This study is a retrospective review of all consecutive surgeries involving insertion of silicone malar implants performed at the Manhattan Eye, Ear and Throat Hospital from January 1, 1985 to April 30, 1993. Sixty-four patients underwent placement of 126 silicone malar implants. Three different sizes were utilized: 23 size 1, 85 size 2, and 18 size 3. A method for silicone intubation of a single canaliculus is described. A lacrimal stent has been crafted from a short segment of silicone tubing and a punctal plug. The distal end of the stent is fixed in the punctum with a punctal plug. The silicone stent, which is attached to the punctal plug, bridges the canalicular laceration. Because the punctal plug remains fixed in the punctum, no sutures are needed to fixate the stent to the eyelid.

Silicone intubation is an alternative to dacryocystorhinostomy in selected patients who have epiphora and obstruction of the nasolacrimal system. Silicone tubing may be passed through the nasolacrimal system and left in place for a prolonged period of time. In many instances, this obviates the need for a dacryocystorhinostomy. If the intubation is not completely successful, a dacryocystorhinostomy can always be carried out at a later date.

Freiberg's disease or osteochondrosis of the second metatarsal head may necessitate surgical intervention because of pain. Several surgical procedures have been recommended. A new surgical procedure similar to the present treatment of hallux rigidus with a silicone rubber implant is described. Silicone oil tamponade is used for the management of complicated retinal detachment. Patients should be closely monitored as many complications may occur with intraocular silicone oil. Short-term complications include temporarily increased intraocular pressure and anterior segment inflammation. Long-term complications include cataract, emulsification, ocular hyper- or hypotension, keratopathy and retinal redetachment associated with proliferative vitreoretinopathy.

Silicone stents are commonly used in dacryocystorhinostomy to help maintain a patent mucosal passageway into the nose. It is not unusual for a tube to become displaced and migrate laterally (stent prolapse), and, at times, it may be difficult to reposition it. We describe how stent prolapse may be prevented by using aneurysm clips to secure the silicone stents once they are in place.

The authors describe the radiologic appearance of fluid silicone in the soft tissues of the gluteal area in a transsexual man. Thickening and striation of the soft tissues was observed in the trochanteric area in plain films. Serpiginous hyper-attenuated bands infiltrating the subcutaneous fat were seen with computed tomography. The patient was asymptomatic in the gluteal area.
