The international journal of tuberculosis and lung disease : the official journal of the International Union against Tuberculosis and Lung Disease2013eng
BACKGROUND: Clinicians in countries with high tuberculosis (TB) prevalence often treat pleural TB based on clinical grounds, as the availability and sensitivity of diagnostic tests are poor.
OBJECTIVE: To evaluate the role of artificial neural networks (ANN) as an aid for the non-invasive diagnosis of pleural TB. These tools can be used in simple computer devices (tablets) without remote internet connection.
METHODS: The clinical history and human immunodeficiency virus (HIV) status of 137 patients were prospectively entered in a database. Both non-linear ANN and the linear Fisher discrimina

To enhance the efficiency of treating tuberculous pleurisy, a complex of therapeutical measures was supplemented by the immunomodulator licopid in 51 patients. The control group included 52 patients with pleurisy who received the same treatment regimen without licopid. Comparing the outcomes of treatment has revealed that licopid reduced the time of treatment, promotes the substantial diminution of antigenemia and the production of tuberculosis antigens, improves cell immunity and natural resistance.

Tuberculosis (TB) has become one of the global burdens of disease, with increasing morbidity and mortality every year. Tuberculosis can affect not only the lungs but also the extrapulmonary organs. The prevalence of drug-resistant tuberculosis (DR-TB) is rising and has caused a higher mortality rate than drug-susceptible tuberculosis (DS-TB). This article presents a patient with a rare co-infection of pulmonary DS-TB and pleural DR-TB. Pulmonary and pleural TB (pTB) was diagnosed using the Xpert MTB/RIF assay. The patient was treated with an individualized DR-TB regimen and recovered.

OBJECTIVE: To study the clinical value of detecting carcinoembryonic antigen levels in pleural effusion (PCEA) and serum (SCEA) and their ratio (P/S) in the differential diagnosis of pleural effusions resulting from tuberculosis and lung cancer.
METHODS: This retrospectively study was conducted among 82 patients with pleural effusion caused by pulmonary tuberculous (TB; control group) and 120 patients with pleural effusion resulting from lung cancer in our hospital between April, 2016 and March, 2018. PCEA, SCEA and P/S were compared between the two groups and among the subgroups of lung canc

INTRODUCTION: Systemic lupus erythematosus (SLE), a multisystemic disease with a global prevalence of 20-150/100 000, predominantly affecting women may have a familial clustering, suggesting a strong genetic predisposition and emphasizing the role of heritable factors in disease pathogenesis. In tuberculosis-endemic regions, pleural effusions are frequently misattributed to tuberculosis, leading to delays in the correct diagnosis and appropriate treatment.
CASE PRESENTATION: A 42-year-old female with a history of continuous chest pain, aggravated on inspiration, intermittent fever with night

A case of pulmonary Mycobacterium kansasii infection with pleural effusion is very rare. We report a case of pulmonary Mycobacterium kansasii infection with pleural effusion, distinguished from pulmonary tuberculosis. A 44-year-old man presented to a clinic with a productive cough, sputum, and loss of appetite for several months. Chest X-ray and chest computed tomography (CT) showed right pleural effusion, centrilobular nodules and infiltrative shadows with cavities in the bilateral lung fields. The direct smear examination showed positive acid-fast bacilli (Gaffky 5). He was referred to our h

BACKGROUND: Fibroblast specific protein-1 (S100A4) is related with many fibrotic diseases, but its role in the pathogenesis of pleural fibrosis has not been fully elucidated. Then we aim to investigate the expression and effect of fibroblast specific protein-1 (S100A4) in pleural tuberculosis and, subsequently, pleural fibrosis.
METHODS: The expression of S100A4 in pleura was examined in 30 patients with pleural tuberculosis and 5 control (disease-free) patients by immunohistochemistry using the streptavidin-peroxidase (S-P) conjugated method.
RESULTS: The expression of S100A4 in pleura was

BACKGROUND: Autoantibodies against the p53 proteins (p53 Abs) can be detected in the serum, ascites, saliva and pleural effusions of various malignant patients. It is suggested that p53 Abs in pleural effusions might have some value for tumour diagnosis, prognosis or monitoring. The present study investigated the prevalence of p53 Abs in the pleural effusions of 90 patients with various diseases.
METHODS: Patients with suspicious pleural effusions in chest film received thoracocentesis and their pleural effusions were collected. The presence of p53 Abs in effusion was detected by immunoblotti

Extracellular vesicles (EVs) have recently emerged as a source of microbe-specific biomarkers for disease diagnosis. In the present study, we evaluated the utility of pleural fluid-derived extracellular vesicles (pEVs) as a source of Mycobacterium tuberculosis (M. tb.) antigens for pleural TB (pTB) diagnosis. EVs were isolated from pleural fluid (PF) samples and were characterized by scanning electron microscopy, and immunoblotting by targeting CD63 and LAMP2 markers. Antigen-detection ELISAs were developed for 2 M.tb.-specific antigens, MPT51 and MPT64 in pEVs (pEV-ELISA) and direct PF sample

Accelerated malignant hypertension frequently manifests as multiple organ dysfunctions. However, persistent symptoms despite appropriate antihypertensive therapy warrant investigation of concurrent pathologies, particularly in patients with risk factors for opportunistic infections. A 57-year-old woman with untreated hypertension presented in August 2024, with markedly elevated blood pressure (208/122 mmHg), systolic dysfunction (ejection fraction, 42.5%) and acute kidney injury (creatinine 4.74 mg/dl). Accelerated malignant hypertension with multiple organ damage was diagnosed based on these

BACKGROUND: Paediatric pleural tuberculosis (TB) is a paucibacillary disease, which increases the difficulty of examination. We aimed to assess the performance of pleural fluid adenosine deaminase (ADA) in the detection of paediatric pleural TB.
METHODS: PubMed, Web of Science Core Collection, Embase and Cochrane Library databases were searched up to 20 December 2021. We used the bivariate and hierarchical summary receiver operating characteristic models to compute pooled estimates for the overall diagnostic accuracy parameters of ADA for diagnosing paediatric pleural TB.
RESULTS: Eight stud

BACKGROUND: The performance of Xpert-MTB/RIF, including the newer Xpert Ultra test, for the diagnosis of pleural TB is poor (~28 - 38%). There are no data on patient characteristics that portend a positive Xpert-Ultra test in pleural fluid. These characteristics could be useful for selecting patients for Xpert-Ultra testing, thus maximising benefits of a positive test, while minimising cost.
OBJECTIVES: To determine the clinical, radiological, microbiological and biochemical characteristics associated with Xpert-Ultra positivity in patients with suspected pleural tuberculosis.
METHODS: We pe

Tuberculosis (TB) remains a major public health burden, particularly in India, and is known for its wide spectrum of clinical presentations ranging from pulmonary to extrapulmonary disease. Pleural effusion is a well-recognised complication of TB and is characteristically exudative in nature. We report a rare case of a 27-year-old immunocompetent female patient who presented with a right-sided pleural effusion that was transudative by Light's criteria. Rather than excluding TB, clinical suspicion was maintained, leading to a thorough diagnostic workup. The patient was ultimately diagnosed with

Tuberculosis is prevalent in high-burden countries, but its cutaneous form, tuberculid, is rare and often misdiagnosed. Lichen scrofulosorum, a type of tuberculid, is uncommon and typically affects children and young adults, sometimes alongside other tuberculosis symptoms. Herein, a very rare case of lichen scrofulosorum in a 20-year-old Indian male with an underlying focus of tuberculosis in the lungs and pleura is presented. Prompt treatment after detailed lab work backed by clinical assessment helped in establishing the diagnosis. He was put on antitubercular therapy, which led to a marked

BACKGROUND: Pleural effusion is a common clinical condition caused by several respiratory diseases, including tuberculosis and malignancy. However, rapid and accurate diagnoses of tuberculous pleural effusion (TPE) and malignant pleural effusion (MPE) remain challenging. Although monocytes have been confirmed as an important immune cell in tuberculosis and malignancy, little is known about the role of monocytes subpopulations in the diagnosis of pleural effusion.
METHODS: Pleural effusion samples and peripheral blood samples were collected from 40 TPE patients, 40 MPE patients, and 24 transud

Tuberculous pleurisy is an infectious disease with a poor prognosis needing early diagnosis. The use of appropriate antituberculosis drugs can improve prognosis. However, the diagnosis of tuberculous pleurisy is often challenging in older patients. Decreased activities of daily living (ADLs) may lead to difficulty in performing invasive procedures to make a definite diagnosis of pleural effusion. We report our experience with a 90-year-old female with the chief complaint of dyspnea with massive pleural effusion. We could not perform an intensive investigation for tuberculous pleurisy. Based on

INTRODUCTION: The diagnosis of pleural tuberculosis remains a clinical challenge due to the paucibacillary nature of disease. Medical thoracoscopy remains the gold standard in diagnosing tuberculous pleuritis.
OBJECTIVE: To establish the diagnostic yield of sago-seed thoracoscopic appearance of pleura in tuberculosis and its correlation with histopathology, tissue AFB culture and tissue Xpert MTB/Rif assay.
METHODS: All consecutive patients with lymphocytic exudative pleural effusion, who fulfilled inclusion criteria of the study underwent medical thoracoscopy under local anesthesia and pleu
