Synergistic but separable sensory changes in postural tachycardia syndrome and chronic migraine.

PURPOSE: Up to 90% of patients with postural tachycardia syndrome (PoTS) report headaches, and comorbid migraine headaches are common. Given this, pathophysiological interaction is possible, which may reveal key aspects of disease expression and treatment opportunities. We hypothesized that PoTS subjects-both with and without migraine-would show features of central sensitization, including allodynia and photophobia. METHODS: Eighty participants were evaluated, including 30 PoTS, 30 chronic migraine (CM), and 20 non-headache healthy controls (NH), using tilt table testing, psychophysical asses

Pain mechanisms in carpal tunnel syndrome: a systematic review and meta-analysis of quantitative sensory testing outcomes.

Carpal tunnel syndrome (CTS) is the most common nerve compression in the arm. A mix of peripheral and central contributions on quantitative sensory testing (QST) has been reported in the literature. Thus, this systematic review or meta-analysis aimed to identify the dominant sensory phenotype and draw conclusive evidence about the presence of central sensitization (CS) in CTS. Based on an a priori published protocol and using PRISMA guidelines, 7 databases were searched (Embase, Web of Science, Scopus, PubMed, SAGE, EBSCOhost, and ProQuest). Eligible studies compared the QST findings of indivi

Analysis of trigeminal nerve disorders after oral and maxillofacial intervention.

BACKGROUND: Quantitative sensory testing (QST) is applied to evaluate somatosensory nerve fiber function in the spinal system. This study uses QST in patients with sensory dysfunctions after oral and maxillofacial surgery. METHODS: Orofacial sensory functions were investigated by psychophysical means in 60 volunteers (30 patients with sensory disturbances and 30 control subjects) in innervation areas of the infraorbital, mental and lingual nerves. The patients were tested 1 week, 4 weeks, 7 weeks and 10 weeks following oral and maxillofacial surgery. RESULTS: QST monitored somatosensory defi

[Assessment of the correlation between taste and smell functioning].

UNLABELLED: There are few studies examining relationship between taste and smell. However their clinical unity is more often indicated. Smell perception abnormalities cause modification in taste of food intensity, disabling perception of taste as a consequence of taste's sniffing failure. Thus disturbances of these senses, which often commonly coexist could cause great discomfort in patients, however taste sense is estimated as less important comparing to smell sense. The aim of that study was to compare the taste perception in patients with normal smell function and patients with hyposmia and

Auditory brainstem response (ABR) to tone pips in hearing-impaired children.

The correspondence between ABR thresholds to tone pips and behavioral thresholds at 500, 1000 and 2000 Hz was studied in 13 normal and 55 hearing-impaired children, including 42 sensori-neural losses and 13 conductive losses. The children's ages ranged from 1 year 3 months to 8 years. Tone pips were provided with 2-cycle rise-fall times and 2-cycle plateaus. The correlations between the two measures were highly significant at 3 frequencies both in sensori-neural losses and conductive losses. In the subjects with sensori-neural losses, the ABR thresholds tended to be lower than the behavioral t

Effects of aging on sensitivity of the pharyngeal and supraglottic areas.

As one ages, sensory discrimination in the oral cavity progressively diminishes, and dysphagia and aspiration are more likely to occur. Whether similar age-related laryngopharyngeal (LP) sensory abnormalities exist and contribute to dysphagia and aspiration is unknown. The purpose of this study was to determine if sensory discrimination in the area of the laryngopharynx innervated by the superior laryngeal nerve diminishes with increasing age. By applying a previously described device and technique that utilized endoscopically delivered air pulse stimulation of the anterior wall of the pyrifor

Quantitative sensory testing in fibromyalgia and hemisensory syndrome: comparison with controls.

Fibromyalgia syndrome (FS) is a chronic painful condition with sensory, motor and affective dysfunctions. Few studies had investigated the trigeminal area, and little is known about its association with hemisensory syndrome, which is characterized by chronic pain restricted to hemibody. Our objective was to investigate sensorial abnormalities with quantitative sensory testing of patients with FS and patients with hemisensory syndrome, compared to controls. Thirteen patients diagnosed with FS according to the American College of Rheumatology, and 12 patients with hemisensory syndrome were evalu

The neuropathy of erectile dysfunction.

These studies were intended to explore the relationship between autonomic neuropathy and erectile dysfunction (ED). Sensory thresholds reflecting the integrity of both large diameter, myelinated neurons (ie pressure, touch, vibration) and small diameter axons (ie hot and cold thermal sensation) were determined on the penis and finger. Data were compared across subjects with and without ED, controlling for age, hypertension and diabetes. The correlation of specific thresholds scores and IIEF values were also examined. Seventy-three patients who visited the academic urology clinics at Montefiore

Differences in gastric mechanosensory function after repeated ramp distensions in non-consulters with dyspepsia and healthy controls.

BACKGROUND: Abnormal visceral mechano-sensory function has been reported in 50% of non-ulcer (functional) dyspepsia patients. However, only a minority of subjects with functional dyspepsia ever seek medical attention. Whether factors promoting health care seeking behaviour explain visceral hypersensitivity is unknown. Decreased rectal thresholds following sigmoid mechanical stimulation have been observed in irritable bowel but this mechanism has not been evaluated in functional dyspepsia. AIMS: To compare visceral mechanosensory function in healthy asymptomatic subjects and non-consulters wit

Patients with painful bladder syndrome have altered response to thermal stimuli and catastrophic reaction to painful experiences.

AIMS: To compare cutaneous sensory thresholds, habituation to somatic stimuli, and tendency towards catastrophic reaction to painful stimuli in patients with Painful Bladder Syndrome (PBS) to controls without PBS. METHOD: Thermal and vibratory sensory thresholds were established in 11 PBS patients and 10 controls at C5, T1, T12, and S3 dermatomes. Supra-threshold thermal stimuli were then applied at T12 and S3 for 60 sec while patients periodically rated the intensity of stimuli using a visual analog scale. A Pain Catastrophizing Scale (PCS) questionnaire was also completed by all participant

A clinical trial on pelvic floor sensory testing in women with and without stress urinary incontinence.

OBJECTIVES: Pelvic floor sensory training is commonly used in clinical practice for the treatment of stress urinary incontinence (SUI); however, there is a lack of comparative studies evaluating pelvic floor sensation between women with SUI and healthy controls. Additionally, normative data for two-point discrimination thresholds in the female pelvic floor region remain scarce. This study aims to compare the results of 4 sensory tests in the pelvic floor region between women with mild SUI and healthy women, in order to provide reference values for two-point discrimination thresholds in this ar

Sensory processing in the auditory and olfactory domains is normal in laryngeal dystonia.

Abnormal sensory discriminatory processing has been implicated as an endophenotypic marker of isolated dystonia. However, the extent of alterations across the different sensory domains and their commonality in different forms of dystonia are unclear. Based on the previous findings of abnormal temporal but not spatial discrimination in patients with laryngeal dystonia, we investigated sensory processing in the auditory and olfactory domains as potentially additional contributors to the disorder pathophysiology. We tested auditory temporal discrimination and olfactory function, including odor id

The measurement and treatment of suppression in amblyopia.

Amblyopia, a developmental disorder of the visual cortex, is one of the leading causes of visual dysfunction in the working age population. Current estimates put the prevalence of amblyopia at approximately 1-3%(1-3), the majority of cases being monocular(2). Amblyopia is most frequently caused by ocular misalignment (strabismus), blur induced by unequal refractive error (anisometropia), and in some cases by form deprivation. Although amblyopia is initially caused by abnormal visual input in infancy, once established, the visual deficit often remains when normal visual input has been restored

Evidence for delayed neurotoxicity produced by methylmercury.

Delayed toxicity as a result of developmental methylmercury exposure was identified in mice two decades ago by Spyker, who observed kyphosis, neuromuscular deficits, and other severe abnormalities as the mice aged. Delayed neurotoxicity was also observed in monkeys treated with methylmercury from birth to seven years of age. When these monkeys reached 13 years of age, individuals began exhibiting clumsiness not present previously. Further exploration revealed that treated monkeys required more time to retrieve treats than did nonexposed monkeys and displayed abnormalities on a clinical assessm

Vibratory and tactile sense of the fingers after working with sanders.

Twenty-four volunteers worked four times in a laboratory under realistic conditions for 80 min with a relatively intensely vibrating plane-sander and a less intensely vibrating band-sander (each sander twice). Immediately before and after exposure, the left and right third finger was measured for vibratory sense with a 100 Hz rod-vibrameter and for width- and depth-sense by means of an apparatus modified after Carlson. The reproducibility of the parameters was poor: coefficients of intra-individual variation were 20 to 60%. After using the plane-sander the threshold of the width-sense in the r

Evaluation of nonnociceptive sensation in patients with congenital insensitivity to pain with anhidrosis.

PURPOSE: Congenital insensitivity to pain with anhidrosis (CIPA) is a rare, autosomal recessive disorder characterized by lack of pain and thermal sensation, anhidrosis, thermodysregulation, and mental retardation. Although nonnociceptive sensation, which is mediated by large-caliber myelinated Abeta fibers, is reported to be normal in CIPA patients, precise clinical assessments of this type of sensation have yet to be performed. The aim of this study was to evaluate peripheral sensory nerve function, including senses of touch/pressure, vibration, joint position, and two-point discrimination,

Measuring peripheral nerve involvement in Friedreich's ataxia.

OBJECTIVE: Experimental therapies under development for Friedreich's Ataxia (FRDA) require validated biomarkers. In-vivo reflectance confocal microscopy (RCM) of skin is a noninvasive way to quantify Meissner's corpuscle (MC) density and has emerged as a sensitive measure of sensory polyneuropathies. We conducted a prospective, cross-sectional study evaluating RCM of MCs and conventional peripheral nerve measures as candidate peripheral nerve markers in FRDA. METHODS: Sixteen individuals with FRDA and 16 age- and gender-matched controls underwent RCM of MC density and morphology, skin biopsie
