Internal fixation of delayed union of fracture with chronic osteomyelitis due to Staphylococcus epidermidis: A case report.

INTRODUCTION: The most feared and challenging complication in treating patients with musculoskeletal trauma is infection after fracture fixation, which can delay healing and, in turn, result in permanent functional loss of limbs or even amputation. CASE PRESENTATION: Here, we describe a case in which a patient presented with stage 4 chronic osteomyelitis on the right tibia and fibula. To treat the late infection by eradicating or reducing the infection, it was recommended to replace the internal fixation with surgical debridement for 6 weeks and antibiotics for 6-12 weeks until the internal f

Bilateral second metatarsal stress fractures after hallux valgus correction with the use of a tension wire and button fixation system.

The pathomechanics and treatments of hallux valgus vary widely by deformity and surgeon, and are extensively described in the orthopedic and podiatric literature. With each newly described treatment, new complications can be encountered. In this report, we describe the case of a 22-year-old woman who underwent bilateral hallux valgus repair with a tension wire and button fixation system, and developed bilateral second metatarsal stress fracture at the point of fixation application during the postoperative period. Although this fixation system has been useful in our practice, like other surgica

[Clinical application and results with fixateur externe in septic and aseptic osteosyntheses at the lower extremity (author's transl)].

Osteosynthesis with fixateur externe is indicated in unstable, infected fractures, infected pseudarthroses and defect pseudarthroses, second-degree and third-degree open fractures as well as closed or unstable old fractures and pseudarthroses with marked damage of the soft parts, as well as in all arthrodeses of the knee joint or ankle joint (foot arthrodeses). The main field of application is the shaft of the lower leg with its adjacent joints. To achieve satisfactory stability, mounting should always be three-dimensional and numerous additional details must be taken care of, all of which are

[Possibilities of application of the fixateur externe at the femur - indications, results (author's transl)].

Besides the methods of screwing of plates and medullary nailing, the fixateur externe (F.E.) is another method which can be considered for treating bone fractures at the femur, F.E. being an exceptional indication. In fresh fractures of the femur, open fractures of the second and third grade as well as debris and multiple-plane fractures are typical indications for F.E. Over and above, F.E. is almost always indicated as an emergency measure in case of initial inoperability (polytrauma, shock, loss of blood, craniocerebral trauma) or continuing general inoperability. F.E. has been found useful

Systematic review of 3D-printed surgical devices for austere environments: an evaluation guideline.

BACKGROUND: Three-dimensional (3D) printing is a common manufacturing technique. It is being adopted to produce essential surgical equipment in austere settings around the world. However, the evaluation of these products remains inconsistent. This raises safety concerns and limits translation to clinical use. To address this, this systematic review aims to identify 3D-printed surgical devices intended for austere environments and examine their evaluation methodologies. The findings will inform the development of an evaluation framework that supports the design of patient-safe devices. METHODS

Biomechanical comparison of different implants for PIP arthrodesis.

BACKGROUND: Surgical correction of hammertoe deformities with arthrodesis of the proximal interphalangeal joint (PIP) is one of the most frequent forefoot procedures. Recently, new intramedullary fixation devices for PIP arthrodesis have been introduced. The aim of this study was to compare a newly developed absorbable intramedullary implant made of magnesium (mm.PIP), an already available intramedullary implant made of titanium (PipTree), and the classical Kirschner-wire (K-wire). METHODS: The three intramedullary devices (mm.PIP, PipTree, and K-wire) for PIP arthrodesis were compared. A cla

Case series study and finite element analysis of a new cervicothoracic fixation device.

OBJECTIVE: To introduce a specialized device designed for the fixation of the cervical-thoracic spine and describe its surgical installation, finite element analysis, and clinical outcomes. METHODS: A finite element model of the C6-T1 segment was developed and validated, and simulations of subtotal resection of anterior cervical vertebrae, artificial vertebrae placement, and titanium plate fixation were performed. The model was subjected to a 75 N load to simulate the weight of the head and a 1 N·m force to simulate flexion, lateral bending, and axial rotation. The analysis focused on range o

Biomechanical evaluation of internal fixation implants for femoral neck fractures: A comparative finite element analysis.

BACKGROUND AND OBJECTIVE: It remains controversial regarding the optimal type of fixation implant for the treatment of femoral neck fractures (FNFs). Biomechanical rational for implant choices can benefit from the integration of finite element analysis (FEA) in device evaluation and design improvement. In this study, we aim to evaluate biomechanical performance of several internal fixation implants for Pauwels type III FNFs under physiological loading conditions using FEA, as well as to assess the biomechanical contribution of medial buttress plate (MBP) augmentation. METHODS: Several fixatio

Arthroscopic Reduction and Internal Fixation of Posterior Cruciate Ligament Avulsion Fracture Using an Adjustable-Length Loop Device.

A displaced avulsion fracture at the tibial attachment of the posterior cruciate ligament is considered an indication for surgical reduction and internal fixation because nonunion and remaining posterior instability of the knee are common consequences of conservative treatment. The problems with standard open surgical techniques are that they are relatively invasive despite the limited operative field and it is impossible to explore intra-articular lesions by the posterior approach. An arthroscopic procedure has the advantage of being minimally invasive and allowing the surgeon to detect and t

Biomechanical characteristics of a novel interspinous distraction fusion device in the treatment of lumbar degenerative diseases: a finite element analysis.

BACKGROUND: A novel interspinous distraction fusion (ISDF) device has been used to treat lumbar degenerative diseases. As a minimally invasive technique, ISDF differs from the traditional interspinous process distraction devices. Currently, biomechanical studies on ISDF are rare. OBJECTIVE: To investigate the biomechanical properties of the ISDF device (BacFuse) which is used to treat lumbar degenerative diseases. METHODS: Three-dimensional L3-L5 models were created. The models were divided into four groups: intact (M1), local decompression alone (M2), internal fixation alone (M3) and local

Decompressive procedures. Indications and techniques.

Hemilaminectomy and dorsal laminectomy are the surgical techniques most suited for decompression of the thoracic, thoracolumbar, lumbar, and lumbosacral spine. Hemilaminectomy is preferred for decompressing the spinal cord before applying fixation devices for fractures/luxations. Dorsal laminectomy is the method of choice for exploration of the spinal canal and for decompression of the lumbosacral region. Either method is applicable to removal of herniated intervertebral disc material and fenestration of other disc spaces.

Closed management of distal femoral fractures.

Since closed treatment is effective in 80% to 85% of distal femoral fractures, operative intervention is not recommended. Unnecessary surgery produces unwarranted complications. The cast-brace technique should be used in the first week or two after injury to prevent deformities commonly associated with distal femoral fractures. The thigh cast should be carefully molded well over the femoral condyles. Distal femoral traction is preferred to proximal tibial traction to correct fracture deformities. If the closed reduction technique is not satisfactory in the early period after injury, operative

Atlantoaxial Joint Distraction and Fusion with DTRAX Intra-Articular Cages: A Cadaveric Feasibility Study and Review of the Pertinent Literature.

PURPOSE: Atlantoaxial joint distraction is a key procedure for the treatment of selected patients affected by basilar invagination (BI). In recent years, several authors have reported various techniques of distraction and fixation of the C1-C2 joint using different types of intra-articular spacers, with or without posterior fixation. We review the pertinent literature and propose a feasibility study on the use of a new device for the distraction of the C1-C2 joint aimed to the descent of the dens out of the foramen magnum suggesting its application on selected cases of BI. METHODS: The GL-DTR

A comparative analysis of titanium anatomic plate and titanium reconstructive plate for treatment of extra-articular fractures of the scapula (Miller types IIb, IIc, and IV).

BACKGROUND: Because of the irregular shape of the scapula and the different types of fractures, a standard internal fixation device is lacking in clinical practice, even though there are many options available. This study aimed to compare the therapeutic efficacy of titanium anatomic and reconstructive plates for extra-articular fractures of the scapula (Miller types IIb, IIc, and IV). METHODS: A retrospective study of 41 patients who underwent treatment for extra-articular fractures of the scapula between March 2017 and March 2020 was conducted. Use of Blount's epiphyseal staples to control massive iliac venous haemorrhage.

Significant injuries to major veins are life-threatening events and may lead to patient exsanguination. Treatment usually consists of direct suture repair. A case in which a serious iliac venous haemorrhage was not able to be controlled with the usual surgical procedures is reported. A desperate attempt to achieve haemostasis using orthopaedic staples was successful. The simplicity of this technique makes it a useful addition to the surgeon's armamentarium when dealing with life-threatening venous haemorrhage.
