OBJECTIVE: Acute scrotal pain is a common urological emergency. Urgent exploration is the standard means of management, since no investigation can confidently exclude torsion of testis from the differential diagnosis.
METHODS: A review of all boys presenting with acute scrotal pain who underwent emergency scrotal exploration between January 1983 and March 2003 was performed.

Acute scrotum is a clinical syndrome which includes various disorders. Differential diagnosis of these disorders is not always easy. We reviewed cases experienced in our hospital, focusing on diagnostic procedures. Final diagnosis in 39 cases were acute epididymitis in 23 cases, torsion of the testis in 13 cases, torsion of appendix epididymis in 2 cases and torsion of appendix testis in 1 case. Two cases of Klinefelter's syndrome with prepenile scrotum were presented. They underwent the operation for prepenile scrotum according to Glenn and Anderson's method. Thereafter urethroplasty was performed in each case. The decrease in response to testosterone in the target organ was noticed in 1 patient, suggesting that some androgen resistance in this case was attributable in part to anomaly of genitalia.

Between April 1986 and July 1990, we experienced 13 cases of acute scrotum with surgical exploration. Six of the patients had torsion of the spermatic cord; three had torsion of an appendix of the epididymis, one had torsion of a testicular appendix, one had testicular rupture, one had acute epididymitis and one was normal. Testicular salvage rates are improved markedly when surgical correction of a ruptured tunica albuginea is performed within 72 hours after a scrotal injury. The authors studied 11 patients who had suffered various types of blunt or penetrating trauma to the scrotum. The importance of ultrasound for the evaluation of the traumatized scrotum and detection of a testicular rupture is stressed. The spectrum of abnormalities that may be present after a scrotal injury is described.

We present the case of a 42-year-old man with a palpable, painless, slow-growing mass, growing over a period of 12 years. The mass was completely extirpated in June 2017 with relative ease from the adjacent structures sparing the penis, testes, and epididymis. Blastomycosis dermatitidis is a fungal pathogen endemic to North America. Infection from inhalation of its conidia has diffuse clinical manifestations. These can include genitourinary system infections which may manifest as ulcerative, draining skin lesions. Pyoscrotum is rarely encountered in neonates and is secondary to a local pathology such as neglected torsion or epididymo-orchitis. Pyoscrotum from an intra-abdominal pathology is possible in the presence of a patent processus vaginalis and has been rarely reported in the context of acute appendicitis. We report our experience with a case of antenatal bowel perforation presenting postnatally after an absolutely asymptomatic "honeymoon" period.

The authors report a rare disease at the junction of urology and cardiovascular surgery (arteriovenous malformation (AVM) of the scrotum). The options for correction of this disease are discussed.
