Drug Injection-Related Norms and High-Risk Behaviors of People Who Inject Drugs in Athens, Greece.

Drug use involves social interactions. Therefore, norms in the proximal environment of people who inject drugs (PWID) can favor behaviors that may result in HIV transmission. This work aimed at studying drug injection-related norms and their potential association with risky behaviors among PWID in Athens, Greece, in the context of economic recession and political activism that followed the fiscal crisis and soon after a recent HIV outbreak had leveled off. The Transmission Reduction Intervention Project (TRIP) was a social network-based approach (June 2013 to July 2015) that involved two group

Sclerosing angiomatoid nodular transformation of the spleen: unusual case presentation in an intravenous drug user.

An unusual presentation of sclerosing angiomatoid nodular transformation in a 42-year-old man who was admitted with jaundice, deranged liver function tests and subsequently diagnosed with acute hepatitis C infection in the context of recent intravenous drug use. During his admission, he had an ultrasound of the abdomen followed by a CT thorax, abdomen and pelvis which showed splenomegaly and a large splenic lower pole mass that was hypoechoic and concerning for lymphoma. A bone marrow biopsy showed no evidence of lymphoma and an ultrasound-guided biopsy of the splenic mass suggested unusual fe

Sequelae of parenteral drug abuse involving the external genitalia.

UNLABELLED: Intravenous drug abuse continues to be a major social problem in the USA. Many drug abusers eventually run out of usual intravenous sites and begin to utilize more unusual sites. Some of these sites include the cervical veins and the superficial dorsal vein of the penis. Sometimes the vein is missed and there is extravasation of the drug subcutaneously or intracorporeally. Acute cavernositis, priapism, penile subcutaneous hematoma, penile ulcerations and corporal fibrosis as well as Fournier's gangrene are some of the sequelae of these adventures. We describe our experience with th

Skin and soft tissue infections in injection drug users.

Skin and soft tissue infections are the most common cause for hospital admission of injection drug users. Cutaneous and subcutaneous abscesses are the most frequent type of SSTI and occur most commonly when drug users are no longer able to inject intravenously and resort to injection directly into skin or muscle. Abscesses may be difficult to differentiate from uncomplicated cellulitis or may be confused with pseudoaneurysms, hematoma, phlegmon, or thrombosed vein. Special studies, including ultrasonography; CT scans, and MR imaging; or careful incision and inspection may be necessary to clari

HIV incidence among needle exchange participants: estimates from syringe tracking and testing data.

This paper develops a statistical procedure for estimating the HIV infection rate among needle exchange clients without using any self-reported information. Instead, data are accumulated by following the distribution and return of sequentially labeled syringes and by testing a sample of returns for the presence of HIV-1 proviral DNA using polymerase chain reaction. For each drug injector in the sample, a maximum likelihood change point model is constructed to determine if a statistically significant upward shift in the fraction of needles testing HIV positive is evident, as would occur if the

[Candida albicans meningitis in 2 parenteral drug addicts. Review of the literature].

BACKGROUND: HIV infection and parenteral drug abuse (PDA) are frequently associated conditions. Both are at increased risk of developing specific types of Candida infections. Localized CNS infection is an exceptional finding. METHODS: We report two cases of meningitis in PDA due to Candida albicans--one of them HIV positive, and review the literature emphasizing the characteristics of 8 HIV positive patients 4 of them PDA. RESULTS: Most frequent presentation symptoms were fever and headache for more than two weeks. Three patients were found normal on neurologic examination. In two cases CT s

[Hodgkin's disease associated with infection by human immunodeficiency virus].

We present four patients with Hodgkin's disease and human immunodeficiency virus infection (HIV). Three patients were addicts to parenteral drugs. The disease was very advanced at the time of diagnosis, stage IVB in all cases. Histologic types were mixed cellularity in two cases, nodular sclerosis in one case and it was not typifiable in the remaining case. The disturbances of cell-mediated immunity that the patients presented were those of the acquired immunodeficiency syndrome (AIDS) as well as the infectious complications. Response to treatment was poor with a high incidence of bone marrow

Barriers and facilitators to initiating and adhering to harm reduction services among people who inject drugs in the United States: a systematic review.

BACKGROUND: Harm reduction services are inequitably accessible and efficient among people with a history of injection drug use (PWHID), particularly among those belonging to vulnerable communities. This systematic review identifies barriers and facilitators to accessing and adhering to harm reduction services among PWHID in priority populations to gain a deeper understanding of the intersectional barriers to these services. METHODS: A systematic search was conducted using four databases (PubMed/MEDLINE, Scopus, CINAHL, and Web of Science) to locate articles examining barriers and/or facilitato

Addressing policy barriers to scaling up needle and syringe programmes: a global call to action.

Needle and syringe programmes (NSPs) are effective, affordable solutions for preventing the transmission of blood-borne viruses among people who inject drugs. Yet, global NSP coverage remains extremely low; only 2% of people who inject drugs live in countries with high coverage, and many low-income and middle-income countries do not have NSPs. This Health Policy reports outputs from an international working group who used implementation science approaches to prioritise barriers and co-design solutions to scale up NSPs across three domains: global policy, national policy, and procurement. We pr

Evaluating spatially targeted HIV interventions and harm reduction services among people who inject drugs in a high-burden setting.

People who inject drugs (PWID) in India continue to experience high HIV incidence while coverage of HIV and harm reduction services within this population remains suboptimal in many settings, highlighting the need to identify novel service delivery points. To evaluate the effectiveness of spatially focused upscaling of interventions at observed venues where PWID injected drugs together, we developed an individual-based dynamic transmission model of HIV informed by detailed injection network, service engagement, and injection venue attendance data collected in a sociometric study of PWID (n = 2

Potential reasons for the decline of new HIV cases among people who inject drugs (PWID) in Kyrgyzstan.

There has been a declining trend in the number of reported cases of human immunodeficiency virus (HIV) among people who inject drugs (PWID) in Kyrgyzstan. The local HIV and public health community has suggested that this decline may be driven by interventions targeting HIV transmission among PWID, changes in drug use patterns, or possible underreporting of new HIV cases. The present study aims to examine these hypotheses using a deterministic compartmental model. Three intervention scenarios were evaluated to assess their impact on PWID population trends, HIV incidence, and case reporting. Sce

Validation of case-ascertainment algorithms using health administrative data to identify people who inject drugs in Ontario, Canada.

OBJECTIVES: Health administrative data can be used to improve the health of people who inject drugs by informing public health surveillance and program planning, monitoring, and evaluation. However, methodological gaps in the use of these data persist due to challenges in accurately identifying injection drug use (IDU) at the population level. In this study, we validated case-ascertainment algorithms for identifying people who inject drugs using health administrative data in Ontario, Canada. STUDY DESIGN AND SETTING: Data from cohorts of people with recent (past 12 months) IDU, including thos
