Optimal Electroencephalogram and Electrooculogram Signal Combination for Deep Learning-Based Sleep Staging.

OBJECTIVE: The traditional sleep staging involves manual scoring of electroencephalogram (EEG), electrooculogram (EOG), and electromyogram s … ation performance of automatic sleep staging against manual sleep staging was evaluated across five sleep stages. RESULTS: The differences in classification performance among various EEG signal combinations were negligible, with accuracies ranging from 81% to 85% (Cohen's kappa, κ = 0.73-0.78). Incorporating the EOG signal into single EEG configurations impro …

Enhancing automatic sleep stage classification with cerebellar EEG and machine learning techniques.

Sleep disorders have become a significant health concern in modern society. To investigate and diagnose sleep disorders, sleep analysis has … ibited significant differences in power spectral density across wakefulness, non-rapid eye movement sleep stages, and rapid eye movement sleep stages, particularly at frequencies >7 Hz. LGBoost, Extreme Gradient Boosting, and Categorical Boosting models showed comparable performance, with LGBoost being selected for further analyses due to its shorter computati …

Microsleeps are Associated with Stage-2 Sleep Spindles from Hippocampal-Temporal Network.

Behavioral microsleeps are associated with complete disruption of responsiveness for [Formula: see text][Formula: see text]s to 15[Formula: see text]s. They can result in injury or death, especially in transport and military sectors. In this study, EEGs were obtained from five nonsleep-deprived healthy male subjects performing a 1[Formula: see text]h 2D tracking task. Microsleeps were detected in all subjects. Microsleep-related activities in the EEG were detected, characterized, separated from eye closure-related activity, and, via source-space-independent component analysis and power analysi

New biomarkers for stage determination in Trypanosoma brucei rhodesiense sleeping sickness patients.

Accurate stage determination is crucial in the choice of treatment for patients suffering from sleeping sickness, also known as human African trypanosomiasis (HAT). Current staging methods, based on the counting of white blood cells (WBC) and the detection of parasites in the cerebrospinal fluid (CSF) have limited accuracy. We hypothesized that immune mediators reliable for staging T. b. gambiense HAT could also be used to stratify T. b. rhodesiense patients, the less common form of HAT.A population comprising 85 T. b. rhodesiense patients, 14 stage 1 (S1) and 71 stage 2 (S2) enrolled in Malaw

Pharyngeal narrowing in end-stage renal disease: implications for obstructive sleep apnoea.

Sleep apnoea is common in patients with end-stage renal disease (ESRD). It was hypothesised that this is related to a narrower upper airway. Upper airway dimensions in patients with and without ESRD and sleep apnoea were compared, in order to determine whether upper airway changes associated with ESRD could contribute to the development of sleep apnoea. An acoustic reflection technique was used to estimate pharyngeal cross-sectional area. Sleep apnoea was assessed by overnight polysomnography. A total of 44 patients with ESRD receiving conventional haemodialysis and 41 subjects with normal ren

Enhanced chemo-responsiveness in patients with sleep apnoea and end-stage renal disease.

Although sleep apnoea is very common in patients with end-stage renal disease, the physiological mechanisms for this association have not yet been determined. The current authors hypothesised that altered respiratory chemo-responsiveness may play an important role. In total, 58 patients receiving treatment with chronic dialysis were recruited for overnight polysomnography. A modified Read rebreathing technique, which is used to assess basal ventilation, ventilatory sensitivity and threshold, was completed before and after overnight polysomnography. Patients were divided into apnoeic (n = 38; a

Influence of zopiclone, a new generation hypnotic, on the intermediate stage and paradoxical sleep in the rat.

This study examined the influence of zopiclone, a third generation hypnotic, on the transition from slow wave sleep to paradoxical sleep (PS) which is increased at the expense of PS by barbiturates and benzodiazepines. The compound decreased sleep latency and increased the latency of the intermediate stage (IS) and PS at 2.5, 5 and 7.5 mg/kg IP. The amount of the IS was decreased because of the decrease in phase number up to 6 h at all doses. PS amount was decreased during 2 h at 2.5 mg/kg and during 4 h at 5 and 7.5 mg/kg also because of the decrease in phase number. The IS never substituted

Automatic Oxford-Medilog 9200 sleep staging scoring: comparison with visual analysis.

We report the results of a comparison study between automatic sleep staging using the ambulatory Oxford-Medilog 9200 system and visual analysis in a group of 30 subjects. Sleep analysis was performed during the second nighttime period of each 48-h polygraphic monitoring. The automatic analysis determined a significant underestimation of total sleep time and stage 2 non-rapid eye movement (NREM) sleep and an overestimation of stage 1. No significant differences were found between the two analyses regarding slow-wave sleep and REM sleep. The differences between automatic and visual analysis of s

Improved non-invasive detection of sleep stages when combining skin sympathetic nerve activity and heart rate variability analysis with AI.

Sleep is a cyclic physiological process that goes into different stages, and every stage has its' importance in the construction or recovery of physiological function. Sleep scoring is performed from polysomnography recordings which requires signals from multiple sensors in a specially equipped Sleep Lab. This is expensive, and normal sleep behavior may be affected due to the new environment and extensive testing equipment. Skin sympathetic nerve activity (SKNA) has been shown to vary between sleep stages, and it can be recorded simultaneously with the ECG on the skin using conventional ECG pa…

Cortical thinning with increasing OSA severity stages in adult comorbid insomnia and sleep apnea and its relevance for respiration measures.

The study aimed to analyze the alterations of cortical thickness (CT) in comorbidity of insomnia and sleep apnea (COMISA) patients of varying severity and the relationships of these CT changes with clinical measures. The study included 34 COMISA patients and 24 healthy controls (HCs). The patients received structural MRI as well as measures associated with respiration, sleep, cognition, and mood, and were classified as mild and moderate (MC) or severe (SC) groups. The CT for COMISA was analyzed through surface-based morphometry (SBM). Voxel-wise differences in CT between two groups were analyz

A Deep Generative Model for Five-Class Sleep Staging With Arbitrary Sensor Input.

Gold-standard sleep scoring is based on epoch-based assignment of sleep stages based on a combination of EEG, EOG and EMG signals. However, a polysomnographic recording consists of many other signals that could be used for sleep staging, including cardio-respiratory modalities. Leveraging this signal variety would offer important advantages, for example increasing reliability, resilience to signal loss, and application to long-term non-obtrusive recordings. We developed a deep generative model for automatic sleep staging from a plurality of sensors and any -arbitrary- combination thereof. We t…

PPG-Based Sleep Staging Using SleepPPGNet: Extension to Wearables, Improvements, Limitations.

The diagnosis of sleep disorders is still often based on polysomnography, an in-lab exam allowing experts to perform accurate sleep staging, although this is labor-intensive, expensive, and exposing patients to unusual sleep conditions. A state-of-the-art deep learning model - called SleepPPGNet - was recently proposed. It achieves an accuracy of 82% and a Cohen's kappa of 0.74 on a completely new dataset through transfer learning, using only raw fingertip photoplethysmography (PPG) as input, paving the way toward more efficient sleep staging methods. We applied this model to PPG data collected

Sleep apnea in end-stage renal disease patients: risk factors and mortality.

Sleep apnea (SA) is highly prevalent in the end-stage renal disease (ESRD) population. However, the impact of SA on mortality in ESRD is unclear. This study investigates the relationship between SA and mortality in ESRD. The United States Renal Data System was queried in a retrospective cohort study to identify ESRD patients aged 18-100 years who initiated hemodialysis between 2005 and 2013. Diagnoses of SA and comorbidities were determined from International Classification of Disease-9 codes and demographic variables from Centers for Medicare and Medicaid Services Form-2728. Cox proportional

Effect of Ultrafiltration on Sleep Apnea and Cardiac Function in End-Stage Renal Disease.

RATIONALE: End-stage renal disease (ESRD) patients have high annual mortality mainly due to cardiovascular causes. The acute effects of obstructive and central sleep apnea on cardiac function in ESRD patients have not been determined. We therefore tested, in patients with ESRD, the hypotheses that (1) sleep apnea induces deterioration in cardiac function overnight and (2) attenuation of sleep apnea severity by ultrafiltration (UF) attenuates this deterioration. METHODS: At baseline, ESRD patients, on conventional hemodialysis, with left ventricular ejection fraction (LVEF) >45% had polysomnog

Impact of kidney transplantation on sleep apnoea in patients with end-stage renal disease.

BACKGROUND: Sleep apnoea is common in patients with end-stage renal disease. Although individual case reports have described an improvement in sleep apnoea following kidney transplantation, there have been no longitudinal studies of a case series to determine what proportion of patients with sleep apnoea improve. METHODS: Dialysis-dependent patients awaiting kidney transplantation and pre-dialysis patients with an identified living donor kidney had overnight polysomnography, which was repeated several months after successful kidney transplantation. Patients were divided into apnoeic and non-a
