Evolution of optic nerve and retina alterations in a child with indirect traumatic neuropathy as assessed by optical coherence tomography.

Herein, we describe the case of a 4-year-old child with indirect traumatic optic neuropathy and serial changes of the optic nerve head and retinal nerve fiber layer (RNFL) documented using optical coherence tomography (OCT). Visual acuity improved despite progressive RNFL thinning and optic disc pallor. We concluded that OCT may be useful for monitoring axonal loss but may not predict the final visual outcome.

The relation between the use of psychoactive substances and the severity of the injury in a group of crash-involved drivers admitted to a regional trauma center.

OBJECTIVE: There is much evidence that driving under the influence of alcohol and/or drugs of abuse is related to an increased accident risk. A remaining question is whether the use of psychoactive substances is also related to clinically more severe accidents. The aim of this study is to explore the relationship between the use of psychoactive substances and the injury severity in a group of crash-involved drivers. METHODS: The study group included all injured car drivers, admitted to the regional trauma center, in the period from May 2000 until August 2001. The outcome of interest was the s

Manual enucleation of both eyes during assault.

Manual enucleation of the eyeball from the orbit during physical assault is rare. Management includes removal of such sightless eyeballs when the optic nerve and extraocular muscles are avulsed. However, other authors recommend replacement of these eyeballs even though visual prognosis is nil and phthisis bulbi is expected. A patient who had bilateral enucleation after assault is described.

Mortality prognostic factors in chest injury.

1,026 multiple trauma patients (P) were compared to P with chest injuries (PCT) (407). Severity indices were related to type of thoracic injury and mortality. The Injury Severity Score (ISS), Glasgow Coma Scale (GCS), Trauma Score (TS), CHOP, and the Respiratory Index (RI) were used. The mortality rate of P was 27.1% but increased to 32.9% for PCT (p less than 0.05). We noted that mortality rate was highly dependent on major chest trauma: 68.6% for flail chest (FC), 56% for lung contusion (LC), 42.3% for hemothorax (HA), and 38.1% for pneumothorax (PN). ISS and RI scores for PCT survivors were

The effect of thoracic trauma scoring system in thoracic trauma patients with rib fracture.

OBJECTIVE: Rib fractures are common in thoracic trauma patients. There are various factors, including flail chest, pulmonary contusion, and accompanying conditions, affecting morbidity and mortality. The study aimed to identify high-risk patients for morbidity and mortality with a scoring system that the authors created. METHODS: Cases over the age of 18 admitted due to trauma and diagnosed with rib fractures between 1 January 2019 and 1 March 2023, were included. Trauma scores were determined by applying the new trauma scoring system. Trauma scores and other variables regarding morbidity and

Development of a novel tool to predict pulmonary complications in trauma patients with and without chest injury.

BACKGROUND: Existing chest trauma scoring tools are often only applicable to patients with chest trauma. We sought to develop a novel Trauma Induced Pulmonary Event (TIPE) score to predict the risk of developing pulmonary complications in all adult trauma patients. METHODS: Multiple logistic regression models were created using the 2017 Trauma Quality Improvement Program (TQIP) to identify independent predictors of pulmonary complications in trauma patients. The weighted average and relative impact of each independent predictor was used to derive a TIPE score. We then validated the score usin

[The Sensory Modality Assessment and Rehabilitation Technique (SMART): a behavioral assessment scale for disorders of consciousness].

INTRODUCTION: Difficulties in detecting bedside signs of consciousness in non-communicative patients still lead to a high rate of misdiagnosis illustrating the need to employ standardized behavioral assessment scales. STATE OF ART: The Sensory Modality Assessment and Rehabilitation Technique (SMART) is a behavioral assessment scale of consciousness that assesses responses to multimodal sensory stimulation in disorders of consciousness. These stimulations can also be considered to have therapeutic value. PERSPECTIVES: We here review the different components and use of the SMART assessment and

Prognostic factors for persistent symptoms in adults with mild traumatic brain injury: protocol for an overview of systematic reviews.

BACKGROUND: Mild traumatic brain injury (mTBI) is an increasing public health problem that can lead to persistent symptoms that have several functional consequences. Understanding the prognosis of a condition is an important component of clinical decision-making and can help guide the prevention of long-term disabilities of patients with mTBI. Several studies and systematic reviews have been conducted in order to understand prognosis of chronic symptoms following mTBI. We aim to synthesize evidence from systematic reviews on factors that affect the risk of persistent symptoms in mTBI-affected

Establishing an Evaluation System and Limb-Salvage Protocol for Mangled Lower Extremities in China.

Road traffic accident-related severely injured extremities account for the majority of disabilities in young people in China. Limb-salvage concepts and techniques vary greatly from physician to physician and from district to district in China. Current severity-scoring systems for lower-extremity injuries lack sensitivity and cannot be used as the sole criterion by which amputation decisions are made. China lacks a national database of mangled lower extremities, which is a priority for both limb-salvage protocols and scoring system development.

Evaluation of NuStatr, a Novel Nonimpregnated Hemostatic Dressing, Compared With Combat Gauze in Severe Traumatic Porcine Hemorrhage Model.

BACKGROUND: Uncontrolled hemorrhage remains one of the most challenging problems facing emergency medical professionals and a leading cause of traumatic death in both battlefield and civilian environments. Survival is determined by the ability to rapidly control hemorrhage. Several commercially available topical adjunct agents have been shown to be effective in controlling hemorrhage, and one, Combat Gauze (CG), is used regularly on the battlefield and for civilian applications. However, recent literature reviews have concluded that no ideal topical agent exists for all injuries and scenarios.

Exploring the impact of access designs on crash injury severity on multilane highways.

OBJECTIVE: Access design is a critical factor that influences the safety and mobility of urban/suburban multilane highways due to the interactions between access movements and through traffic. An effective way for improving the safety and mobility of multilane highways is to control access maneuvers by implementing appropriate access designs. Understanding the impact of access designs on crash injury severity is beneficial for implementing effective countermeasures to mitigate crash injury at access points. Thus, the objectives of this article are to investigate the impact of access designs on

Intra-abdominal solid organ injuries: an enhanced management algorithm.

The organ injury scale grading system proposed by the American Association for the Surgery of Trauma provides guidelines for operative versus nonoperative management in solid organ injuries; however, major shortcomings of the American Association for the Surgery of Trauma injury scale may become apparent with low-grade injuries, in which conservative management may fail. Nonoperative management of common intra-abdominal solid organ injuries relies increasingly on computed tomographic findings and other clinical factors, including patient age, presence of concurrent injuries, and serial clinica

Traumatic air in spinal canal (pneumorrhachis).

BACKGROUND: Pneumorrhachis (PR) means the presence of air in the spinal canal; it is an exceptional, but important radiographic finding, which may be associated with different aetiologies and pathways of air entry into the spinal canal. CASE REPORT: An 18-year-old male was admitted to hospital after a road traffic accident. He was conscious, with several abrasions and subcutaneous haematoma and emphysema on the left side, but no pneumothorax. Ultrasound of the abdomen showed a grade 2 splenic rupture with minimal bleeding. CT revealed no fractures but the presence of air in the spinal canal.

Paramedic rapid sequence intubation for severe traumatic brain injury: perspectives from an expert panel.

Although early intubation has become standard practice in the prehospital management of severe traumatic brain injury (TBI), many patients cannot be intubated without neuromuscular blockade. Several emergency medical services (EMS) systems have implemented paramedic rapid sequence intubation (RSI) protocols, with published reports documenting apparently conflicting outcomes effects. In response, the Brain Trauma Foundation assembled a panel of experts to interpret the existing literature regarding paramedic RSI for severe TBI and offer guidance for EMS systems considering adding this skill to
