Protean Neurologic Manifestations of Two Rare Dermatologic Disorders: Sweet Disease and Localized Craniofacial Scleroderma.

PURPOSE OF REVIEW: To describe diverse neurologic and neuroradiologic presentations of two rare, immunologically mediated skin conditions: Sweet disease and localized scleroderma (morphea). RECENT FINDINGS: Core syndromes of neuro-Sweet disease (NSD) are steroid responsiveness, recurrent meningitis, and encephalitis. Focal neurologic, neuro-vascular, and neuro-ophthalmologic syndromes have been reported recently in NSD. A variety of steroid-sparing treatments and biologics have been used for relapsing NSD. Localized craniofacial scleroderma is associated with seizures, headaches, and, less co

Impact of a new simplified disability scoring system for adult patients with localized scleroderma.

Localized scleroderma (LoS) involves dermal but not internal inflammation and fibrosis. Cosmetic changes often impact quality of life (QOL), however, impairment of activities of daily living (ADL) in LoS patients has not been investigated. To determine what factor(s) are associated with ADL in adult patients with LoS, we performed a retrospective observational study in 177 Japanese adult LoS patients using a novel LoS disability score based on Barthel's indices of ADL: feeding, bathing, grooming, dressing, bowels, bladder, toilet use, transfers, mobility and stairs. LoS disability scores incre

Longstanding epileptic encephalopathy and linear localized scleroderma: two distinct pathologic processes in an adolescent.

We report a 7-year-old boy who unexpectedly developed a multi-drug resistant epilepsy with negative neuroimaging results, followed by the insidious appearance of linear localized scleroderma involving the right leg. When the boy was 16 and severely affected by epileptic encephalopathy, we have evaluated this case for the first time: his localized scleroderma had reached the right buttock and positive anti-nuclear antibody was the only positive laboratory test. Methotrexate administered for 12 months was ineffective in improving both the organization of his electroencephalographic pattern and s

Oesophageal motility and lower oesophageal sphincter competence in progressive systemic sclerosis and localized scleroderma.

Oesophageal motility and lower oesophageal sphincter (LOS) competence were investigated in 13 patients with progressive systemic sclerosis (PSS) and in 16 patients with localized scleroderma (LS) by means of oesophageal manometry and 24-h pH monitoring of the distal oesophagus. Results were compared with those of a control group consisting of asymptomatic volunteers. Marked abnormalities in oesophageal motility and in acid exposure in the distal oesophagus were observed in PSS patients only. The mean resting pressure of the LOS was 10.1 +/- 1.5 mmHg in PSS, 21.4 +/- 1.1 mmHg in LS, and 23.8 +/

Neurological involvement in localized scleroderma: a systematic review of clinical manifestations and neuroimaging findings.

BACKGROUND: Localized scleroderma (LoS) is an autoimmune disorder leading to skin sclerosis and subcutaneous tissue atrophy. Apart from the disfiguring craniofacial lesions, the extracutaneous involvement in LoS has been increasingly recognized and studied. Among all, neurological involvement is common and worth special attention. OBJECTIVES: This systematic review summarizes the neurological symptoms, neuroimaging findings, and other complementary examination results of LoS. METHODS: Records were retrieved using keywords related to the disease, neurological symptoms, and neurological examin

Parry-Romberg Syndrome With Localized Scleroderma: A Report of Two Pediatric Cases From Oman.

Parry-Romberg syndrome (PRS) is a rare, acquired neurocutaneous disorder characterized by progressive hemifacial atrophy, primarily affecting the skin, soft tissue, and underlying bone. The etiology remains unclear, though autoimmune, inflammatory, and genetic factors have been implicated. PRS frequently coexists with localized scleroderma (en coup de sabre), leading to diagnostic and therapeutic challenges. Here, we present two pediatric cases from tertiary care hospitals in Oman, emphasizing clinical presentation, diagnostic findings, and treatment response. The first patient, a 10-year-old

Fat Grafting Reduces Skin Hyperpigmentation of Localized Scleroderma Patients: A Prospective Self-controlled Study.

BACKGROUND: Localized scleroderma (LS) is characterized by skin fibrosis, hyperpigmentation and soft tissue atrophy. Fat grafting has been widely used to correct LS deformity. OBJECTIVE: To investigate the effect of fat grafting on the skin pigmentation of LS lesions. METHODS: A prospective self-controlled study was conducted. Skin melanin and erythema indexes were measured by Mexameter® MX18 before and 3 months after surgery. Differences between lesions and contralateral normal sites were compared to evaluate changes induced by fat grafting. Localized Scleroderma Cutaneous Assessment Tool a

Coexistence of systemic and localized scleroderma: a systematic literature review and observational cohort study.

OBJECTIVE: SSc and localized sclerosis (LoS) are considered clinically distinct entities. We describe herein the coexistence of SSc and LoS by both a systematic literature review and an observational cohort study of unselected SSc patients. METHODS: Original studies documenting the coexistence of SSc and LoS were identified in three electronic databases by means of a systematic literature search according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines. Additionally, the coexistence of SSc and LoS was studied in a prospective cohort of SSc patient

Successful autologous hematopoietic stem cell transplantation for a patient with rapidly progressive localized scleroderma.

Autologous hematopoietic stem cell transplant (HSCT) for rapidly progressive disease has not been reported in localized scleroderma. Our patient, a 16-year-old girl had an aggressive variant of localized scleroderma, mixed subtype (linear-generalized) with Parry Romberg syndrome, with no internal organ involvement, that was unresponsive to immunosuppressive therapy and was causing rapid disfigurement. She was administered autologous HSCT in June 2011 and has maintained drug-free remission with excellent functional status at almost 3.5 years of follow-up.

Autoantibodies to the extracellular matrix microfibrillar protein, fibrillin 1, in patients with localized scleroderma.

OBJECTIVE: Serum autoantibodies to fibrillin 1, the major component of microfibrils in the extracellular matrix, recently have been reported to occur in the tight skin mouse and in patients with systemic sclerosis, but not in patients with other connective tissue diseases. This study was undertaken to determine whether antifibrillin 1 antibodies could be detected in patients with localized forms of scleroderma. METHODS: Sera from 50 patients with localized scleroderma (27 with linear scleroderma and 23 with morphea) and 51 normal controls were tested for IgG and IgM antifibrillin 1 autoantibo

Serum aminoterminal propeptide of type III procollagen in progressive systemic sclerosis and localized scleroderma.

Sera from 31 patients with progressive systemic sclerosis (PSS), 5 patients with widespread localized scleroderma (LS), and 3 patients with lichen sclerosus et atrophicus were analyzed for aminoterminal propeptide of type III procollagen (PIIINP) using a radioimmunoassay based on human propeptide. Thirty-eight per cent of the patients with PSS had levels above normal range, including all of the 3 patients with diffuse scleroderma. The same applies to 4 of 5 patients with widespread localized LS, while PIIINP in all 3 patients with lichen sclerosus et atrophicus were within normal levels. In pa

Clinical and serologic expression of localized scleroderma. Case report and review of the literature.

A patient is presented in whom both linear scleroderma and eosinophilic fasciitis developed. The latter is rare in childhood. A positive antinuclear antibody, a positive rheumatoid factor, and seizures also developed. In reviewing the literature, we concluded that the clinical and histologic pattern of cutaneous sclerosis may be more valuable than serologic findings in predicting both the likelihood of, and the expected sites for, systemic involvement. The literature also suggests that central nervous system (CNS) involvement is rare in most sclerosing syndromes but that linear scleroderma may

JAK inhibitors for the treatment of juvenile localized scleroderma: a case report and literature review.

BACKGROUND: Juvenile localized scleroderma (JLS) is a chronic autoimmune and fibrosing disease that can cause significant damage and impairments. Currently, management of JLS is challenging due to the paucity of antifibrotic drugs. In patients with severe JLS, systemic glucocorticoids(GCs) combined with methotrexate(MTX) are recommended, but some patients show intolerance or poor response. Janus kinase (JAK) inhibitors have demonstrated efficacy and tolerability in treating cutaneous thickening in vivo and in vitro. We aim to describe a JLS patient treated with upadacitinib(UPA) in our hospita

Histopathological and molecular analysis in dermis and epidermis of patients with systemic and localized scleroderma.

OBJECTIVE: Scleroderma has a wide range of clinical manifestations due to vasculopathy, autoimmunity, altered endothelium function, and abnormal fibrosis, which are accused in the pathogenesis of the disease. The aim of this study is to shed light on the pathogenesis of the disease in childhood via dermal immunohistochemical analysis of the cases. METHODS: A single-blind clinical trial is conducted with evaluation of the tissue samples obtained from patients. The samples are stained with PAS, hematoxylin and eosin, E-Cadherin, Connective tissue growth factor (CTGF), Tunnel, and staining for T

Evaluation of Thiol Disulfide, Ischemia Modified Albumin, and Prolidase Parameters in Patients with Localized Scleroderma.

INTRODUCTION: Localized scleroderma is a rare inflammatory skin disease that causes sclerosis in the dermis and subcutaneous tissue. Oxidative stress may play a role in the etiology or be responsible for the chronicity or progression of the disease. OBJECTIVES: We aimed to investigate the presence of oxidative stress in patients with localized scleroderma by examining thiol-disulfide balance, ischemia-modified albumin (IMA), and prolidase parameters. METHODS: Twenty patients over the age of 18 who were diagnosed with localized scleroderma both clinically and histopathologically and 20 contro

Narrow-band reflectance spectrophotometry and infrared thermography for assessment of skin lesions in localized scleroderma.

BACKGROUND: Infrared thermography (IRT) is a useful method to detect activity/inflammation in localized scleroderma (LoS); however, inactive skin lesions with a severe degree of dermal and subcutaneous atrophy may show false-positive results. Narrow-band reflectance spectrophotometry (NBRS) is an objective, non-invasive technique of measuring erythema and hyperpigmentation severity, yet has not been extensively studied in LoS. OBJECTIVES: The aim of this research was to compare the spectrophotometric results with thermographic examination of LoS lesions. METHODS: The lesions were assessed us

Use of a novel Indentometer to evaluate skin induration in localized scleroderma and psoriasis vulgaris.

INTRODUCTION: Diseased skin in localized scleroderma (LS) and plaque psoriasis (PPs) is characterized by induration that can be evaluated by non-invasive bioengineering methods. In this study, we applied a new measurement device based on indentometry to determine the changes of skin mechanical properties in patients with LS and PPs. MATERIAL AND METHODS: A total of 30 sclerodermatous plaques in 12 patients with LS and 46 psoriatic plaques in 19 patients with PPs were measured with Indentometer IDM 800 (Courage + Khazaka, Cologne, Germany). The device measures the penetration depth of the prob
