The Washington Regimen: rehabilitation of the hand following flexor tendon injuries.

This article describes the use of the "Washington Regimen" of early controlled motion in the rehabilitation of flexor tendon injuries of the hand. This regimen is derived from a combination of Kleinert's controlled active extension with rubber-hand passive flexion, Duran's controlled passive techniques, and the modification of the Kleinert orthosis that uses a palmar pulley system. Based on results of clinical investigations, this regimen of early controlled motion appears effective in inhibiting peritendinous scarring, joint contractures, and other complications that commonly occur secondary …

Extensor tendon injuries. Presentation of a series of ninety-nine cases.

A series of 99 cases of injuries of the extensor system, followed in the 3 Traumatology and Orthopedics Ward (Dijon, France) is presented. Results were analyzed according to Verdan's classification. In most cases, a Jenning "barb-wire" type repair, introduced in France by Y. Allieu, was made. Early mobilization and skin tension problems are emphasized; two cases of degloving on the dorsum of the hand are presented with details. As well, a special place concerning the results of repair in the proximal interphalangeal joint must be reserved, because of the very frequent association of articular

Injuries of the Extensor Tendons of the Hand and Forearm.

Extensor tendon injuries are commonly encountered after trauma. These injuries often result in acute weakness and have widely differing treatment options depending on the severity and the location of the injury within the upper extremity. Zone I injuries can often be treated nonoperatively with the potential for pinning of any large bony avulsion fragments. For zone II injuries, the tendons are often amenable to surgical repair with extension splinting for 6 weeks. Injuries at the central slip in zone III are often treated with closed extension splinting. In zone IV, these injuries are often t…

Stem cell therapy for tendon injury.

Tendon injury may occur suddenly or progressively, and can be divided into tendon rupture or tendinopathy based on the severity of injury. It is frequently found in professional or nonprofessional people who are making repetitive movements. In aged people, tendon degeneration becomes obvious; their tendon injuries are then frequently evident. No effective therapies for tendon injury are currently available. In this article, we review the tendon structure, mechanisms of tendon injury, and tendon healing process. More importantly, cell-based therapies for tendon injury are fully addressed, which…

The athlete's hand: ligament and tendon injury.

Hand injuries are common, accounting for up to 20% of emergency department visits nationwide. An understanding of the complex and detailed anatomical relationships of small joints facilitates the accurate diagnosis of many common sports-related injuries of the hand. We review the common ligamentous and tendinous injuries of the hand including collateral ligament injuries, gamekeeper's thumb, extensor mechanism injuries, pulley injuries, boutonniere deformities, and mallet and jersey lesions. The mechanism of injury, clinical presentation and treatment, pertinent anatomy, and imaging findings a

MR imaging of muscle and tendon injury.

The nature and MRI appearance of injuries to the muscle including delayed onset muscle soreness, muscle contusion, muscle strain, and compartment syndromes are described and examples shown. The nature of degenerative disease of tendon (tendinosis) is discussed and representative examples of the MRI appearance of various stages are illustrated. The role of MR in the clinical management of these disorders is discussed where appropriate.

Tendon injury repair in a war wound.

During 18 months of the 1991-1992 war against Croatia, 4425 injured were treated at the Department of Surgery, Osijek University Hospital. Among them 46 had tendon injuries, 20 with tendon laceration and 26 with total rupture. In 12 wounded primary repair was performed, 4 of an extensor tendon, 7 of a flexor tendon and 1 of both flexor and extensor. Four patients with primary flexor tendon repair had a good functional result. In other 34 no primary repair was done. Good function was achieved in 18 cases.

[Isolated injury of the subscapular muscle tendon].

Avulsion of the subscapularis tendon is caused by a combined abduction and external rotation trauma of the upper limb. Weakness of internal rotation and a positive apprehension test are clinical signs of the injury. A fracture of the lesser tuberosity in the x-ray film or a disruption of the subscapularis tendon documented in dynamic sonography of the shoulder clarify the diagnosis. Operative treatment is indicated of prevent weakness of internal rotation as well as anterior instability. This recommendation is supported by four cases with full range of motion and good anterior stability after

Tendon transfers for brachial plexus injury.

Brachial plexus injuries result in severe functional deficits in the upper limb. The authors review a group of 74 patients with brachial plexus injuries who underwent 160 tendon transfer operations, as well as 94 additional procedures, in an attempt to augment lost function. Following evaluation of functional recovery, 58 percent of the patients were rated Good, 34 percent Improved, and 8 percent Unimproved. The authors conclude that significant benefit can be obtained by peripheral reconstruction and tendon transfers in patients with brachial plexus injuries.

Lateral Epicondylitis/Extensor Tendon Injury.

Pain over the lateral aspect of the elbow without nerve injury or elbow instability often is diagnosed as lateral epicondylitis or, colloquially, tennis elbow. It is a common complaint, seen most frequently in women between ages 40 and 60, although it is common in men too. Typical presenting symptoms include pain with prolonged wrist extension activities, pain with resisted wrist or elbow extension, and pain at rest radiating from the elbow along the dorsum of the forearm.

[Injuries of the peroneal tendons : Often overlooked].

CLINICAL ISSUE: Injuries of the peroneal tendons are rare and often overlooked. Typical pathologies are tendinitis, tears and dislocation. Accompanying injuries are fractures. They are often associated with instability in the ankle and rearfoot deformities; therefore, these pathologies should be excluded or taken into consideration in the treatment. The clinical examination is crucial for the diagnosis. DIAGNOSTIC WORK-UP: Ultrasound and magnetic resonance imaging (MRI) examinations are very helpful; however, the true extent of the tendon pathology is often first seen during surgery. Bony inj

Extensor tendon: anatomy, injury, and reconstruction.

Although seemingly simple in its anatomy and function, the extensor mechanism of the hand is actually a complex set of interlinked muscles, tendons, and ligaments. A thorough understanding of the extensor anatomy is required to understand the consequences of injury at various levels. Reconstructive options must restore normal function. Whereas primary repair of anatomic structures is frequently possible in acute injury, it is rarely possible in chronic situations. Technically exacting procedures may be necessary to restore function.

Ultrasound Diagnosis of A2 Pulley Injuries: Landmark-Specific Tendon-Bone Distance and Flexor Tendon Curvature in a Cadaveric Study.

BackgroundA2 pulley ruptures are common in rock climbers, yet diagnosing partial ruptures remains challenging due to variability in tendon-bone distance (TBD) measurements in ultrasound assessments.ObjectiveThis study evaluated TBD at multiple anatomic landmarks to differentiate intact, partial, and complete A2 pulley ruptures; compared landmarks for optimal ultrasound diagnosis; and investigated flexor tendon curvature as an additional indicator.MethodsForty-two fingers from 14 fresh-frozen cadaveric arms were examined using high-frequency ultrasound. Fingers were assigned to 5 simulated grou
