PURPOSE OF THE STUDY: Simultaneous ipsilateral femoral and tibial fractures cause a floating knee. Treatment of such patients is complicated by fat-embolism syndrome (12 per cent), local soft tissue damage, ipsilateral knee ligament tear (5 to 39 per cent) and delayed shortening or torsional deformity. The residual radioanatomic changes influencing the functional, subjective, and clinical outcome of 131 tibial condyle fractures were studied. Clinical function was found to deteriorate rapidly with increasing values of residual medial tilt of the tibial plateau, whereas lateral tilt of the plateau was well tolerated up to 5 degrees. Osteonecrosis and osteopenia are widely recognized skeletal complications of therapy in recipients of renal transplants. These conditions are often complicated by fractures, usually in the femora, pelvis or vertebrae. The authors describe two sedentary adult allograft recipients who presented with leg pain and in whom bone scintigraphy, serial radiographs, and clinical progress were suggestive of insufficiency fractures of the tibiae. Although seldom reported, insufficiency fractures should be considered in the differential diagnosis of renal allograft recipients with leg pain.

From 1975 to 1984, 134 consecutive lateral tibial plateau fractures were treated as part of this prospective study. Eighty-six fractures (64%) were followed until union, with an average follow-up period of 22.7 months. Thirty-eight segmental tibial shaft fractures were treated with closed Grosse-Kempf interlocking nails and retrospectively followed up for > or = 1 year (average 32 months). There was a 97% union rate and a 4.5 +/- 1.6 month union period. Knee as well as ankle range of motion was satisfactory. Significant complications included one aseptic nonunion (3%), which healed after the static locked nail was dynamized. We recommend that whenever possible, closed interlocking nailing should be used to treat closed or mild open (wound size < 1 cm) segmental tibial shaft fractures.

We analysed the results of 93 tibial shaft fractures treated with the Grosse-Kempf locked nail. Twenty-six fractures were comminuted, 19 were open grade I to II, and 54 were located outside the middle third of the tibia. The deep infection rate was 3.2%. There were only two poor results. The use of this method is recommended and discussed.

In a nonrandomized prospective study, 111 tibial shaft fractures (55 closed, 56 open) were treated with flexible intramedullary nails. Open injuries were nailed immediately following irrigation and debridement in the vast majority of cases. Ninety-four percent of the fractures united in an average time of 18.8 weeks. In extensive type III open tibial fractures, limb salvage is considered worthwhile when there is good distal circulation and nerve function. One hundred ten tibial condylar fractures incurred during the period from 1972 to 1978 were reviewed retrospectively. Fifty-two knees were reviewed more than one year after initial injury and were subjected to critical evaluation using a point rating system. Overall, the results were acceptable in 84% of patients. The use of a cast of traction generally produced very good results. Case bracing was used in 31% of the series. Open reduction was reserved for 17 severe and challenging fractures; six of the 17 had unsatisfactory functional results.

We report the case of a teenage male who had a slip and fall from a motorcycle and sustained injury to his left knee. A clinicoradiological evaluation revealed a closed displaced osteochondral fragment of the posterolateral tibial plateau.
