The association of posterior thalamic strokes with the presence of chronic "thalamic" pain was described in the early 1900s and revisited in a recent review of these patients. Acute pain in corporal structures is associated with the spinothalamic tract (STT), which originates in the dorsal horn of the spinal cord, whereas that associated with cranial structures is associated with the spinal division of the trigeminal nucleus. These pathways terminate in the ventral posterior nucleus (VP), including its posterior and inferior subnuclei and its core, which is classically associated with tactile

OBJECTIVE: The thalamus is a key hub in seizure propagation, and its nuclei are emerging targets for neuromodulation. However, the contributions of individual nuclei to epileptic networks remain unclear, particularly in children, who are less studied than adults. We investigated structural and functional thalamic alterations across different pediatric focal epilepsies and their associations with clinical features and postsurgical outcomes.
METHODS: We retrospectively studied children with temporal lobe epilepsy (TLE), frontal lobe epilepsy (FLE), and posterior quadrant epilepsy (PQE) and heal

OBJECTIVE: Epilepsy patients are often grouped together by clinical variables. Quantitative neuroimaging metrics can provide a data-driven alternative for grouping of patients. In this work, we leverage ultra-high-field 7-T structural magnetic resonance imaging (MRI) to characterize volumetric atrophy patterns across hippocampal subfields and thalamic nuclei in drug-resistant focal epilepsy.
METHODS: Forty-two drug-resistant epilepsy patients and 13 controls with 7-T structural neuroimaging were included in this study. We measured hippocampal subfield and thalamic nuclei volumetry, and applie…

Awakening from sleep reflects a profound transformation in neural activity and behavior. The thalamus is a key controller of arousal state, but whether its diverse nuclei exhibit coordinated or distinct activity at transitions in behavioral arousal state is unknown. Using fast fMRI at ultra-high field (7 Tesla), we measured sub-second activity across thalamocortical networks and within nine thalamic nuclei to delineate these dynamics during spontaneous transitions in behavioral arousal state. We discovered a stereotyped sequence of activity across thalamic nuclei and cingulate cortex that prec…

The ventral midline thalamus comprises the reuniens and rhomboid nuclei (ReRh). These nuclei are bidirectionally connected not only with two key brain structures relevant to cognition - the (medial) prefrontal cortex and the hippocampus - but also with over thirty other brain regions, often reciprocally. Over the past two decades, these nuclei have attracted increasing interest for their roles in various memory functions, including working memory and systems consolidation, as well as in behavioral flexibility, emotions such as fear, and the regulation of plasticity. Unsurprisingly, the Re nucl

BACKGROUND: Functional stereotactic neurosurgery has been rediscovered as a powerful tool to treat various manifestations of medically refractory dystonia. The purpose of this study was to provide the first systematic review of previous experience with thalamic and basal ganglia surgery in the treatment of cervical dystonia (CD).
METHODS: A comprehensive review of the literature published between 1950 and 1990 was performed. In addition, data from an unpublished doctoral thesis on 162 patients with CD operated on by Mundinger between 1972 and 1986 were analyzed.
RESULTS: Overall, there is pu

1. Medial lemniscal axons were identified by extra- and intracellular recording in the thalamic ventral posterior lateral nucleus (VPL) of cats and injected intracellularly with horseradish peroxidase (HRP). 2. Axons were characterized in terms of their latencies of response to stimulation of the medial lemniscus in the medulla, their receptive fields, and the temporal patterns of their discharge in response to stimulation of the receptive field with natural, hand-held stimuli. One-hundred sixty-six axons were placed in five operational groups: hair transient (Ht) (n = 41); hair sustained (Hs)

The dorsal lateral geniculate nucleus (dLGN) of the mouse (strain 944 with microphthalmus syndrome, P. Hertwig 1942) has been morphometrically investigated. The eyes of the gray littermates develop normally (control animals) while the white littermates show the microphthalmus syndrome (microphthalmic animals). Four male twentyone day old animals were examined. Following perfusion with formaldehyde (diluted 1:4 with H2O) Nissl stained frontal sections were used for classification of neurons. we differentiate between projection neurons (PN) (geniculo-cortical relay neurons) and interneurons (IN)

Thalamic nuclei appear to play an important role in memory functioning. Severe anterograde amnesia has been reported with lesions of the dorsomedial and anterior nuclei and their connections. We report the case of a woman with discrete thalamic lesions involving the anterior and dorsomedial nuclei who had a persistent anterograde amnesia. Contrary to a previous case report describing impairment only with explicit memory, this patient had impairment with both implicit and explicit memory. This case is discussed in terms of the relevant literature associating the diencephalon, amnesia, and impli…

Cervical dystonia (CD) is a movement disorder characterized by a stereotyped pattern of involuntary turning or tilting of the head, often combined with jerky or tremulous movements. Hypotheses for the origin of CD have traditionally focused on the basal ganglia, but the contemporary discussion has considered the potential role of altered cerebellar function. As basal ganglia and the cerebellum largely project to the different thalamic nuclei, alterations in pallidal versus cerebellar output could be reflected in the activity of these thalamic regions. In this study, we analyzed a unique histor…

Dementia with Lewy bodies (DLB) is characterized by progressive dementia with two of three core symptoms; Parkinsonism, visual hallucinations or disturbances of consciousness/fluctuating attention. Dementia in Parkinson's disease (PDD) has similar neuropsychiatric characteristics. Reduced nigrothalamic dopamine and altered thalamic D2 receptors may mediate some of the non-motor symptoms of DLB and PDD. The study aims were to ascertain whether thalamic D2 density was altered in Parkinson's disease (PD), PDD and DLB, and whether D2 density was related to core symptoms. Thalamic D2 receptor bindi

The thalamic projections to somatic sensory-motor (SSM) cortex and adjacent cortical areas of the Virginia opossum were studied using anterograde and retrograde axoplasmic transport techniques. Large injections of horseradish peroxidase and/or tritiated amino acids were made in the parietal cortex to identify all of the thalamic nuclei that are interconnected with this large cortical area. Very restricted injections were then made in physiologically identified subdivisions of SSM cortex, in the remaining posterior portion of parietal cortex, and in the anteriorly adjacent postorbital cortex. T…

The thalamus, traditionally viewed as a sensory relay station, is now recognized for its critical role in higher-order cognitive functions, including memory. While most research has focused on its distinct nuclei, the thalamus also contains crucial white matter tracts, such as the mammillothalamic tract (MTT), part of the Papez circuit. Despite its established role in memory, the MTT remains underappreciated in studies on the cognitive role of the thalamus, increasing the risk of misattributing memory functions to nearby thalamic nuclei. This study investigates the memory impact of thalamic st…

The comorbidity of chronic pain and attention deficit complicates treatment, as each condition intensifies the other through mechanisms that are not well understood. The locus coeruleus (LC) integrates a variety of somatosensory and emotional inputs and has been implicated in both chronic pain and attention deficit disorders. We hypothesized that the LC and its projections may contribute to the pathophysiology of comorbid chronic pain and attention deficit. We found that in male mice with chronic sciatic nerve constriction injury (CCI), LC neurons were easily activated by mechanical and therma

In this study, thalamic connections of the caudal part of the posterior parietal cortex (PPCc) are described and compared to connections of the rostral part of PPC (PPCr) in strepsirrhine galagos. PPC of galagos is divided into two parts, PPCr and PPCc, based on the responsiveness to electrical stimulation. Stimulation of PPC with long trains of electrical pulses evokes different types of ethologically relevant movements from different subregions ("domains") of PPCr, while it fails to evoke any movements from PPCc. Anatomical tracers were placed in both dorsal and ventral divisions of PPCc to

The distribution of thalamic cells projecting to the head of the caudate and their interrelations with thalamo-cortical cells were studied in the cat with different combinations of fluorescent tracers. Injections in the head of the caudate were combined with the injections in the pericruciate, proreal, suprasylvian, anterior cingulate, occipital and ectosylvian cortices. The following results were obtained: (i) Injections in the head of the caudate resulted in retrograde labeling of thalamic cells medially and laterally to the anteromedial (AM) nucleus, and in the medioventral part of the vent
