symptoms consistent with the diagnosis of carotid territory ischemic stroke. Every participant underwent pharmacologic dilation of both pu…

Retinal Endovascular Surgery with Tissue Plasminogen Activator Injection for Central Retinal Artery Occlusion.

PURPOSE: To report 2 cases of central retinal artery occlusion (CRAO) who underwent retinal endovascular surgery with injection of tissue plasminogen activator (tPA) into the retinal artery and showed a remarkable improvement in visual acuity and retinal circulation. METHODS: Standard 25-G vitrectomy was performed under local anesthesia. Simultaneously, tPA (80,000 units/mL) solution was injected into the retinal artery of the optic disc for 2-3 min using a microneedle. Changes in visual acuity, fundus photography, optical coherence tomography (OCT), fluorescein angiography, and laser speckle…

[A case of central retinal vein occlusion followed by branch retinal artery occlusion].

BACKGROUND: The influence of retinal artery circulation on central retinal vein occlusion (CRVO) is not well known. CASE: A 73-year-old man with hypertension complained of transient visual loss in his left eye. At initial examination, ophthalmoscopy of his left eye showed mild dilatation and tortuosity of the retinal vein and a few dot hemorrhages. Fluorescein angiography disclosed normal arm-to-retina circulation time and no leakage of dye, but delayed retinal circulation time. These findings indicated venous stasis retinopathy of CRVO. In spite of intravenous infusion of prostaglandin E1 an…

[A case of central retinal vein occlusion accompanied by central retinal artery occlusion].

A 81-year-old woman developed central retinal vein occlusion (CRVO) in her left eye subsequent to central retinal artery occlusion (CRAO). She noticed sudden visual loss in the left eye. At initial examination, her left visual acuity was 0.03, and only a small visual field was preserved at temporal area. The left fundus showed findings of mild non-ischemic CRVO. In addition white clouded retina was recognized at the left posterior pole which indicated CRAO. FAG showed remarkably prolonged arterial circulation, but no retinal capillary obliteration. Then retinal hemorrhage increased rapidly and…

A case series on treatment of central and branch retinal artery occlusion with intravenous tenecteplase: Tenecteplase for retinal artery occlusions.

OBJECTIVES: Central and branch retinal artery occlusions are disabling ischemic strokes of the retina for which established acute treatments are lacking. This is the first published report of the use of intravenous tenecteplase (TNK) for retinal artery occlusion, in which we describe the clinical course of four patients with central retinal artery occlusion (CRAO) and one patient with branch retinal artery occlusion (BRAO). MATERIALS AND METHODS: Patients were retrospectively recruited to the study from two stroke centers. Clinical course was determined from review of electronic medical recor…

Incidence of Retinal Artery Occlusion After Carotid Endarterectomy Versus Carotid Artery Stenting.

PURPOSE: Retinal artery occlusion (RAO) is a rare but vision-threatening complication of carotid revascularization, yet its comparative incidence after carotid endarterectomy (CEA) versus carotid artery stenting (CAS) is poorly characterized. We compared the risk of RAO and associated neurologic outcomes after CEA and CAS. METHODS: Retrospective multicenter cohort study using the TriNetX Research Network (January 1, 2004-March 15, 2026). Adults undergoing CEA or CAS with ≥6 months of follow-up and no prior diagnosis of any studied outcome condition (retinal artery or vein occlusion, glaucoma,…

Fusiform Left Carotid-Ophthalmic Artery Aneurysm Presenting with Central Retinal Artery Occlusion.

Central retinal artery occlusion (CRAO) is a devastating ocular emergency characterized by acute painless visual loss in the ipsilateral eye. We describe the case of acute non-arteritic CRAO associated fusiform internal carotid-ophthalmic artery aneurysm with intraluminal thrombus. Despite the rarity of this condition, we suggest that carotid-ophthalmic artery aneurysm should be included in the differential diagnosis of CRAO.

The association of central retinal artery occlusion and extracranial carotid artery disease.

To determine the incidence of associated carotid artery disease and the effect of carotid endarterectomy on subsequent neurologic sequelae, a retrospective study of 66 patients with central retinal artery occlusion (CRAO) was undertaken. Ipsilateral extracranial carotid artery disease was present in 23 of 33 patients (70%) who had carotid arteriography. Sixteen patients had carotid endarterectomy following their CRAO (Group I) and 50 did not (Group II). Seven of the 40 patients available for follow-up in Group II had a subsequent stroke (mean follow-up: 54 months). Of the seven Group II patien…

Retinal artery pressure and cerebral artery perfusion pressure in cerebrovascular occlusive disease.

The objectives of the investigation were to measure the retinal artery pressure (RAP) and cortical artery pressure (CAP) in patients undergoing superficial temporal artery to middle cerebral artery (STA-MCA) bypass, to study the relationship between these pressures, and to evaluate our ability to predict CAP on the basis of RAP. The 44 patients undergoing bypass surgery included 26 with ipsilateral internal carotid artery (ICA) occlusion (Group I), 5 with bilateral ICA occlusion (Group II), 4 with inaccessible ICA stenosis proximal to the ophthalmic artery (OA) (Group III), 2 with ICA stenosis…

Selective retinal arterial whitening in retinal astrocytic hamartoma in tuberous sclerosis.

This case report aims to describe and understand the mechanism behind the selective retinal arterial whitening in a case of retinal astrocytic hamartoma (RAH) associated with tuberous sclerosis complex (TSC). Multimodal imaging of a late-adolescent female with TSC-associated RAH demonstrated unique vascular features including selective whitening of the retinal arteries with sparing of the retinal veins. Genetic testing confirmed a TSC2 (tuberin) gene mutation, known to affect the mammalian target of rapamycin (mTOR) signalling pathway. To understand the mechanism behind selective arterial invo

Branch retinal arterial occlusions in multifocal retinitis with optic nerve edema.

OBJECTIVE: To determine the natural history and visual prognosis of patients with branch retinal arterial occlusions secondary to multifocal retinitis. METHODS: Cases were reviewed for seven patients who exhibited multifocal retinitis and branch retinal arterial occlusion. The average age of the patients was 27 years (age range, 14 to 19 years). RESULTS: Six patients had systemic illnesses associated with their ocular findings. Four patients were scratched by a cat or exposed to a cat with fleas within 1 month of symptoms. Three of these patients were tested and had positive cat-scratch dise

Retinal artery occlusion during carotid artery stenting with distal embolic protection device.

Retinal artery occlusion associated with carotid artery stenosis is well known. Although it can also occur at the time of carotid artery stenting, retinal artery occlusion via the collateral circulation of the external carotid artery is rare. We encountered two cases of retinal artery occlusion that were thought to be caused by an embolus from the external carotid artery during carotid artery stenting with a distal embolic protection device for the internal carotid artery. A 71-year-old man presented with central retinal artery occlusion after carotid artery stenting using the Carotid Guardwir…

Selective thrombolysis performed through meningo-ophthalmic artery in central retinal artery occlusion.

The poor natural history of central retinal artery occlusion (CRAO) is usually not modified with conventional, conservative management techniques. Guidelines for selective intraarterial ophthalmic thrombolysis are still lacking. While many centers continue to perform this procedure with promising results, others are reluctant due to conflicting findings in recent studies. We present our experience in a 36-year-old male with CRAO. Based on the patient's clinical presentation, we planned to perform selective intraarterial ophthalmic thrombolysis via the ophthalmic artery. When angiography demons…
