INTRODUCTION: Avascular necrosis of bone, due to various etiologies, is not an uncommon skeletal pathology seen in clinical practice. Treatment of these conditions includes treating the resultant pain, stiffness, and limitations of movement surgically with bone grafts both vascularized and non-vascularized and arthroplasty in most advanced stages.

A 4/5 compartment pedicled vascularised bone graft from the distal radius combined with internal fixation with a Herbert type cannulated screw was used to treat non-union fractures of the proximal pole of the scaphoid in 13 patients. Authors analyse, based on an all-round inquiry, the operations performed for pseudarthrosis of the scaphoid in 1988. It was found that 344 of 472 operations were reconstructive, 128 palliative. The single types of operation are briefly described. They hint at the fact that the operative treatment of scaphoid pseudarthrosis in Hungary is carried out mostly in Centers.

Fractures of the scaphoid bone are nowadays better known and treated. However, the types of fractures and fragment dislocations are not well described. The authors analysed 325 fresh and old scaphoid fractures and describe six basic types, clearly defined by location and direction of the fracture line. In fresh cases, the dislocation is mainly rotational. In older cases there is an additional bone resorption. The six basic types of fracture lines are demonstrated radiologically and the course of untreated cases is shown.

Fifty patients with radiographic evidence of an acute scaphoid fracture were studied prospectively. Technetium bone scans were performed on all patients soon after injury in an attempt to detect fractures at risk of non-union or avascular necrosis.
