This article provides video and narration to describe the technical details of how to inject local anesthesia and perform the extensor indicis proprius to extensor pollicis longus tendon transfer in patients with wide-awake local anesthesia and no tourniquet. Lidocaine for anesthesia and epinephrine for hemostasis are the only 2 medications given to the patient. Sedation and the tourniquet are not required. Wide-awake patients are comfortable, cooperative, and educable, and are able to help the surgeon set the correct tension for the transfer. They flex and extend the thumb before the skin is …

Ultrashort echo time imaging allows the short T(2) Achilles tendon to be directly visualized with MRI. Radiofrequency saturation 1 kHz or less off-resonance has been used previously to improve image contrast. In this study, magnetization transfer was investigated in the Achilles tendon of eight normal volunteers and one patient with psoriatic arthritis. 2D Ultrashort echo time images were acquired using saturation pulses 2-100 kHz off-resonance at 4 pulse powers. On-resonance saturation recovery images were also obtained to estimate T(1). The results were fitted to a two compartment quantitati

OBJECTIVES: The transfer of the tibialis posterior tendon to the paralysed tendons on the anterior aspect of the ankle not only restores the function of the paralyzed muscles, but also removes the deforming force on the medial aspect of the foot. In this study, we evaluated patients who underwent tibialis posterior tendon transfer for the treatment of drop foot.
METHODS: The study included 41 patients (24 males, 17 females; mean age 32 years; range 11 to 73 years) who underwent tibialis posterior tendon transfer for drop foot. The mean duration of paralysis was 51.5 months (range 4 to 240 mon…

OBJECTIVE: Irreparable rotator cuff injuries in young patients with moderate to high levels of physical activity remain a challenging pathology for shoulder surgeons. Irreparable anterior rotator cuff injuries require treatment that seeks to restore the dynamic anterior forces of the glenohumeral joint. Among surgical treatment options, transfer of the pectoralis major and minor muscle, have shown good functional outcomes. This technique attempts to reproduce the vector of the subscapular muscle in cases with irreparable tear. Restoration of the dynamic external and internal couple forces to m

Background: Temporalis tendon transfer (TTT) often relies on external incisions in the nasolabial fold, temporal region, or both. Herein, we studied smile outcomes of a TTT technique via a single intraoral incision without external skin incisions. Objective: To measure the difference in static perioral positions before and after intraoral TTT. Methods: Five patients underwent an externally scarless TTT technique via an intraoral approach. Iris measurements were used to estimate pixel-to-millimeter conversion for facial analysis of vertical and horizontal oral commissure excursion vectors using…

Split anterior tibialis tendon transfer is a common orthopedic surgical procedure for varus foot deformities. In the absence of the peroneus tertius, the peroneus brevis may be used in the transfer. Little is known about the prevalence of each transfer type. The goal of this study is to understand trends in tendon transfer for patients who undergo split anterior tibialis tendon transfer. Records of all patients who underwent tibialis anterior tendon split transfer at our institution between January 2004 and September 2020 were reviewed. After, 337 subjects who underwent tibialis anterior tendo…

BACKGROUND: Poor awareness among the patients and lack of resources for proper management of nerve injuries leads to delayed presentation of most traumatic ulnar nerve injuries. When this injury is present in the proximal forearm it leads to poor outcomes as Ulnar nerve repair with grafts takes a prolonged time to restore function. Addition of a tendon transfer to this procedure can allow patients to return to work earlier. The purpose of this study is to examine whether the addition of a tendon transfer to nerve repair surgery will lead to improved hand function and rehabilitation earlier tha…

INTRODUCTION: The main deformity following an adult lower brachial plexus injury is the loss of finger flexion. Distal nerve transfers have been used to restore finger and thumb flexion followed by tendon transfers for intrinsic replacement for opening of the fingers. When patients present beyond 6 months, only tendon transfers are applicable. Since the brachioradialis (BR) is always spared in such injuries, it is the ideal muscle to provide finger flexion. Wrist extensor power may not be normal for the use of the radial wrist extensor to serve as donor. BR to FDP transfer provides reasonable …

Pseudoaneurysm of the posterior tibial artery is an uncommon condition that, left untreated, can lead to hemorrhage, thrombosis, or emboli. We present the case of a 54-year-old male who developed pseudoaneurysm of the posterior tibial artery 4 months after undergoing tibialis posterior tendon transfer for management of peroneal nerve palsy, which had developed as a complication of hip arthroplasty.

Enhancement of upper-extremity function, specifically shoulder function, after brachial plexus injury requires a good understanding of nerve repair and transfer, with their expected outcome, as well as shoulder anatomy and biomechanics enabling the treating surgeon to use available functioning muscles around the shoulder for transfer, to improve shoulder function. Surgical treatment should address painful shoulder subluxation in addition to improvement of function. The literature focuses on improving shoulder abduction, but improving shoulder external rotation should take priority because this

Posttraumatic ulnar subluxation of the extensor tendon over the metacarpal head results from rupture of the radial sagittal fibers. The patient will complain of limited digital extension and pain. Various techniques have been described to correct the disorder by either reefing the sagittal fibers or using a slip of extensor tendon around the radial collateral ligament. Unfortunately, these techniques are either technically not feasible, reefing of the sagittal fibers, or result in significant stiffness, using a strip of extensor tendon. The author describes an extraarticular technique which us

Twelve patients with drop-foot secondary to sciatic or common peroneal nerve palsy treated with transfer of the tibialis posterior tendon were followed-up for a mean of 90 (24-300) months. In 10 patients the results were 'excellent' or 'good'. In 11 patients grade 4 or 5 power of dorsiflexion was achieved, although the torque, as measured with a Cybex II dynamometer, and generated by the transferred tendon, was only about 30% of the normal side. Seven patients were able to dorsiflex their foot to the neutral position and beyond. The results appeared to be better in men under 30 years of age wi

BACKGROUND: Transfer of the flexor hallucis longus (FHL) tendon aims to restore function and relieve pain in chronic Achilles tendon (AT) disease. The goal of the present study was to investigate the clinical and radiographic outcomes of FHL transfer to the AT and to compare the transtendinous technique to the transosseous technique. We hypothesized that the type of technique would have a notable impact on outcome.
METHODS: Forty patients (42 ankles) were retrospectively reviewed and divided into group 1 (transtendinous technique, 22 patients/24 ankles) and group 2 (transosseous technique, 18

BACKGROUND: Thumb replantation following complete traumatic avulsion requires complex techniques to restore function, especially in cases of avulsion at the level of the metacarpophalangeal joint (MCP I) and avulsion of the flexor pollicis longus (FPL) at the musculotendinous junction. Possible treatments include direct tendon suture or tendon transfer, most commonly from the ring finger. To optimize function and avoid donor finger complications, we performed thumb replantation with flexion restoration using brachioradialis (BR) tendon transfer with palmaris longus (PL) tendon graft.

The flexion deformity of the wrist in cerebral palsy is quite common, carrying disability to the effective use of the hands. From 1980 to 1990, 36 transferences of the pronator teres to the extensor carpi radialis were reviewed. The authors describe the operative technique by a unique incision, that may be used in cerebral palsy, and conclude that it is a good method for correction of deformity.

Twenty-four children with spastic equinovarus deformity due to cerebral palsy were treated by anterior transfer of the posterior tibial tendon and Achilles tendon lengthening. In five patients, the operation was performed on both sides, making a total of 29 feet available for evaluation after an average follow-up of five years. Only 38 per cent of the results were graded "good" or "satisfactory." Sixty-two per cent were rated as "poor" because of valgus, calcaneus or equinus deformity severe enough to require re-operation. The post-operative deformity was generally evident one or more years af

BACKGROUND: Both superior capsular reconstruction (SCR) and middle trapezius tendon (MTT) transfer can be performed in nonarthritic young and active patients with isolated irreparable supraspinatus tendon tears (IISTTs). However, to our knowledge, no comparative clinical studies have been conducted on these procedures.
PURPOSE: To evaluate and compare the clinical and radiological outcomes of SCR and arthroscopic-assisted MTT transfer in patients with IISTTs.
STUDY DESIGN: Cohort study; Level of evidence, 3.
METHODS: A total of 60 patients with IISTTs were categorized into 2 groups accordin
