Stuttering: an investigation into cerebral dominance for speech.

Over the past decade research has suggested that stutterers have bilateral cerebral motor or auditory speech areas. Three typical adult stutterers showed normal unilateral left cerebral dominance for speech on the intracarotid sodium amylobarbitone (amytal) test, but one 'dysphatic' stutterer had bilateral cortical speech representation. The latter is a very rare finding in right handed individuals and presumably is a consequence of the head injury induced aphasia that preceded the onset of stuttering.

Solution-Focused Brief Therapy for Stuttering in the Public Schools: Children Solve Their Own Stuttering Problems in This Case Study.

PURPOSE: This clinical focus article follows the case studies of three school-age children who stutter in solution-focused brief therapy (SFBT), highlighting treatment features and demonstrating positive outcomes. Empowerment and self-agency are emphasized as desired characteristics. Children searched within themselves and acted to influence therapy results. Techniques such as self-disclosure and fluency shaping were incorporated into this approach. METHOD: In a public school district, participants with moderate-to-severe stuttering used the Clinical Use of Self-Reports to measure their perce…

Fluency Should Not Be a Goal in Stuttering Therapy.

PURPOSE: Fluency is an understandable hope, but this article argues that it should not be a goal in stuttering therapy. The problem is not that easier speech is undesirable, nor that fluency cannot increase for many people who stutter. The problem is that current therapies do not directly train fluency itself. In stuttering, moreover, fluency is often used to refer to both fluent-sounding speech and speech experienced as spontaneous, comparatively less effortful, and relatively unselfconscious, and those meanings are not equivalent. METHOD: The article distinguishes fluent-sounding speech fro…

Stuttering: A Disorder of Energy Supply to Neurons?

Stuttering is a disorder characterized by intermittent loss of volitional control of speech movements. This hypothesis and theory article focuses on the proposal that stuttering may be related to an impairment of the energy supply to neurons. Findings from electroencephalography (EEG), brain imaging, genetics, and biochemistry are reviewed: (1) Analyses of the EEG spectra at rest have repeatedly reported reduced power in the beta band, which is compatible with indications of reduced metabolism. (2) Studies of the absolute level of regional cerebral blood flow (rCBF) show conflicting findings, …

Theory and therapy in stuttering: a complex relationship.

UNLABELLED: There are many treatments currently available for stuttering, for both children and adults. These range from direct interventions intended to reduce the severity and/or frequency of the speech behaviors of stuttering, to those intended to alleviate the anxiety and other mental health issues that can accompany the disorder. However, as there are little supporting data for many of these treatments, there is little consensus about which to use. Another way to evaluate stuttering treatments is to explore the extent to which they address the cause of the disorder. However, the cause of …

[Stuttering in preschool age: a survey of studies].

Most often stuttering is identified with a speech disorder or speech disfluency. However, it resembles a disorder consisting of linguistic, biological, psychological and social factors, as well as relations between them. The onset of stuttering usually occurs in the preschool age or even earlier, though relatively little concern has been devoted to this period. The article presents a review of the latest research on stuttering in preschool age. A distinct disproportion between particular elements of stuttering structure have been observed. Research concerns mainly the linguistic factor and is …

Olanzapine for the treatment of acquired neurogenic stuttering.

Stuttering is a vocal phenomenon, which manifests itself as disturbances in speech fluency. While stuttering is most commonly treated with speech therapy and psychotherapy, a number of antipsychotic agents have been investigated as possible treatments. We present the case of a 37-year-old man who developed a post-concussive syndrome with psychosis and associated stuttering after his second exposure to a blast from an improvised explosive device (IED). After treatment with olanzapine, both his psychosis and his stuttering showed significant improvement. We also discuss stuttering and review pre…

Penile fracture in a patient with stuttering priapism.

Penile fracture commonly results from trauma of sexual intercourse or masturbation. It is common in the Middle East and America, but rare in Nigeria and sub-Sahara Africa. We present a case of penile fracture, an uncommon urologic emergency, complicating priapism, another urologic emergency; precipitated in an unusual circumstance. This report illustrates a 30-year-old undergraduate who has suffered stuttering priapism for about a week and developed penile fracture while forcefully packing the erect organ. He presented early in the hospital and had emergency surgical repair. Prognosis was good…

Facing the challenge of treating stuttering in the schools.

There are many difficult issues facing speech-language pathologists who work with school-age children who stutter; however, the challenges are not insurmountable. Effective treatment for children who stutter is possible, even in school settings. This chapter introduces two issues of Seminars in Speech and Language aimed at helping clinicians improve their treatment of stuttering in school-age children. The first issue addresses basic aspects of evaluation and treatment, including procedures for assessing children's communication abilities, developing appropriate goals for therapy, and utilizin…

Successful treatment of stutter priapism with an antiandrogen.

Recurring periods of prolonged erections in patients with sickle cell disease (stutter priapism) are uncommon, yet troubling sequelae of hemoglobinopathies. Medical or surgical therapy is variably successful in these men. We report a case of stutter priapism successfully treated with an oral antiandrogen.

Parents as partners in the treatment of childhood stuttering.

Speech-language pathologists involved in treating childhood stuttering have increasingly emphasized the need for parental involvement in intervention. In this article, we review prevailing theoretical models of stuttering and illustrate various ways in which these models can be used to guide practitioners in determining how best to include parents in intervention. We argue that successful parental involvement depends upon the ability of parents and clinicians to reach consensus on issues such as the parents' role in the onset and development of stuttering, the rationale for including parents i…

A PET study of the neural systems of stuttering.

The cause of stuttering is unknown. Failure to develop left-hemispheric dominance for speech is a long-standing theory although others implicated the motor system more broadly, often postulating hyperactivity of the right (language nondominant) cerebral hemisphere. As knowledge of motor circuitry has advanced, theories of stuttering have become more anatomically specific, postulating hyperactivity of premotor cortex, either directly or through connectivity with the thalamus and basal ganglia. Alternative theories target the auditory and speech production systems. By contrasting stuttering with…

Hemispheric lateralization for speech perception in stutterers.

Sixteen adult, right-handed, moderate-to-severe atutterers (12 males, 4 females) and 20 nonstuttering controls (10 males, 10 females) were given a dichotic nonsense-syllable test to determine hemispheric lateralization for speech. Both male and female stutterers evidenced right-ear advantages in syllable identification similar in magnitude to those found for normals. These data confirm other reports of no difference in cerebral speech lateralization for stutterers and nonstutterers and, therefore, lend no support to theories that relate stuttering to abnormalities in cerebral lateralization. A…
