Diagnostic Test Analysis of Intraoperative Direct Sonication in Detecting Periprosthetic Joint Infection: A Systematic Review and Meta-Analysis.

Periprosthetic joint infection (PJI) remains a challenging complication after arthroplasty. Intraoperative direct sonication (sonicating explanted components or periprosthetic tissue in the operating room and inoculating processed fluid into blood culture bottles) may improve pathogen detection by disrupting biofilm. We conducted a systematic review and meta-analysis to evaluate the diagnostic accuracy of this test for suspected PJI. We searched CENTRAL, MEDLINE (PubMed), and Embase (ProQuest) on June 10, 2025, without language restrictions, and screened trial registries (ICTRP, ClinicalTrials…

Do Sonication Results of Mobile Parts Predict Failure After Debridement and Implant Retention in Acute Periprosthetic Joint Infection?

In 57 periprosthetic joint infections with presumed acute presentations, the 2-year successful outcomes of debridement and implant retention were 64% (when sonication results revealed <1000 cfu/mL) and 21% (≥1000 cfu/mL). The odds for failure were 5.2 (P < .01) when the sonication result reported ≥1000 cfu/mL.

Effect of sonication on staphylococcal and gram-negative biofilms relevant to prosthetic joint infections: an in vitro study.

BACKGROUND: Periprosthetic joint infections (PJIs) are severe complications of arthroplasty in which biofilm formation on implant surfaces compromises microbiological diagnosis and antimicrobial efficacy. Although staphylococci remain the predominant pathogens, Gram-negative bacilli have increasingly been associated with diagnostic failure and unfavorable clinical outcomes. This study aimed to evaluate the formation, maturation, and structural organization of Gram-positive and Gram-negative bacterial biofilms and to investigate the effects of a standardized sonication protocol on biofilm disru…

Comparative Phenotypic and Genomic Features of Staphylococci from Sonication Fluid of Orthopedic Implant-Associated Infections with Poor Outcome.

Staphylococcus spp. remain the leading biofilm-forming agents causing orthopedic implant-associated infections (OIAI). This is a descriptive study of phenotypic and genomic features identified in clinical isolates of S. aureus and coagulase-negative Staphylococcus (CoNS) recovered from OIAIs patients that progressed to treatment failure. Ten isolates were identified by matrix-time-of-flight laser-assisted desorption mass spectrometry (MALDI-TOF-MS) and tested for antibiotic susceptibility and biofilm formation. Genotypic characteristics, including, MLST (Multi Locus Sequence Typing), SCCmec ty

Comparison of Genus Specific PCR and Culture with or without Sonication for Microbiological Diagnosis of Vascular Graft Infection.

OBJECTIVES: Vascular graft infections (VGIs) are severe and require prolonged adequate antimicrobial therapy. However, up to 45% of conventional cultures are negative. Sonication and genus specific PCRs for microbiological diagnosis of VGI was evaluated. METHODS: Samples were prospectively obtained from explanted vascular grafts in Bordeaux University Hospital. Conventional bacterial cultures with and without prior sonication of samples were performed. A genus specific PCR assay panel, targeting the most frequent bacteria involved in VGI (Staphylococcus, Streptococcus, Enterococcus, and Enter…

Sonic Hedgehog Pathway Modulation in Medulloblastoma: Focus on Vismodegib (GDC-0449).

Medulloblastoma (MB) is the most common malignant brain tumor in children, classified into four molecular subgroups: WNT, sonic hedgehog (SHH), Group 3, and Group 4. The SHH pathway is crucial in the pathogenesis of SHH-subgroup MB (SHH-MB), influenced by mutations in patched homolog 1 (PTCH1), smoothened (SMO), and suppressor-of-fused (SUFU). Targeting this route has shown potential, with vismodegib, an SMO inhibitor, demonstrating effectiveness in both preclinical and clinical investigations. Notwithstanding its therapeutic promise, vismodegib's systemic toxicity, especially bone abnormaliti

Trisomy 21 alters ciliary localization of Sonic Hedgehog signaling proteins.

Trisomy 21 (T21), the cause of Down syndrome, impairs cilia-dependent Sonic Hedgehog (SHH) signaling. We investigated how T21 affects SHH signaling at the primary cilium and found that Smoothened (SMO) protein, the main effector of SHH signaling, is diminished in T21 primary cilia. Specifically, T21 cells exhibit both decreased SMO localization to cilia and reduced SMO activation. Our findings suggest that reduced ciliary SMO in T21 results from defective entry into the cilium rather than impaired transport to centrosomes. Moreover, unlike the ciliogenesis defects observed in T21, the SMO defe

EXOSC10 haploinsufficiency causes primary microcephaly by derepression of Sonic hedgehog signalling.

The evolutionarily conserved RNA exosome complex modulates gene expression during development. Mutations in RNA exosome complex subunits have been implicated in various human brain disorders, suggesting that defects in RNA decay are linked to impaired neural development. In our study, we identified de novo variants of EXOSC10 in microcephalic individuals. The patient's phenotype can be replicated by heterozygous conditional knockout of Exosc10 in the developing mouse forebrain. The heterozygous loss of Exosc10 in the developing mouse cortex leads to increased cell cycle exit, with a premature

RXR Gamma Enables Oligodendrocyte Differentiation by Suppressing Sonic Hedgehog Signaling.

Overcoming remyelination failure is one of the main targets in therapeutic strategies for multiple sclerosis. This process requires the differentiation of oligodendrocyte precursor cells (OPCs) to mature myelinating oligodendrocytes (OLs), a process known to be controlled by thyroid hormone, nuclear receptors, and sonic hedgehog (SHH). Retinoid X receptor gamma (RXRg) is one of the nuclear receptors acting as a positive regulator of remyelination, but little is known about its mechanisms of function. Using transcriptomic and pharmacological analysis of primary neural stem cell-derived OPCs, we

Impact of Sonic Hedgehog-dependent sphenoid bone defect on craniofacial growth.

OBJECTIVES: The main objective of this study was to evaluate how an apparently minor anomaly of the sphenoid bone, observed in a haploinsufficient mouse model for Sonic Hedgehog (Shh), affects the growth of the adult craniofacial region. This study aims to provide valuable information to orthodontists when making decisions regarding individuals carrying SHH mutation. MATERIALS AND METHODS: The skulls of embryonic, juvenile and adult mice of two genotypes (Shh heterozygous and wild type) were examined and measured using landmark-based linear dimensions. Additionally, we analysed the clinical c

Downregulation of Sonic hedgehog signaling induces G2-arrest in genital warts.

BACKGROUND: Human papillomavirus (HPV) infected keratinocyte dysfunction results in the formation of genital warts, and the specific role of Sonic hedgehog (SHh) signaling in genital warts remains elusive. Thus, this study aimed to identify the correlation between wart formation and SHh signaling. MATERIALS AND METHODS: In this study, nine male patients with genital warts were recruited, and the expression of SHh and its downstream signal molecules Patched-1 and GLI family zinc finger 1 (Ptch1 and Gli1) was detected. Moreover, G2-phase cells in the collected genital warts samples were assesse

Sonic hedgehog is Essential for Proximal-Distal Outgrowth of the Limb Bud in Salamanders.

The developing forelimb has been a foundational model to understand how specified progenitor cells integrate genetic information to produce the tetrapod limb bauplan. Although the reigning hypothesis is that all tetrapods develop limbs in a similar manner, recent work suggests that urodeles have evolved a derived mode of limb dvelopment. Here, we demonstrate through pharmacological and genetic inactivation of Sonic hedgehog (Shh) signaling in axolotls that Shh directs expansion and survival of limb progenitor cells in addition to patterning the limb across the proximodistal and antero-posterio

Loudness and short-term annoyance of sonic boom signatures at low levels.

Supersonic aircraft produce a sonic boom when flying faster than the speed of sound. To rule out detrimental effects for inhabitants of overflown areas, civil supersonic flights (like the Concorde) were only allowed to fly at supersonic speed when over water. Due to the progress in aircraft design, the supersonic boom may be reduced considerably in the future. In this study, listening tests were carried out with a variety of low boom simulations and conventional sonic boom signatures in a similar level range. Participants rated the loudness and the short-term annoyance of 24 sonic boom signals

The Role of Sonic Hedgehog in Human Holoprosencephaly and Short-Rib Polydactyly Syndromes.

The Hedgehog (HH) signalling pathway is one of the major pathways controlling cell differentiation and proliferation during human development. This pathway is complex, with HH function influenced by inhibitors, promotors, interactions with other signalling pathways, and non-genetic and cellular factors. Many aspects of this pathway are not yet clarified. The main features of Sonic Hedgehog (SHH) signalling are discussed in relation to its function in human development. The possible role of SHH will be considered using examples of holoprosencephaly and short-rib polydactyly (SRP) syndromes. In

Evaluation of Pseudorandom Sonications for Reducing Cavitation With a Clinical Neurosurgery HIFU Device.

Transcranial high-intensity focused ultrasound is used in clinics for treating essential tremor (ET) and proposed for many other brain disorders. This promising treatment modality requires high energy resulting eventually in undesired cavitation and potential side effects. The goals of the present work were: 1) to evaluate the potential increase of the cavitation threshold using pseudorandom gated sonications and 2) to assess the heating capabilities with such sonications. The experiments were performed with the transcranial magnetic resonance (MR)-compatible ExAblate Neuro system (InSightec, …

Sonic Hedgehog Pathway Inhibition in the Treatment of Advanced Basal Cell Carcinoma.

Advanced basal cell carcinoma (BCC) represents a small proportion of BCCs that are not amenable to standard therapies due to lack of efficacy, high recurrence risk, and excessive morbidity. Implication of the Sonic hedgehog (Shh) pathway in the development of BCC has led to the development of systemic Shh pathway inhibitors, providing patients with advanced BCCs new treatment options and improved survival. There are currently two Food and Drug Administration (FDA)-approved Shh inhibitors, vismodegib and sonidegib, for advanced basal cell carcinomas. Vismodegib has approval for locally advanced

Sonic Hedgehog Signaling Is Required for Cyp26 Expression during Embryonic Development.

Deciphering how signaling pathways interact during development is necessary for understanding the etiopathogenesis of congenital malformations and disease. In several embryonic structures, components of the Hedgehog and retinoic acid pathways, two potent players in development and disease are expressed and operate in the same or adjacent tissues and cells. Yet whether and, if so, how these pathways interact during organogenesis is, to a large extent, unclear. Using genetic and experimental approaches in the mouse, we show that during development of ontogenetically different organs, including t

Atrioventricular canal defect and genetic syndromes: The unifying role of sonic hedgehog.

The atrioventricular canal defect (AVCD) is a congenital heart defect (CHD) frequently associated with extracardiac anomalies (75%). Previous observations from a personal series of patients with AVCD and "polydactyly syndromes" showed that the distinct morphology and combination of AVCD features in some of these syndromes is reminiscent of the cardiac phenotype found in heterotaxy, a malformation complex previously associated with functional cilia abnormalities and aberrant Hedgehog (Hh) signaling. Hh signaling coordinates multiple aspects of left-right lateralization and cardiovascular growth
