Single-molecule spectroscopy of the temperature-induced collapse of unfolded proteins.

We used single-molecule FRET in combination with other biophysical methods and molecular simulations to investigate the effect of temperature on the dimensions of unfolded proteins. With single-molecule FRET, this question can be addressed even under near-native conditions, where most molecules are folded, allowing us to probe a wide range of denaturant concentrations and temperatures. We find a compaction of the unfolded state of a small cold shock protein with increasing temperature in both the presence and the absence of denaturant, with good agreement between the results from single-molecu

Avascular necrosis of the talus: a pictorial essay.

The talus is predisposed to avascular necrosis (AVN), or bone death due to ischemia, owing to its unique structure, characteristic extraosseous arterial sources, and variable intraosseous blood supply. Both traumatic and atraumatic causes have been implicated in talar AVN. The risk of posttraumatic AVN can be predicted using the Hawkins classification system. In addition, the "Hawkins sign" can be used as a radiographic marker that excludes the development of AVN. At radiography, talar AVN typically manifests as an increase in talar dome opacity (sclerosis), followed by deformity and, in sever

Left coronary artery anomaly: an often unsuspected cause of sudden death in the military athlete.

More than 300,000 cases of sudden cardiac death (SCD) occur in the United States each year. Left coronary artery anomaly (LCAA), although rare, is second only to hypertrophic cardiomyopathy as the most common cause of SCD associated with structural cardiovascular abnormalities. This case illustrates SCD secondary to LCAA in a military athlete. A 19-year-old soldier collapsed after an 8-km run. On arrival at the emergency room, he was unresponsive and in asystole. Despite successful resuscitation and aggressive management, the patient died the next morning. Autopsy revealed an anomalous left co

Airway Anatomy and Surgical Management in Obstructive Sleep Apnea.

Obstructive sleep apnea (OSA) is a sleep-related breathing disorder characterized by recurrent collapse of a structurally vulnerable upper airway during sleep. Airway caliber and stability are influenced by multiple anatomical structures, including the nasal cavity, soft palate, lateral pharyngeal walls, tongue base, hyoid complex, and craniofacial skeleton. Detailed knowledge of these structures is therefore essential for rational surgical planning. This narrative review summarizes key anatomical features relevant to upper pharyngeal surgery, including nasal obstruction and the internal nasal

Alveolar overdistension is an important mechanism of persistent lung damage following severe protracted ARDS.

It is now widely accepted that mechanical ventilation may damage the lung, but the mechanism of lung damage is not clear. Possible causes include overdistension of aerated alveoli by inappropriately large tidal volumes (volutrauma), shear stresses generated during the recruitment and de-recruitment of lung units at the junction of aerated and collapsed lung, and infective or ischaemic necrosis of persistently collapsed lung. Computerized tomography allows noninvasive assessment of lung structure during and after acute lung injury, and may provide insight into the mechanism of lung damage. Usin

Comparative Study of Functional Nasal Reconstruction Using Structural Reinforcement.

IMPORTANCE: Nasal reconstruction after Mohs surgery is a unique challenge in that it must satisfy both functional and aesthetic goals. Despite some advocacy in the literature for using structural reinforcement to achieve both functional and aesthetic outcomes in soft-tissue reconstruction, no study has validated this claim by comparing reconstruction with and without structural support. OBJECTIVE: To evaluate the effectiveness of and need for structural reinforcement when reconstructing the nasal alar and sidewall subunits. DESIGN, SETTING, AND PARTICIPANTS: This study was a retrospective re

Obesity and upper airway control during sleep.

Mechanisms linking obesity with upper airway dysfunction in obstructive sleep apnea are reviewed. Obstructive sleep apnea is due to alterations in upper airway anatomy and neuromuscular control. Upper airway structural alterations in obesity are related to adipose deposition around the pharynx, which can increase its collapsibility or critical pressure (P(crit)). In addition, obesity and, particularly, central adiposity lead to reductions in resting lung volume, resulting in loss of caudal traction on upper airway structures and parallel increases in pharyngeal collapsibility. Metabolic and hu

Region specific Raman spectroscopy analysis of the femoral head reveals that trabecular bone is unlikely to contribute to non-traumatic osteonecrosis.

Non-traumatic osteonecrosis (ON) of the femoral head is a common disease affecting a young population as the peak age of diagnosis is in the 40 s. The natural history of non-traumatic ON leads to a collapse of the femoral head requiring prosthetic replacement in a 60% of cases. Although trabecular bone involvement in the collapse is suspected, the underlying modifications induced at a molecular level have not been explored in humans. Here, we examine changes in the molecular composition and structure of bone as evaluated by Raman spectroscopy in human end-stage ON. Comparing samples from femor

Fracture of a Supera Interwoven Nitinol Stent After Treatment of Popliteal Artery Stenosis.

PURPOSE: To present a Supera stent fracture following treatment of popliteal artery stenosis. CASE REPORT: A 60-year-old man previously treated with angioplasty/stenting of a popliteal artery lesion with a Supera stent presented at 8 months with in-stent occlusion that proved to be secondary to stent fracture. CONCLUSION: Supera interwoven nitinol stents, despite having higher radial force and conformability than classic tube stents, are not free from fracture. Moreover, due to their peculiar structure, a break in these stents results in complete loss of integrity and consequent device colla

The muscles of the upper airways.

The upper airways are a complex structure with multiple functions; many of the muscles participate in the act of respiration. The neural control of upper airway muscles is distinct from that of chest wall muscles under a variety of circumstances. Coordinated activation of upper airway muscles, both regionally and inter-regionally, results in changes in upper airway size and resistance, alterations in the route of airflow, and increases in the ability of the airways to resist collapse. Several disorders have now been described in which neuromuscular drive abnormalities and/or mechanical dysfunc

From "Passive Response" to "Controllable Design": The Evolution of Crystal Structure and Morphology of Hollow FePt Nanochains by Annealing Treatment.

Atomic migration behavior in reactive atmospheres, especially the competitive mechanism between short- and long-range transport, is vital to determine the stability and performance of alloy nanomaterials. In this work, taking hollow Fe3Pt7 nanochains as a model system, in situ scanning electron microscopy and molecular dynamics simulations were combined to reveal the effects of annealing temperature and reaction atmosphere on the structure evolution. Vacuum annealing from room temperature to 900 °C results in size compression, pore formation, collapse of the hollow structure, and phase transit

Trafficking defects in WASH-knockout fibroblasts originate from collapsed endosomal and lysosomal networks.

The Arp2/3-activator Wiskott-Aldrich syndrome protein and Scar homologue (WASH) is suggested to regulate actin-dependent membrane scission during endosomal sorting, but its cellular roles have not been fully elucidated. To investigate WASH function, we generated tamoxifen-inducible WASH-knockout mouse embryonic fibroblasts (WASHout MEFs). Of interest, although EEA1(+) endosomes were enlarged, collapsed, and devoid of filamentous-actin and Arp2/3 in WASHout MEFs, we did not observe elongated membrane tubules emanating from these disorganized endomembranes. However, collapsed WASHout endosomes h

Collapse After Carpal Bone Tunneling: An Underreported Complication?

PURPOSE: To report the complication of carpal bone tunnel collapse in scapholunate reconstruction. METHODS: The authors present six cases of carpal collapse or bone necrosis after bone tunneling for ligamentous reconstruction, three in the scaphoid bone and three in the lunate. RESULTS: All six cases were secondary to ligament reconstruction for chronic scapholunate dissociations, none of which had preoperative structural scaphoid or lunate deformities. CONCLUSIONS: Although the incidence of this occurrence is unknown, the consequences could be substantial; therefore, patients should be war

The relationship of external and internal sidewall dimensions in the adult Caucasian nose.

OBJECTIVES/HYPOTHESES: Nasal bone length is commonly referenced in the rhinoplasty literature. It has been suggested that short nasal bone length may predispose one to a greater risk of middle vault collapse after rhinoplasty. However, there are limited data available on what constitutes the normal dimensions of these pertinent structures of the nasal sidewall. In addition, no data exist on the gender and ethnic variability of such dimensions. This article reports on measurements of nasal bones and associated structures in adult Caucasian cadavers and their relationships to the nasal sidewall.

Objective Evaluation of Nasal Obstruction in Cleft Lip and Palate Patients: A Preliminary Study.

Introduction: Cleft lip and/or palate (CLP) is frequently associated with persistent nasal obstruction, often due to structural deformities unaddressed by primary surgical repair. While subjective assessment tools are commonly used to evaluate nasal patency, they underestimate functional impairment, particularly nasal valve collapse. This study aims to objectively evaluate nasal obstruction and identify its anatomical causes in CLP patients after primary rhinoplasty. Methods: We conducted an observational study involving 21 children aged 8-16 with CLP who had undergone primary cheilorhinoplast

Genioglossus Advancement and Hyoid Surgery.

The structure and dimensions of the mandible, tongue, and hyoid complex are important variables in the pathophysiology of obstructive sleep apnea at the hypopharyngeal level. Genioglossus advancement is based on mandibular osteotomy, which brings the genioglossus muscle (GGM) forward and prevents posterior collapse during sleep. The genioglossus advancement technique has recently undergone several modifications; each has attempted to minimize surgical morbidity while improving the incorporation and advancement of the GGM. The hyoid bone has been of interest in sleep apnea and apnea-related sur

Nuclear medicine imaging in osteonecrosis of hip: Old and current concepts.

Osteonecrosis (ON) is caused by inadequate blood supply leading to bone death, which results in the collapse of the architectural bony structure. Femoral head is the most common site involved in ON. Magnetic resonance imaging (MRI) is a commonly used imaging modality to detect early ON. When MRI is inconclusive, bone scan is helpful in detecting ON during early phase of the disease. As newer nuclear medicine equipment, like single photon emission computed tomography/computed tomography (CT) and positron emission tomography/CT, are emerging in medical science, we review the role of these imagin

Complete palmar dislocation of the lunate in rheumatoid arthritis. Avascularity without avascular changes.

We report a case of complete palmar dislocation of the lunate in a rheumatoid patient. X-rays showed a normal bone structure of the lunate without sclerosis or collapse and on MRI an almost normal signal intensity was found. The lunate was removed. Histological examination showed complete necrosis of both marrow and bone cells, and tetracycline labelling showed no fluorescence. This case illustrates that neither X-ray nor magnetic resonance imaging (MRI) can detect complete bone necrosis. When X-ray or MRI changes do occur, these are indications of cellular events following some degree of spon

Increased Expiratory Computed Tomography Density Reveals Possible Abnormalities in Radiologically Preserved Lung Parenchyma in Idiopathic Pulmonary Fibrosis.

OBJECTIVES: Idiopathic pulmonary fibrosis (IPF) is a progressive lethal chronic lung disease with unclear pathogenesis. Radiological hallmark is the pattern of usual interstitial pneumonia accentuated in peripheral and basal areas with otherwise preserved lung structure. One hypothesis is that alveolar collapse and consequent induration lead to fibrotic transformation of lung tissue. The aim of the study was to investigate normal-appearing tissue during expiration for signs of collapsibility and differences from other diseases or controls. MATERIALS AND METHODS: We retrospectively assessed a

Adaptive radiotherapy in a case of adenoid cystic carcinoma of bronchus and its favourable impact on treatment outcome: A case report.

Adenoid cystic carcinoma is a very rare malignancy commonly originating from the salivary glands of the head and neck. It is again very scantily seen in sites like the bronchus. Surgical resection is the mainstay of treatment but many a times the tumour site and size preclude a favourable outcome of surgery and the patient is advised for other forms of local treatment like Radiotherapy. Here, we present a case report of a young female with primary adenoid cystic carcinoma of the bronchus. The tumour was located in the carina and the left bronchus which was obstructing the airway resulting in c
