Role of thromboelastography in the management of snake bite: A case report from India.

In the absence of a specific laboratory test of envenomation, there is a need for an alternative mechanism for the early recognition of envenomation following hematotoxic snake-bite in tropical countries. Abnormalities of clotting are commonly associated with hematotoxic snake bite either due to systemic envenomation or due to the release of an inappropriate tourniquet applied as 'first-aid' often by the rural people before presentation to the hospital. Thromboelastography (TEG) has been used to monitor the coagulation abnormalities in various clinical scenarios. Here we narrate our experience

A definite bite by the Ornamental Snake (Denisonia maculata) causing mild envenoming.

CONTEXT: Many bites from mildly venomous elapids occur but identification or presence of systemic envenoming is rarely confirmed. OBJECTIVE: To confirm systemic envenoming and binding of venom components to a commercial antivenom in a definite bite by the Ornamental Snake (Denisonia maculata) using enzyme immunoassays. CASE: A 9-year old boy was bitten by an identified Ornamental Snake. He developed nausea, vomiting, local pain, and swelling. He had a leucocytosis (white cell count, 20.8 × 10(9)/L), an elevated international normalised ratio (INR) of 1.6, but otherwise normal blood tests inc

Clinical predictors of acute kidney injury following snake bite envenomation.

BACKGROUND: Snake bite envenomation is a major public health concern in developing countries. Acute kidney injury (AKI) is as important cause of mortality in patients with vasculotoxic snake bite. AIMS: This study was to evaluate the clinical profile of snake bite patients and to determine the predictors of developing AKI following snake bite. MATERIALS AND METHODS: Two hundred and eighty-one patients with snake envenomation were included. Eighty-seven patients developed AKI (Group A) and 194 (Group B) did not. History, examination findings and investigations results were recorded and compar

Reversible posterior leukoencephalopathy in a venomous snake (Bothrops asper) bite victim.

An 18-year-old man developed posterior reversible leukoencephalopaty after being bitten by a venomous snake (Bothrops asper). It is possible that this previously unrecognized neurological complication of snake bite envenoming occurred as the result of endothelial dysfunction induced by the venom of the offending snake. This pathogenetic mechanism has also been implicated as the cause of cerebral infarctions in snake bite victims. Alternatively, the leukoencephalopathy might have been a complication of antivenom therapy.

Hugh Alistair Reid OBE MD: investigation and treatment of snake bite.

Alistair Reid was an outstanding clinician, epidemiologist and scientist. At the Penang General Hospital, Malaya, his careful observation of sea snake poisoning revealed that sea snake venoms were myotoxic in man leading to generalized rhabdomyolysis, and were not neurotoxic as observed in animals. In 1961, Reid founded and became the first Honorary Director of the Penang Institute of Snake and Venom Research. Effective treatment of sea snake poisoning required specific antivenom which was produced at the Commonwealth Serum Laboratories in Melbourne from Enhydrina schistosa venom supplied by t

Acute renal failure following snake bite: case report and review.

We present an adult male patient who presented with history of having been bitten by a snake. Despite administration of antivenom and adequate hydration, he developed acute renal failure secondary to disseminated intravas-cular coagulation and rhabdomyolysis. He improved with daily hemodialysis and at discharge had normal renal function. Although we did not perform a renal biopsy on him, the clinical course was highly suggestive of acute tubular necrosis. The pathogenesis, pathology, as well as therapy of renal involvement following venomous snake bite is discussed.

Death from a snake bite: associated with the supine hypotensive syndrome of pregnancy.

A young pregnant woman, near term, died shortly after being bitten by an immature brown snake (genus Pseudonaja). Analysis of the bitten area suggested that the degree of envenomation was mild, and could not satisfactorily explain her rapid demise. It was concluded that the supine hypotensive syndrome of pregnancy was a contributing factor in her death.

Renal failure following snake bite. A clinicopathological study of nineteen patients.

Clinical, hematological, and renal biopsy findings in 19 unselected patients with renal failure following snake bite, including eight bitten by Russell's viper, are presented. Hematological findings were variable and did not influence treatment. Renal histology was a good guide to prognosis; patients with normal histology recovered with conservative management, those with tubular necrosis responded well to dialysis, while those with cortical necrosis responded only partially. Peritoneal dialysis was found to be an adequate form of treatment in the majority of patients.

Compartment syndrome of upper limb following snake bite, a case report.

INTRODUCTION: Snakebites, recognized as a neglected tropical disease by the WHO, cause significant morbidity and mortality globally. Although antivenom is the primary treatment, managing complications like compartment syndrome (CS) and soft tissue necrosis remains challenging. This case report describes a 39-year-old woman who developed CS following a green pit viper bite and subsequent antivenom administration, necessitating a fasciotomy. CASE: The patient sustained a bite to her right middle finger, receiving 10 vials of polyvalent anti-snake venom. The next day, she developed symptoms sugg

Snake bite-induced renal medullary angitiis in a child: A case report.

Snake bite envenomation is common in tropical countries during the summer. Snake bite-induced acute kidney injury (AKI) has varied histopathological manifestations such as acute cortical necrosis, acute tubular necrosis (ATN), and acute interstitial nephritis. However, snake bite-induced renal medullary angiitis has rarely been reported. We describe a nine-year-old child with AKI following viperine snake bite and renal biopsy revealed pigment cast nephropathy, ATN and medullary angiitis.

Calcific myonecrosis following snake bite: a case report and review of the literature.

INTRODUCTION: Calcific myonecrosis is a rare condition in which muscle in a limb compartment undergoes necrosis and becomes peripherally calcified with central liquefaction. The patient usually presents with a slowly progressive enlarged mass that sometimes can be misdiagnosed as soft tissue sarcoma. Most of the reported cases showed that the disease occurs often after trauma or compartment syndrome. However, the case of calcific myonecrosis following snake bite is rarely reported. CASE PRESENTATION: A 66-year-old Thai woman presented with a gradually progressive enlarged mass over a period o

A bite by the Twin-Barred Tree Snake, Chrysopelea pelias (Linnaeus, 1758).

INTRODUCTION: The Twin-Barred Tree Snake, Chrysopelea pelias, is a colubrine that, like other members of the genus Chrysopelea, is able to glide in the arboreal strata. Little is known about the effects of its bite. This report is the first clinically documented bite by this relatively uncommon rear-fanged species. CASE REPORT: The patient was a 19-year-old female who arrived at the Emergency Department (ED) of an urban teaching hospital 6 h after being bitten by a snake that was later provisionally identified as a C. pelias. Noted on presentation were bite marks on the right middle toe with

[Evaluation of snake bite incidence in the Sahelian zone of Senegal, example of Niakhar].

An exhaustive household survey was carried out in the study zone of Niakhar, a rural sahelian area in Senegal 150 km east of Dakar. Results were compared with those of the data base updated quarterly since 1983 and to the notifications of snakebites in the 7 health centres of the zone (130 inhabitants per km2). The questionnaire related to the snakebites (identification of victims, circumstances of bite, symptoms and treatment). The annual incidence was low (23 bites per 100,000 inhabitants) as the morbidity (by 20 envenomations per 100,000). The case fatality rate was relatively high (7%) and

[Epidemiological and clinical aspects of snake bites in the municipalities of the state of Amazonas, Brazil].

In the State of Amazonas, accidents with snakes are a public health problem. For this reasons, the objective of this work was to carry out a descriptive study of the snake accidents attended in the health units of 34 municipalities, one district and two border platoons in the State of Amazonas. The characteristics most commonly observed among those involved in snake accidents were: farmers (50. 4%), male (81.3%), belonging to the working age-group (72.1%), bitten on an upper limb (88.5%) by a "jararaca" (48.6%) or a "surucucu" (46.8%) in the rural part of the municipality (70.2%). The local si
