[Unsuccessful operative treatment of acute thoracic and thoracolumbar injuries with neurologic deficit].

OBJECTIVE: To analyze the main factors for unsuccessful operative treatment of acute thoracic and thoracolumbar injuries with neurologic deficit. METHODS: 82 patients with acute thoracic and thoracolumbar injuries with neurologic deficit who had had unsuccessful operations in other hospitals were reviewed. Clinical materials and radiographs including plane and functional films and CT scans were analyzed. RESULTS: In the 82 patients 30 had unsuccessful reduction or spinal deformity, and 31 canal encroachment. In 12 pedicle screws located out of the pedicle, 2 penetrated the canal and 3 presse

[Use of internal fixator in injuries of the thoracic and lumbar spine].

Between 1984 and 1989 51 patients with spinal traumata received surgical treatment by means of Fixateur interne. The results of patients' postsurgical check-up were as follows: Cases with incomplete neurological dysfunctions (levels Frankel B-D)-especially lumbar-showed improvements of neurological deficiencies up to three levels Frankel after emergency decompression and stabilization. The majority of cases with complete paraplegia-especially thoracic-did not subside. This holds true even for the results of a second check-up after about three years. A correlation between the degree of stenosis

Halo-thoracic brace immobilization in 188 patients with acute cervical spine injuries.

The authors report 188 patients with acute cervical spine injury with fracture who underwent Halo-thoracic brace immobilization. The majority of the fractures were considered unstable. Early neurological assessment revealed 24 patients without neurological deficit. There were 164 patients with associated cervical cord injury; 84 patients with incomplete, and 80 patients with complete tetraplegia. Management consisted of skull traction and application of the Halo-thoracic brace about 1.3 weeks after admission. The average radiological union time was 11.5 weeks following a mean of 10.2 weeks of

The surgical management of acute and chronic injuries of the thoracic aorta.

The clinical diagnosis of traumatic descending thoracic aortic aneurysm is not obvious but is usually suggested on a roentgenogram of the chest. The diagnosis, therefore, is based upon a high index of suspicion and confirmed by an angiogram. Femoral-femoral bypass is a safe approach, assuring adequate distal perfusion and, thereby, avoiding problems of ischemia. By assuring adequate distal perfusion and preserving all possible intercostal arteries, spinal cord injury was avoided in this series.

Chronic Blunt Traumatic Thoracic Aortic Injuries: Report of three cases from Oman.

Blunt thoracic aortic injuries are potentially lethal. Those who survive may form an organised haematoma in the periadventitial space resulting in a pseudoaneurysm, which may be identified incidentally decades later. While the role of thoracic endovascular aortic repair (TEVAR) in acute settings has been established, its role in chronic cases is yet to be defined. We report three cases that were diagnosed incidentally six, nine and 18 years after the injury. Two were managed by TEVAR while the third declined intervention and is on annual follow-up. Patients with asymptomatic and stable pseudoa

[Organ injuries due to thoracic trauma : Diagnostics, clinical importance and treatment principles].

Thoracic trauma can be a life-threatening condition due to the involvement of vital organs, such as the heart, lungs, tracheobronchial tree and the great vessels. A coordinated interdisciplinary management is vital for the survival of the injured person. Modern diagnostic procedures provide an essential basis for the surgical treatment of patients. Surgical treatment principles include insertion of chest drainage, emergency thoracotomy, complex bronchoplastic and vascular reconstructive techniques and cardiac surgical maneuvers. For this reason highly complex surgical procedures are available,

Epidemiology of concomitant injuries in traumatic thoracic aortic rupture: a meta-analysis.

Traumatic thoracic aortic rupture is a highly lethal injury. For those who arrive alive at the hospital, it is of utmost importance to quickly evaluate concomitant injuries and prioritize therapeutic interventions. We aimed to review the frequency of concomitant injuries in patients with thoracic aortic rupture, according to anatomic location and type of injury. A systematic literature search of six medical databases led to the identification of 90 publications; 27 categories of thoracic aortic rupture concomitant injuries were thereafter created. The respective pooled proportions and 95% conf

Trend of management of traumatic thoracic aortic injuries in a single center.

The purpose of this study was to review the shift in the trend of management and mid-term outcomes of all patients who sustain thoracic aortic injury. A Retrospective analysis was performed of all patients sustaining blunt thoracic aortic trauma admitted to our unit. Forty-seven patients were presented with injury to the thoracic aorta following blunt chest injury. Ten patients underwent open surgical repair of their thoracic aortic injury. The mean age ± SD (range) was 29.4 ± 7.9 years (18-41) with a mean Injury Severity Score (ISS) of 41 ± 14.7 (25-75). Fifteen patients underwent thoracic en

Single-center experience with endovascular treatment of acute blunt thoracic aortic injuries.

AIM: Endovascular repair of the thoracic aorta (TEVAR) has been recently considered an appealing alternative to open treatment of traumatic aortic injuries. However, the use of this technique in emergency is often limited by hemodynamic instability, severe associated lesions and unavailability of adequate materials. Dedicated stent-grafts are not currently available. We report our results in treating blunt traumas of the thoracic aorta using three different commercially available stent-grafts. METHODS Between 2003 and 2010, 28 patients (22 males, mean age 38.9±12.1 years) underwent TEVAR for a

Thoracic injuries in professional rugby players: mechanisms of injury and imaging characteristics.

Professional rugby players are prone to traumatic thoracic injuries due to the use of minimal protective gear to cover the torso. In the 2007 Rugby World Cup, thoracic injuries occurred at a rate of 8.3 cases/1000 player-hours. CT and MRI play an important role in the diagnosis of these injuries. Vital internal organs, such as the heart, lungs, trachea, liver and large blood vessels lie within close proximity to the bony structures and what seems to be a simple rib fracture or clavicular dislocation can have potentially life-threatening complications that are not detected by conventional radio…

Endovascular repair in traumatic thoracic aortic injuries: comparison with open surgical repair.

PURPOSE: Thoracic endovascular aortic repair (TEVAR) has emerged as an alternative to open surgical repair (OSR) of traumatic thoracic aortic injury (TTAI). Herein immediate and midterm outcomes of TEVAR are compared with those of OSR. MATERIALS AND METHODS: Health records were used to identify patients with TTAI presenting between April 1995 and September 2006. Preoperative patient characteristics, intraoperative variables, procedural costs, and outcomes were recorded. RESULTS: A total of 103 patients were identified. Twenty-two died before treatment, 19 were treated conservatively, 36 rece

Vascular injuries of the neck and thoracic inlet: helical CT-angiographic correlation.

Imaging evaluation of patients suspected to have arterial injuries of the neck has traditionally been performed by using conventional angiography as an alternative to surgical exploration. However, there has been recent interest in and growing experience with use of noninvasive imaging techniques for the assessment of vascular injuries. Contrast material-enhanced helical computed tomographic (CT) angiography is increasingly being used to evaluate trauma patients in stable condition who are at risk for vascular injuries. It allows characterization of traumatic vascular lesions in the neck such

[Minimally invasive ventral spondylodesis in injuries to the thoracic and lumbar spine].

Thirty-eight patients with 40 fractures of the thoracic spine and the thoracolumbar junction were treated by a minimally invasive procedure, which includes partial corporectomy, the interposition of a tricortical bone graft and anterior stabilization by plate spondylodesis under thoracoscopic control. For 36 patients the operation was successfully performed in a complete thoracoscopic way; in 2 patients conversion to an open technique was necessary. Two postoperative complications such as a reversible lesion of the thoracodorsalis nerve and a transient irritation of nerve root L1 on the approa

[Significance and mechanism of thoracic and lumbar spine injuries in traffic accidents].

An analysis of 96 persons who sustained injuries to dorsal and lumbar vertebrae during traffic accidents, established that spinal injuries (sustained by 3.8% of the 96 persons) are quite rare. Pedestrians, the elderly, and polytraumatized persons with extensive head injuries as accompanying trauma following particularly serious accidents are at especially high risk. The most frequent spinal injuries are compression fractures, which especially often give rise to the injury pattern found in the spine of motorcycle riders, pedestrians, and car passengers not wearing seat belts. In these cases a s
