[Kawasaki's syndrome in the adult and bilateral sacroiliitis].

We present the case of a 23 year old patient with Kawasaki syndrome which resolved without sequelae after salicylate treatment. An peculiar finding was that of typical symptoms and signs of bilateral sacroiliitis demonstrated by symmetrically increased articular uptake of technetium 99 diphosphonate. This sacroiliitis disappeared completely after resolution of the syndrome. HLA-B27 ag was negative. Articular affection in Kawasaki syndrome comprises pain and/or swelling of large and small peripheral joints. Acute sacroiliitis is a finding not previously described. We believe this to have been a…

Magnetic resonance imaging in the detection of sacroiliitis.

The value of magnetic resonance imaging (MRI) in establishing the diagnosis of sacroiliitis was studied in 20 patients with established or suspected disease on conventional radiographs and in 10 healthy subjects. Coronal T1 weighted, axial T2 weighted and proton density MRI images of the sacroiliac joints in addition to plain film radiographs were obtained. All films were graded from 0 to 4 according to the modified New York criteria and independently for changes in joint width, erosions, sclerosis and ankylosis. Using the modified New York criteria, more abnormalities were detected by MRI tha…

MRI Findings in a Dog with Unilateral Sacroiliitis.

Sacroiliitis is a rare pathology recently reported in the veterinary literature, mainly in large-breed dogs. The main associated clinical signs are lumbosacral pain and/or lameness of one or both hindlimbs. However, it is a nonspecific presentation and can be mistaken for other clinical disorders with a higher prevalence, such as caudal lumbar disc herniation or degenerative lumbosacral stenosis, among others. The present report describes the MRI findings in a German shepherd dog with unilateral sacroiliitis secondary to a possible trauma and the relevance of advanced imaging techniques in the…

Sacroiliitis: A Review on Anatomy, Diagnosis, and Treatment.

INTRODUCTION: Sacroiliitis is an inflammation of one or both of the sacroiliac (SI) joints, most often resulting pain in the lower back that can extend down the legs. Pain arising from the SI joint can be difficult to diagnose and treat due to the intricate surrounding ligamentous structure, nerve innervation, and its role in transferring weight from the upper body to the lower limbs. SI joint dysfunction accounts for up to 25% of cases of lower back pain and has a debilitating effect on patient functionality. This review aims to provide comprehensive coverage of all aspects of SI joint pain, …

Etiopathogenesis of sacroiliitis: implications for assessment and management.

The sacroiliac joints connect the base of the sacrum to the ilium. When inflamed, they are suspected to cause low back pain. Inflammation of the sacroiliac joints is called sacroiliitis. The severity of the pain varies and depends on the degree of inflammation. Sacroiliitis is a hallmark of seronegative spondyloarthropathies. The presence or absence of chronic sacroiliitis is an important clue in the diagnosis of low back pain. This article aims to provide a concise overview of the anatomy, physiology, and molecular biology of sacroiliitis to aid clinicians in the assessment and management of …

Sacroiliitis in Children With Familial Mediterranean Fever.

BACKGROUND/OBJECTIVE: Familial Mediterranean fever (FMF) is an autosomal recessive disease, characterized by recurrent, self-limited attacks of fever with serositis. Various diseases were reported to be associated with FMF. The aim of this study was to investigate the frequency and characteristics of sacroiliitis in children with FMF. METHODS: Files of FMF patients who had been seen in 2 reference hospitals in Ankara were retrospectively evaluated. Patients with FMF and concomitant sacroiliitis were included to the study. All patients had magnetic resonance imaging evidence of sacroiliitis. …

Brucella canis sacroiliitis and discospondylitis in a dog.

A 1.5-year-old neutered male Labrador retriever dog was presented with a 1-month history of pelvic limb lameness. Physical examination revealed lumbosacral and pelvic pain. Diagnostic imaging findings were consistent with discospondylitis and bilateral sacroiliitis. Brucellosis was definitively diagnosed with rapid slide agglutination (RSAT) and agar gel immunodiffusion (AGID) tests. Brucella canis has not been associated with sacroiliitis and should be included as a differential diagnosis for dogs presented with clinical or radiographic signs of sacroiliitis.

The prevalence of sacroiliitis and spondyloarthritis in patients with sarcoidosis.

Introduction. Sarcoidosis is a chronic granulomatous disease, which can involve different organs and systems. Coexistence of sarcoidosis and spondyloarthritis has been reported in numerous case reports. Purpose. To determine the prevalence of sacroiliitis and spondyloarthritis in patients previously diagnosed with sarcoidosis and to investigate any possible relation with clinical findings. Materials and Methods. Forty-two patients with sarcoidosis were enrolled in the study. Any signs and symptoms in regard to spondyloarthritis (i.e., existence of inflammatory back pain, gluteal pain, uveitis,…

Pyogenic sacroiliitis in children-a diagnostic challenge.

Osteoarticular infections in paediatric patients are associated with significant morbidity. Pyogenic sacroiliitis is rare and accounts for approximately 1-2% of osteoarticular infections in children. Diagnosis of this disease has been difficult in the past due to its deep location and may be delayed due to the lack of specific clinical signs and symptoms. We identified 11 paediatric patients with clinical-radiological signs of pyogenic sacroiliitis during an 8-year period and observed an unusual cluster of four cases during the last 11 months. Early diagnosis was possible due to a reproducible…

Occurrence of sacroiliitis in patients with seronegative oligoarthritis.

The aim of this study was to investigate sacroiliitis in patients with seronegative oligoarthritis. Thirty consecutive patients with seronegative oligoarthritis and no other signs of spondylarthropathy were included. Sacroiliac (SI) joints were investigated by both radiography and magnetic resonance imaging. HLA B27 antigen was studied and family history was reexamined in 2006. Five patients had sacroiliitis. Additionally, 15 patients had HLA B27 antigen or family history of either psoriasis or ankylosing spondylitis. Our conclusion is that during the first decade of seronegative oligoarthriti…

[Grading sacroiliitis with emphasis on MRT-imaging].

Cross-sectional imaging techniques play a decisive role in identification, localization, and characterization of alterations in the sacroiliac joint during the early stage of seronegative spondylarthropathy (SpA). Although several studies showed that the diagnostic capabilities of MRI and CT are superior to those of conventional radiography, they have not yet become established and accepted as methods for evaluating the grade of ankylosing spondylitis (AS) in contrast to conventional radiography. The lack of acceptance for MRI and/or CT methods for evaluating and grading changes in the sacroil
