Family and carer smoking control programmes for reducing children's exposure to environmental tobacco smoke.

BACKGROUND: Children's exposure to other people's tobacco smoke (environmental tobacco smoke, or ETS) is associated with a range of adverse health outcomes for children. Parental smoking is a common source of children's exposure to ETS. Older children in child care or educational settings are also at risk of exposure to ETS. Preventing exposure to ETS during infancy and childhood has significant potential to improve children's health worldwide. OBJECTIVES: To determine the effectiveness of interventions designed to reduce exposure of children to environmental tobacco smoke, or ETS. SEARCH ME

Correlation between tobacco control policies and preterm births and low birth weight in Europe.

OBJECTIVE: To assess the correlation between tobacco control policies- particularly smoking bans in work and public places-and the prevalence of preterm births and low birth weight in the European countries. METHODS: This is an ecological study and the unit of analysis set at the country level. Tobacco control data in Europe were obtained for the years 2010 and 2013 as measured by the Tobacco Control Scale (TCS), which reflects the level of implementation of tobacco control policies. Prevalence data for preterm births and low birth weight were obtained from two sources: the European Perinatal…

Can tobacco control endgame analysis learn anything from the US experience with illegal drugs?

The goals of tobacco control endgame strategies are specified in terms of the desired levels of tobacco use and/or tobacco related health consequences. Yet the strategies being considered may have other consequences beyond tobacco use prevalence, forms and related harms. Most of the proposed strategies threaten to create large black markets with potential attendant harms: corruption, high illegal earnings, violence and/or organised crime. Western societies of course have considerable experience with these problems in the context of prohibition of drugs such as cannabis, cocaine, heroin and met…

WHO framework convention on tobacco control and its implementation in South-East Asia region.

The birth of the WHO Framework Convention on Tobacco Control (WHO FCTC) took place in response to the global tobacco epidemic and it became the most important global tobacco control instrument. Duly recognizing tobacco use as an important public health problem and in the wake of rising prevalence of and mortality related to tobacco use, almost all Member States of the South-East Asia Region signed and ratified the WHO FCTC. Following the ratification, Member countries have enacted comprehensive national tobacco control laws and regulations. Most countries have covered some important provisions…

Discriminative stimuli that control instrumental tobacco-seeking by human smokers also command selective attention.

RATIONALE: Incentive salience theory states that acquired bias in selective attention for stimuli associated with tobacco-smoke reinforcement controls the selective performance of tobacco-seeking and tobacco-taking behaviour. OBJECTIVES: To support this theory, we assessed whether a stimulus that had acquired control of a tobacco-seeking response in a discrimination procedure would command the focus of visual attention in a subsequent test phase. METHODS: Smokers received discrimination training in which an instrumental key-press response was followed by tobacco-smoke reinforcement when one

Combined effect of tobacco and alcohol on laryngeal cancer risk: a case-control study.

OBJECTIVE: To provide information on the effects of alcohol and tobacco on laryngeal cancer and its subsites. METHODS: This was a case-control study conducted between 1992 and 2000 in northern Italy and Switzerland. A total of 527 cases of incident squamous-cell carcinoma of the larynx and 1297 hospital controls frequency-matched with cases on age, sex, and area of residence were included. Odds ratios (ORs) and corresponding 95% confidence intervals (CIs) were estimated using multiple logistic regression. RESULTS: In comparison with never smokers, ORs were 19.8 for current smokers and 7.0 fo

Association between exposure to tobacco control measures and current nicotine use among adolescents in Saudi Arabia: Evidence from the 2022 global youth tobacco survey.

OBJECTIVES: The World Health Organization's six tobacco control strategies (Monitor, Protect, Offer help, Warn, Enforce, and Raise; MPOWER) aim to reduce tobacco use, yet little is known about their impact on individual-level adolescent nicotine use in Middle Eastern countries. This study examined whether exposure to MPOWER policies is associated with current nicotine use among adolescents in Saudi Arabia. METHODS: This cross-sectional analysis used nationally representative data from the 2022 Global Youth Tobacco Survey conducted in schools across Saudi Arabia. The analytic sample included 5…

Assessing and minimizing risk of bias in randomized controlled trials of tobacco cessation interventions: Guidance from the Cochrane Tobacco Addiction Group.

The Cochrane Tobacco Addiction Group has created risk of bias tools, which are topic-agnostic. In 2012 the Cochrane Tobacco Addiction Group created guidance specific to considerations for reviews of randomized controlled trials of tobacco cessation interventions, building on existing Cochrane tools. The guidance covers issues relating to selection bias, performance bias, detection bias, attrition bias and selective reporting. In this paper, we set out to make this guidance publicly available, so that others can use and cite it. We provide advice for using this tool to appraise trials criticall

Evolution of tobacco control in India: a narrative review of the legislative and regulatory approach.

Tobacco is the only consumer product that kills half its users yearly. The challenges posed by tobacco control are limitless especially in a country like India where in addition to smoked forms, other smokeless forms of tobacco are also highly prevalent. Apart from being a health hazard tobacco is also a great environmental hazard. Policies for controlling tobacco use also include policy to prevent people from second hand smoke, which is aimed at improvement of air quality. According to the National Non-Communicable Disease Monitoring Survey, 2017-18, daily tobacco use was 32.8% in adults (18-…

Neuromelanin-MRI and tobacco smoking in first episode psychosis patients and controls.

Tobacco smoking is highly prevalent among patients with psychosis and is associated with worse clinical outcomes. Dopamine functioning is implicated in both psychosis and tobacco use. Neuromelanin-sensitive magnetic resonance imaging (NM-MRI) of the substantia nigra is an emerging imaging technique that provides an indirect marker of dopamine function. Previous studies have shown higher NM-MRI signal in patients with psychosis. However, to date, no NM-MRI studies have examined the effects of tobacco smoking. This pre-registered study investigates the association between tobacco smoking and NM-

Coronary Artery Vasospasm Requiring Cardiac Autotransplantation Yet Controlled With Tobacco.

Coronary artery vasospasm is typically managed through avoidance of triggers and with symptomatic treatments with calcium channel blockers and long-acting nitrates. Here, we report a rare case of medically refractory coronary artery vasospasm associated with genetic predispositions that initially required cardiac autotransplantation followed paradoxically by nicotine for long-term symptomatic control. (Level of Difficulty: Intermediate.).

Population-based tobacco treatment: study design of a randomized controlled trial.

BACKGROUND: Most smokers do not receive comprehensive, evidence-based treatment for tobacco use that includes intensive behavioral counseling along with pharmacotherapy. Further, the use of proven, tobacco treatments is lower among minorities than among Whites. The primary objectives of this study are to: (1) Assess the effect of a proactive care intervention (PRO) on population-level smoking abstinence rates (i.e., abstinence among all smokers including those who use and do not utilize treatment) and on utilization of tobacco treatment compared to reactive/usual care (UC) among a diverse popu

A longitudinal study of transitions between smoking and smokeless tobacco use from the ITC Bangladesh Surveys: implications for tobacco control in the Southeast Asia region.

BACKGROUND: In Southeast Asia, tobacco use is a major public health threat. Tobacco users in this region may switch between or concurrently use smoked tobacco and smokeless tobacco (SLT), which makes effective tobacco control challenging. This study tracks transitions of use among different product users (cigarettes, bidis, and SLT) in Bangladesh, one of the largest consumers of tobacco in the region, and examines factors related to transitions and cessation. METHODS: Four waves (2009-2015) of the International Tobacco Control (ITC) Bangladesh Survey with a cohort sample of 3245 tobacco users…

A Scoping Review of Theory-Based Tobacco Control Efforts for Sexual and Gender Minority Populations.

INTRODUCTION: Understanding which theories have been effectively applied in tobacco control efforts for sexual and gender minority (SGM) individuals will help identify constructs to incorporate into future tobacco control efforts aimed at reducing SGM tobacco disparities. This scoping review summarizes the use of theory in SGM-focused tobacco control efforts. METHODS: We searched four databases using SGM- and tobacco-focused terms in November 2023. Two trained coders independently screened each title and abstract, reviewed the full text, and extracted data about theories used and which tobacc…

Tobacco control policies predict quit attempts, but household smoking predicts cessation success across 29 countries.

BACKGROUND: Tobacco use remains a global public health challenge, leading to over 8 million annual deaths and significant economic burden. Effective tobacco control and cessation interventions are essential to mitigate these impacts. METHODS: Using data from the Global Adult Tobacco Surveys (between 2011 and 2021) and WHO reports from 29 countries, this study analysed determinants of quitting behaviour among n=51 196 individuals. Random Forest classification models were employed to identify key predictors for two outcomes: quit attempts and successful cessation. The model incorporated individ…
