Juvenile hyaline fibromatosis (JHF) is an extremely rare hereditary genetic disease of autosomal recessive transmission that is characterized by large cutaneous tumors commonly involving the scalp, papulonodular skin lesions, flexural joint contractures, gingival hyperplasia, and osteolytic bone lesions. JHF is usually diagnosed in young infants and in children younger than 5 years, and the lesions characteristic of this disorder consist of fibrous tissue and homogenous amorphous eosinophilic hyaline material. We report the case of a 9-year-old girl with severe gingival hyperplasia, nasal enla

UNLABELLED: The high morbidity and mortality of tuberculous meningoencephalitis (TBM) warrants an early diagnosis and treatment. BCG vaccine has been proven to reduce the incidence of disseminated disease in children. We report on two siblings (2-year-old boy and 4-year-old girl) with simultaneous TBM, whose parents originated from Kosovo, Albania, but presently reside in Germany. Early diagnosis of TBM was delayed, and at first the misdiagnosis of viral meningoencephalitis was made. Antituberculosis treatment was not initiated despite profound hyponatremia, hydrocephalus, and signs of inflamm…

Familial hypomagnesemia-hypercalciuria with nephrocalcinosis and renal insufficiency in childhood is a rarely described disease. Two siblings of consanguineous Tunesian parents (first cousins), a 2-year-old boy and a 4-year-old girl presented with renal insufficiency and severe bilateral nephrocalcinosis. Both were found to have decreased serum and intracellular magnesium concentrations, increased urinary excretion of magnesium and calcium, mild glomerular and severe tubular proteinuria and low citrate excretion in urine. Pathological biochemical findings and the severity of nephrocalcinosis o…

Cystic leukoencephalopathy with megalencephaly is a newly described entity with mild clinical involvement. Patients suffer from developmental problems and seizures in childhood. Progression is gradual into adulthood. Typical magnetic resonance imaging findings include subcortical cysts and diffuse leukoencephalopathy. The etiology is unknown with possibly autosomal-recessive inheritance. We present two pairs of siblings with this disease and emphasize the characteristic and variable patterns even within the same family.

An association between anticonvulsant drugs taken during pregnancy and congenital abnormalities was first identified by Meadow et al. in 1968. Manson and Frederic clarified teratogenic effects of hydantoin in their epidemiological studies in 1973. Varied malformations due to hydantoin intake during pregnancy include digit and nail hypoplasia, growth retardation, typical facial appearance, rib anomalies, abnormal palmar creases, hirsutism, and low hairlines. Ambiguous genitalia is rarely associated with this syndrome. We present two siblings, aged three years and three months, with fetal hydant…

Trichothiodystrophy (TTD) is a rare autosomal recessively inherited disorder which is characterized by sparse and brittle hair with low cystine content. It is often associated with physical and mental retardation. We report 2 cases of TTD in 2 sibs who were born to related parents. The children showed clinical features typical of TTD and in addition other symptoms such as epilepsy, ataxia, spasticity, strabismus, atopic dermatitis, dysarthria and hyperextensible fingerjoints. The sulfur content of hair was reduced to about 50% of normal values and scanning electron microscopy of hair showed tr

The National Registry of Childhood Tumours contains over 51000 records of children born in Great Britain who developed cancer under the age of 15 years. Patterns of childhood cancer among families containing more than one child with cancer have been studied. A total of 225 "sib pair' families have been ascertained from interviews with parents of affected children, from hospital and general practitioner records and from manual and computer searches of names and addresses of patients. A number of special groups have been identified, including those with a known genetic aetiology such as retinobl

We here report on a new family affected with WWS. Two elder siblings had presented with epileptic seizures, eye abnormalities as well as multiple skeletal dysplasias (the latter finding in the first child only) in the neonatal period, and died in their first years. Postmortem examination of the second child revealed …

Two male siblings with chorioretinopathy and pituitary dysfunction (CPD) were found to have empty sellas on CT scan. Extensive ophthalmological and endocrinological investigation revealed the absence of gonadotrophins in both brothers and growth hormone deficiency in one. ACTH, TSH, and posterior pituitary function were normal. Karyotyping in one brother revealed a 46XY complement. Ophthalmological evaluation of three other siblings and both parents revealed normal vision and no evidence of retinopathy. The brothers represent two additional examples of CPD syndrome, suggesting a genetic etiolo…

beta-Thalassaemia is a chronic disease causing serious symptoms to the patients and considerable burden to their families. The psychopathology and psychosocial adjustment of 71 siblings of thalassaemic patients and 71 matched control subjects were investigated. Siblings of thalassaemic patients were found to have significantly more psychiatric disorders than the control subjects (p < 0.05). The frequency of psychopathology among the patients' siblings, when compared to the controls, was significantly greater after the age of 10 years (p < 0.01), with no difference between the two groups before…

Two siblings are reported who have features of the Marden-Walker syndrome. They have congenital joint contractures, camptodactyly, talipes equinovarus, abnormal facies, global delay in developmental milestones, hypotonia and failure to thrive. About 20 cases have been reported in the world literature. A summary of some of the cases is presented. This syndrome appears to be an autosomal recessive trait in some families.

The clinical manifestation of McArdle disease rarely occur in children. A brother and a sister aged 12 and 7 years respectively are presented. Both are sons of a consanguinous marriage. The clinical manifestations of the girl began when she was seven. The dominant clinical feature was her difficulty in performing physical exercises. At this time the brother was asymptomatic. There were no abnormalities on physical examination. The level of the serum CK was fluctuating at a level above normal in both patients. There was not lactic acidosis after exercise test. The level of lactic acid didn't ri

Two siblings with infantile lactic acidosis and mitochondrial myopathy are described. The first child, a girl, died at 5 months of age from severe lactic acidosis after about 3 weeks of progressive muscular hypotonia. The younger brother had congenital lactic acidosis but no other symptoms until 6 months of age when progressive muscle weakness appeared. Treatment with dichloroacetate lowered the serum lactic acid level but did not affect his clinical condition. At 13 months of age, cardiomyopathy was diagnosed and he died at the age of 29 months of circulatory failure. Both children had mitoch…

We describe two brothers born 16 months apart who were hospitalized with culture-proved chlamydial pneumonia of infancy due to the same immunotype of Chlamydia trachomatis. Presumably the mother had either been chronically infected or had been reinfected. Chlamydial pneumonia of infancy is a preventable disease if genital infection in pregnant women is diagnosed and treated. However, if pregnant women at high risk are not screened for C. trachomatis, multiple offspring may acquire the infection and develop serious perinatal chlamydial disease.

The relationships of 35 community-based mildly mentally retarded adults and their siblings were examined through participant-observation techniques over an 18-month period. Five kinds of sibling relationships were identified based on the independent ranking of each sibling pair on a continuum that reflected three major relationship dimensions: warmth, frequency of contact, and degree of involvement. These relationships were described as were some general patterns that seem to have the greatest impact on relations between retarded adults and their siblings.

Reported here are two siblings with sudanophilic leukodystrophy occurring in early infancy. Soon after birth, high fever, distension of the abdomen and spasticity of the extremities were noted. Neurologic disorders, frequent convulsive seizures, and malnutrition caused both siblings to die at the age of 3 and 6 months, respectively. Neuropathologic examination of the brain revealed diffuse demyelination in the cerebral and cerebellar white matter as well as a disturbance of the myelination, especially in the descending pathway. Numerous sudanophilic lipids and glial fibers proliferated in the …
