Streptococcal infection and the chronic carrier state.

Group A beta-hemolytic streptococcal pharyngitis is an extremely common problem for the family physician. Clinical signs are not reliable in making the diagnosis. Infections with few or no symptoms are common. Throat culture is the best aid to diagnosis. One-quarter or more of family contacts harbor the organism. Good mamagement includes accurate bacteriologic diagnosis, adequate use of the appropriate penicillin preparation, repeat cultures, treatment of family members, if indicated, and recognition of the chronic carrier state.

Recurrence of group B streptococcal infection.

It is clear that the group B streptococcus has become a major pathogen of young infants within the comparatively recent past. Further it is clear that, as with other endemic and epidemic pathogens, increasing clinical and laboratory experience brings to light variations not initially evident. In addition, therapy presumably effective in initial cases may not continue to be so. The following papers by coincidence were received over a relatively brief period of time. Hence, for emphasis of some of the problems related to this organism, they are presented as a group.

A family outbreak of serious streptococcal infection.

A localized outbreak of scarlet fever was caused by an M-nontypable T-1 (NT/1) group A beta-hemolytic Streptococcus. The result was a fatal outcome in a 5-year-old child and a near fatality in his 10-year-old sibling. The outbreak was confined to family units that had members with whom the children were close playmates. Extensive epidemiologic studies in neighborhood schools and local hospital emergency rooms did not demonstrate a disemination of the causative serotype in the community.

[Group A-streptococcal infection during pregnancy].

Group A-streptococcal (GAS) infection can lead to various clinical presentations and is fulminant when it reaches the deep tissues, leading to a high morbidity and mortality. The severity of postpartum GAS infections is widely known. In this case report we describe the course of disease in a pregnant patient with GAS toxic shock syndrome with initial complaints of abdominal pain, diarrhea and fetal demise at first presentation. Within 10 hours this patient died. It is important to stay vigilant for a fulminant GAS infection in pregnant patients, to recognize it quickly and treat it adequately.

Secondary streptococcal infection following influenza.

Secondary bacterial infection following influenza type A virus (IAV) infection is a major cause of morbidity and mortality during influenza epidemics. Streptococcus pneumoniae has been identified as a predominant pathogen in secondary pneumonia cases that develop following influenza. Although IAV has been shown to enhance susceptibility to the secondary bacterial infection, the underlying mechanism of the viral-bacterial synergy leading to disease progression is complex and remains elusive. In this review, cooperative interactions of viruses and streptococci during co- or secondary infection w

Invasive group A streptococcal infection in pregnancy.

We conducted a literature review of 55 pregnancies with symptomatic Group A streptococcus (Streptococcus pyogenes) infection reported in English (20 cases), French (2 cases) and Japanese (33 cases) to seek ways of improving prognosis. Multiparous women (83% [39/47]) in the third trimester (90% [47/52]) were prone to infection from winter to spring (75% [21/28]). Onset was heralded by flu-like symptoms, such as high fever (94% [46/49]), with upper respiratory (40% [22/55]) and/or gastrointestinal symptoms (49% [27/55]). Characteristic findings were early onset of shock (91% [50/55]) and infecti

[Invasive streptococcal infection as a complication of chickenpox].

Two girls aged 11 months and 6 years, presented with an invasive group A streptococcal (GAS) infection during the course of primary varicella. The infant had severe cellulitis of the left arm and leg, fever and bacteraemia. She developed osteomyelitis of ulna and tibia. The 6-year-old girl had a fever > 38.5 degrees C, hypotension, an acute respiratory distress syndrome, liver function abnormalities, and positive cultures of blood and joints. Her clinical picture was compatible with a GAS-associated toxic shock syndrome. If during the course of primary varicella persistent fever, secondary fev

An outbreak of foodborne streptococcal throat infection.

Streptococcal pharyngitis is a common disease of epidemic nature, usually transmitted by saliva droplets. We present an epidemiologic analysis of an outbreak of streptococcal pharyngitis in a military unit involving 75 soldiers. The causing organism was Streptococcus pyogenes (group A beta hemolytic Streptococcus, T28 M56), which was isolated from 53 affected individuals. The source of the epidemic was probably an army cook. The infection was transferred by a cabbage salad to individuals who became symptomatic within 24-48 h. This epidemic is a reminder of an exceptional route of streptococcal

Prevention of group B streptococcal infection.

Group B streptococci continue to be major perinatal pathogens, both for mothers and their infants, and are associated with significant morbidity, mortality, and its attendant cost to society. Approaches to prevention are directed toward either eliminating exposure to the organism or enhancing host resistance, that is, chemoprophylaxis and immunoprophylaxis. Intrapartum chemoprophylaxis has been shown to effectively interrupt vertical transmission of group B streptococci from the genitally colonized mother to the infant and to decrease the incidence of both maternal and early-onset neonatal gro

[Pathogenesis of glomerulonephritis following streptococcal infection].

Group A streptococci are responsible for the induction of serious pyoderma and pharyngitis in humans. The nonsuppurative sequelae "acute rheumatic fever" (ARF) and "acute poststreptococcal glomerulonephritis" (APGN) do very often occur with variable attack rates in epidemics with these microorganisms. At present, immune complex formation in the blood circulation system, in situ complex formation within the glomeruli, antibody cross-reactions between beta-hemolytic group A streptococci and human kidney tissue as well as tissue damage independent of specific antibodies are intensively discussed

[Streptococcal infection: variants and morphological manifestations].

82 cases of streptococcal infection (SI) in children who died in 1977-1985 are studied. Higher incidence of SI in children particularly in those who died at home is emphasized. As a rule, the proper diagnosis was not established clinically. By manifestations the observations were divided into two groups: 1) SI with a pronounced generalization including a pharyngeal one (31 cases), 4 of them with a rash (scarlet fever); extrapharyngeal (7 cases), 6 of them with a rash (scarlet fever); 2) SI without pronounced generalization (localized) including 33 cases with the involvement of the lungs and to

Cutaneous group B streptococcal infection.

A 20-year-old man had a penile ulceration from which a pure, heavy growth of group B streptococcus was cultured. Cutaneous infection with this organism is unusual, and it is most often reported in postpartum women, diabetic patients, and immunocompromised individuals. Vaginal colonization with group B streptococcus is common. Since traumatic erosions or ulcerations of the penis are not uncommon, it is likely that a certain number of unrecognized infections of this type occur.

Acute urticaria associated with streptococcal infection.

Eighty-one cases of acute urticaria were treated in our institution over a two-year period. In 13 of 32 cases screened for streptococcal pharyngitis, either positive throat culture, significant streptococcal exoenzyme antibodies (Streptozyme test), or both were found. Other causative factors for the acute urticaria had been excluded. It is our feeling that beta-hemolytic streptococcal infection may be a causative agent in many cases of acute urticaria, and that the infection may not always be clinically apparent. We therefore recommend a throat culture and determination of streptococcal enzyme

Neonatal group B streptococcal infection.

Group B streptococcus is now recognized as a major cause of serious neonatal infection. Between January 1973 and July 1979 35 infants with 'early onset' group B streptococcal infection were admitted to the Special Nursery, Aberdeen Maternity Hospital. Ten infants died, nine of whom were of low birth weight. Current research is directed towards defining 'high risk' groups and establishing an effective means of prevention. The concept of early administration of antibiotics to 'high risk' infants, particularly those of low birth weight, is recommended. While there is already evidence in support o
