Drug-induced hepatitis and the risk factors for liver injury in pulmonary tuberculosis patients.

INTRODUCTION: Short-course chemotherapy containing rifampicin and isoniazid in combination has proved to be highly effective in the treatment of tuberculosis, but one of its adverse effects is hepatotoxicity. Various risk factors have been found to be associated with drug-induced liver injury (DILI) in general population. The study aimed to determine the prevalence of drug-induced hepatitis and the risk factors associated with the DILI among the patients of pulmonary tuberculosis in Indian patients. SETTING AND DESIGN: Prospective nested case control study. MATERIALS AND METHODS: Out of the

Hepatitis C virus co-infection increases the risk of anti-tuberculosis drug-induced hepatotoxicity among patients with pulmonary tuberculosis.

BACKGROUND: The country of Georgia has a high prevalence of tuberculosis (TB) and hepatitis C virus (HCV) infection. PURPOSE: To determine whether HCV co-infection increases the risk of incident drug-induced hepatitis among patients on first-line anti-TB drug therapy. METHODS: Prospective cohort study; HCV serology was obtained on all study subjects at the time of TB diagnosis; hepatic enzyme tests (serum alanine aminotransferase [ALT] activity) were obtained at baseline and monthly during treatment. RESULTS: Among 326 study patients with culture-confirmed TB, 68 (21%) were HCV co-infected,

The role of chronic hepatitis in isoniazid hepatotoxicity during treatment for latent tuberculosis infection.

BACKGROUND: To examine chronic viral hepatitis (CVH) as a risk factor for hepatotoxicity during isoniazid (INH) treatment for latent tuberculosis infection (LTBI). METHODS: A search of MEDLINE (1966-May 2008) was conducted using the terms 'tuberculosis', 'antitubercular', 'therapeutics', 'treatment', 'prevention', 'prophylaxis', 'hepatitis', 'toxic hepatitis', 'hepatotoxic', 'liver' and 'injury'. Peer-reviewed, English-language articles describing the relationship between a history of CVH and occurrence of hepatotoxicity during LTBI treatment were selected. We limited CVH diagnoses to reports

Prevalence and interaction of hepatitis B and latent tuberculosis in Vietnamese immigrants to the United States.

OBJECTIVES: Southeast Asian immigrants, with a high prevalence of both hepatitis B and latent tuberculosis, constitute a large proportion of immigrants to the United States. Isoniazid hepatotoxicity may be associated with hepatitis B. This study was conducted to document the prevalence and interaction of hepatitis B, latent tuberculosis, and isoniazid toxicity. METHODS: Hepatitis B surface antigen (HBsAg) and tuberculin skin testing was done on 743 Vietnamese immigrants to the Midwest between January, 1991 and December, 1999. HBsAg positive cases were tested for hepatitis B e antigen (HBeAg).

Late vertebral and pulmonary alveolar echinococcosis after hepatic surgery mimicking spinal and disseminated tuberculosis: a case report.

BACKGROUND: Alveolar echinococcosis (AE) is a rare but potentially fatal zoonotic disease that primarily affects the liver, while extrahepatic involvement usually indicates advanced disease and poor prognosis. Vertebral and pulmonary AE may closely mimic tuberculosis (TB), particularly in TB-endemic regions, leading to substantial diagnostic challenges. We report a case of late vertebral and pulmonary AE after previous hepatic hydatid surgery that was initially misdiagnosed as spinal and disseminated pulmonary TB. CASE PRESENTATION: A 28-year-old Tibetan woman from a nomadic family presented

Towards precision dosing of contezolid: moderate hepatic impairment increases plasma concentration of contezolid in anti-tuberculosis treatment.

BACKGROUND: Contezolid is a novel oxazolidinone antibiotic with potent antimicrobial activity against Mycobacterium tuberculosis. To date, the plasma concentration profile of contezolid and its influencing factors remain poorly characterized. METHODS: This prospective observational study included patients who were diagnosed as tuberculosis and received contezolid (800 mg every 12 h) between July 2024 and October 2025. All patients underwent therapeutic drug monitoring to obtain steady-state trough plasma concentrations (Cmin) of contezolid. Demographic characteristics and laboratory examinatio

Diagnosis of isolated hepatic tuberculosis in an immunocompetent individual: a case report and review of misdiagnosed cases.

INTRODUCTION: Hepatic tuberculosis is a rare disease entity even in endemic areas of Mycobacterium tuberculosis. It is usually accompanied by pulmonary tuberculosis or enteric tuberculosis. Due to the non-specific clinical symptoms and imaging features may lead to misdiagnosis and even unnecessary surgery. CASE PRESENTATION: A 44-year-old woman presented with complaints of abdominal pain for the past 2 months. The patient was diagnosed with a solitary hepatic tuberculosis abscess that was identified by abdominal CT and liver biopsy. The patient was started on anti-tuberculous therapy (isoniaz

Adrenal tuberculosis mimicking a malignancy by direct hepatic invasion: emphasis on adrenohepatic fusion as the potential route.

A 64-year-old female with primary adrenal insufficiency presented with a right adrenal mass showing quantitative nonadenoma features on dedicated adrenal computed tomography (CT). CT showed direct invasion of the mass to the adjacent hepatic parenchyma, and high uptake was noted on 18F-fluorodeoxyglucose positron emission tomography/CT. Laparoscopy revealed gross invasion of the adrenal lesion into the liver, which led to the en bloc resection including the involved liver. Polymerase chain reaction analysis of the surgical specimen revealed adrenal tuberculosis.

Hepatic and splenic tuberculosis following infliximab therapy for refractory ulcerative colitis: a case report and literature review.

INTRODUCTION: Tumor necrosis factor-α (TNF-α) inhibitors are first-line agents for steroid-refractory/resistant ulcerative colitis (UC), but their immunosuppressive effect elevates the risk of opportunistic infections, particularly tuberculosis (TB). Extrapulmonary TB such as hepatic and splenic TB is rare but prone to misdiagnosis. A 55-year-old male was diagnosed with UC in 2003, relieved by sulfasalazine and mesalazine for recurrence in 2021. In 2024, post-perianal abscess surgery, UC relapsed with positive cytomegalovirus (CMV), treated with glucocorticoids and ganciclovir. The patient sub

Isolated Hepatic Tuberculosis Discovered Incidentally During Laparotomy for Gastric Perforation in an Immunocompetent Patient: A Case Report.

Isolated hepatic tuberculosis is a rare clinical entity, particularly in immunocompetent patients. It is characterized by frequently silent symptoms and nonspecific radiological abnormalities, which complicates its diagnosis in the absence of specific clinical guidance. We present the case of a 39-year-old man with no significant medical history who was admitted to the emergency department with acute peritonitis resulting from gastric perforation. An exploratory laparotomy confirmed the prepyloric perforation and incidentally revealed the presence of two subcapsular hepatic nodules located in

Hepatitis C and risk of tuberculosis amongst United States veterans: a national cohort study.

BACKGROUND: Tuberculosis (TB) and hepatitis C virus (HCV) infections share many risk factors and settings. We sought to determine and compare the incidence of TB amongst HCV-positive and negative individuals in a large national cohort of United States veterans, controlling for multiple confounders. METHODS: We conducted a retrospective-cohort study using data from The Electronically Retrieved Cohort of HCV-Infected Veterans (ERCHIVES) between 2001 and 2024 among United States veterans in care in the Veterans Affairs (VA) healthcare system. HCV-positive veterans were matched to HCV-negative ve

Beyond Bartonellosis: A Case Report of Hepatic Tuberculosis as a Final Diagnosis in a Child With Down Syndrome.

Hepatic tuberculosis is a rare form of extrapulmonary manifestation. This case report depicts a 6-year-old boy with Down syndrome who presented with prolonged fever, weight loss, and hepatomegaly. The investigation revealed liver abnormalities suggestive of a granulomatous process, with the initial diagnosis of bartonellosis and, later, tuberculosis, with molecular testing and culture from a fragment of the liver biopsy. He underwent treatment and exhibited an excellent clinical response. This case highlights the importance of including hepatic tuberculosis in the differential diagnosis of liv
