Patients with rotator cuff tear and degenerative shoulder joint disease commonly experience severe pain and reduced performance during activities of daily living. A popular way to treat these patients is by means of reverse anatomy shoulder prosthesis. Studying the kinematics of subjects with reverse anatomy implant would be useful in order to gain knowledge about functionality of different designs. It is hypothesized that the kinematics of these subjects, in the absence of rotator cuff muscles, differs from that of normal subjects. In this study the upper limb kinematics of 12 subjects with a…

AIM: Results of shoulder replacement in fracture sequelae are not satisfying. This is due to an insufficient function of the rotator cuff, mostly because of a secondary necrosis of the tubercles. The aim of this study was to investigate if the differentiated use of anatomic and reversed prostheses in dependence of the preoperative anatomic situation is able to optimise the results.
METHOD: The aim of this study was a comparison of 55 patients with secondary trauma prostheses in fracture sequelae. In 36 cases (especially fracture sequelae types 1 and 2 according to Boileau), anatomic head pros

CASE: A 64-year-old woman underwent open reduction and internal fixation for four-part proximal humeral fracture-dislocation with palpable distal pulses. Upon dislocated head removal, life-threatening hemorrhage occurred from posterior circumflex humeral artery tear. Bleeding was controlled through packing, resuscitation, and open vascular repair. Owing to unavailable shoulder prosthesis and prolonged operative time, a temporary cement prosthesis with Steinmann pins was used. At the 3-year follow-up, partial function was restored with no loosening.
CONCLUSION: This case highlights axillary br…

BACKGROUND: Cobalt, a ferromagnetic metal with a wide range of uses, has been known to cause systemic toxicity in humans that results in thyroid dysfunction, cardiomyopathy, hematologic abnormalities, and neuropathy. Arthroprosthetic cobaltism is a term used to describe systemic cobalt toxicity originating from cobalt-containing prosthetic joints. Ocular toxicity from elevated serum levels of cobalt is a rare phenomenon.
CASE REPORT: We describe a case of bilateral toxic optic neuropathy resulting from a damaged cobalt-containing shoulder prosthesis in a 76-year-old female. The patient exhibi…

BACKGROUND: The depth of the polyethylene insert in relation to the glenosphere radius, also referred to as the polyethylene constraint, is an implant-design factor directly related to reverse total shoulder arthroplasty (rTSA) stability. However, little is currently known about the objective levels of constraint of the various commercially available polyethene inserts. As such, the primary objective of this study was to report the "depth-to-radius" (D/r) ratio of polyethylene inserts from several commercially available rTSA implant systems. Additionally, as various reporting metrics exist to

Factors associated with upper limb prosthesis adoption are not well understood. In this study, we explored how prosthesis usability experience relates to the extent of prosthesis adoption through the development of a structural equation model (SEM). First, items related to prosthesis usability were developed and refined using cognitive testing and pilot testing and employed in a survey of 402 prosthesis users (mean age 61.7 (sd 14.4), 77.1% Veterans). The SEM examined two unidimensional latent constructs: Prosthesis Usability Experience and Prosthesis Adoption-and each had multiple measured in

Pigmented villonodular synovitis is a reactive condition characterised by exuberant proliferation of synovial villi and nodules. It may be localised or diffuse and can cause severe articular damage. This report is of 2 patients with pigmented villonodular synovitis of the shoulder causing extensive arthritic changes. Both patients underwent shoulder hemiarthroplasty and total synovectomy and achieved satisfactory painless range of motion, with no signs of local recurrence or loosening of the prosthesis after 4 to 5 years of follow-up.

INTRODUCTION: The application of the refinements of linking rules to the outcome measurement methods provides an opportunity to obtain information concerning what the available instruments actually measure and how the instrument measures certain outcomes. The purpose of the study was to analyze the content of 4 commonly used upper limb amputee scales using the refined International Classification of Functioning, Disability, and Health (ICF) linking rules.
METHODS: The Upper Extremity Functional Status module of the Orthotics and Prosthetics User Survey (OPUS-UEFS), Trinity Amputation and Pros

BACKGROUND: The use of a metal radial head prosthesis to help stabilize an elbow with traumatic instability is appealing because internal fixation of multifragment, displaced fractures of the radial head is susceptible to either early or late failure. The newer modular prostheses are easier to size and implant, but their effectiveness has not been investigated, to our knowledge.
METHODS: Twenty-seven patients in whom a radial head replacement with a modular metal spacer prosthesis had been performed to treat traumatic elbow instability were evaluated with use of the Mayo Elbow Performance Ind

BACKGROUND: Shoulder hemiarthroplasty (HA) is a viable treatment option for complex proximal humerus fractures in younger patients (<65 years), although clinical outcomes can vary. In recent years, the pyrolytic carbon head (PCH) prosthesis has emerged as an alternative to the cobalt-chromium (CoCr) head in shoulder osteoarthritis, potentially reducing glenoid erosion. This study aimed to compare the outcomes of the PCH prosthesis HA with the traditional CoCr head in the treatment of complex proximal humerus fractures (PHFs), with the hypothesis that the PCH group would demonstrate superior re

INTRODUCTION: Proximal humeral bone loss in total shoulder arthroplasty (TSA) is more frequent than in hemiarthroplasty. Factors such as age, gender, inclination angle, and radiolucent lines may also contribute. Additionally, current bone loss grading systems are often not sensitive enough to detect slight bone changes, especially at the medial calcar where bone loss is commonly observed. This study uses a new, more detailed bone loss grading system to evaluate factors that could influence bone loss at the proximal humerus.
MATERIALS AND METHODS: In this single-center prospective study, patie

AIM OF THE STUDY: To assess the clinical and computed tomography (CT) outcomes of shoulder replacement with a novel bone ingrowth all-polyethylene glenoid component (APGC).
METHODS: Twenty-eight patients (30 shoulders) with osteoarthritis, mean age 62.3 years (range, 45-75), were implanted with the novel component between 2011 and 2013. Patients were evaluated by active range of motion (ROM), Constant-Murley score (CMS), simple shoulder test (SST), X-rays, and multidetector CT at two months and at a mean follow-up of 31 months (range, 24-39). Early and late follow-up CT scans were available f

HYPOTHESIS: Reverse shoulder arthroplasty is growing in popularity for patients with deficient rotator cuffs; however, the phenomenon of scapular notching continues to be a concern. This study examined the effects of humeral component version in the Aequalis Reversed Shoulder Prosthesis (Tornier, Edina, MN) on impingement of the humeral prosthesis against the scapula to test the hypothesis that the mechanical contact of the humeral component with the scapular neck is influenced by the version of the humeral component.
MATERIALS AND METHODS: Seven shoulders from deceased donors were tested aft…

BACKGROUND: The concept of onlay design reverse shoulder arthroplasty has been introduced to overcome complications observed with the traditional Grammont-type prosthesis. The aim of this study was to determine the influence of arm lengthening on the short-term clinical outcome in onlay reverse shoulder arthroplasty and investigate the effect of humeral tray offset positioning on arm lengthening and range of motion.
METHODS: We retrospectively evaluated 56 patients undergoing reverse shoulder arthroplasty with the Aequalis Ascend Flex prosthesis (Tornier, Bloomington, MN, USA) at a minimum 2

Shoulder joint involvement in juvenile idiopathic arthritis (JIA) is common; however, it is first clinically manifested only after a delay. Initially, the aim should be to preserve the joint for as long as possible by means of adequate drug treatment and arthroscopic synovectomy. If definitive joint destruction nevertheless occurs hemiarthroplasty, anatomical and reverse shoulder arthroplasty are available. Hemiarthroplasty can result in pain amelioration and was the gold standard for a long time but it remains inferior to total shoulder arthroplasty in terms of pain reduction, improvement of

BACKGROUND: People with upper limb absence use compensatory movements to accommodate lack of motion in the prosthetic hand. The purpose of this study was to determine if the type of prosthesis used (i.e. body-powered or myoelectric) affects compensatory movements during activities of daily living.
METHODS: Twelve transradial body-powered and/or myoelectric prosthesis users performed up to six unimanual and bimanual activities of daily living. Trunk range of motion and peak upper limb angles for each task were compared between prostheses.
FINDINGS: Compensatory movement generally did not diff

BACKGROUND AND PURPOSE: Persistent postsurgical pain is a well-recognized problem after various types of surgery such as amputation and thoracotomy. The prevalence of persistent pain, and the extent to which it involves neuropathic pain, is highly dependent on the type of surgery. We investigated the prevalence of, characteristics of, and risk factors for persistent pain 1-2 years after shoulder replacement.
PATIENTS AND METHODS: A questionnaire was sent to patients who underwent primary shoulder replacement between April 2011 and April 2012, and whose data were recorded in the Danish Shoulde
