Abdominal pain and swelling as an initial presentation of spinal tuberculosis.

Spinal tuberculosis (Pott's disease) is one of the common extra-pulmonary presentations of tuberculosis. Spinal tuberculosis commonly presents with back pain, fever and night sweats. In this report, we present a case of spinal tuberculosis complicated by bilateral large psoas abscesses. The patient presented with bilateral flank pain and swellings rather than the classic presentation of back pain. The aim of this report is to draw the attention of physicians to this uncommon presentation of spinal tuberculosis, as an early recognition of such condition may expedite diagnosis and treatment, the

Clinical features and outcomes of spinal tuberculosis in southern Taiwan.

BACKGROUND/PURPOSE: The early diagnosis and appropriate management of spinal tuberculosis (TB) is challenging for clinicians. This study aimed to characterize the clinical features and factors affecting treatment outcomes. METHODS: A retrospective study of patients with spinal TB over a 7-year period at a medical center in southern Taiwan was conducted. Clinical features, underlying diseases, laboratory results, imaging findings, therapy, treatment duration and outcomes were analyzed. RESULTS: Forty-eight patients (24 men and 24 women) were diagnosed with spinal TB. Their mean age was 64.3 y

Routine surgery in addition to chemotherapy for treating spinal tuberculosis.

BACKGROUND: Tuberculosis is generally curable with chemotherapy, but there is controversy in the literature about the need for surgical intervention in the one to two per cent of people with tuberculosis of the spine. OBJECTIVES: To compare chemotherapy plus surgery with chemotherapy alone for treating people diagnosed with active tuberculosis of the spine. SEARCH METHODS: We searched the Cochrane Infectious Diseases Group Specialized Register (February 2010), CENTRAL (The Cochrane Library 2010,Issue 1), MEDLINE (1966 to February 2010), EMBASE (1974 to February 2010), LILACS (1982 to Februar

Results of nonsurgical treatment of thoracic spinal tuberculosis in adults.

BACKGROUND: The indications for surgery in spinal tuberculosis have been controversial, and more so recently, in the era of renewed understanding of the concept of multi-drug-resistant tuberculosis along with newer modalities of spinal instrumentation. Indications for surgery need to be redefined in this context. PURPOSE: To assess the efficacy and results of nonsurgical treatment in thoracic spinal tuberculosis in adult patients, and redefine indications for surgery. STUDY DESIGN: We present a retrospective analysis of 70 adults with thoracic spinal tuberculosis, with varying presentations,

[Spinal lesions in generalized and multiple organ forms of tuberculosis].

The paper presents the results of examination of 152 patients with generalized and multiorgan lesions, mainly involving the vertebral column. The tuberculous process is defined in 64.4% of the patients as a generalized one by virtually simultaneously involving several organs and primarily the spine and respiratory organs and in 35.6% as a multiorgan one when symptoms of tuberculous spondylitis or tuberculosis of other organs rather than the lung appear some period (3 years or more) after organ involvement. Recommendations are given for pre- and postoperative treatment. All the patients with a

[Indicators of systemic and local immunity in patients with spinal tuberculosis].

The examination of 33 patients with tuberculous spondylitis and its sequelae revealed inhibition of T-cell immunity in 40% of the examinees, antituberculous antibodies in the blood of 73% of patients. An enzyme immunoassay detected antituberculous antibodies in cerebrospinal fluid of 70% seropositive sufferers with spinal tuberculosis. Positive results of this test serve a direct indication to spinal decompression and intensive pre- and postoperative antituberculosis treatment.

Results of treatment of spinal tuberculosis by "middle-path" regime.

The efficacy of modern drugs in the treatment of tuberculosis of the spine has been evaluated by a personal follow-up for three to ten years. Operation on the vertebral lesion was done only for those patients with or without neural complications who failed to respond favourably to drug therapy and rest. Thus absolute indications for operation were present in only 6 per cent of cases without neural involvement and in 60 per cent of patients with neural deficit. Of patients who responded to drug therapy alone, only 19 per cent revealed increase of kyphosis by more than 10 degrees. The diseased a

Comparing gene expression profiles of adults with isolated spinal tuberculosis to disseminated spinal tuberculosis identified by 18FDG-PET/CT at time of diagnosis, 6- and 12-months follow-up: classifying clinical stages of spinal tuberculosis and monitoring treatment response (Spinal TB X cohort study).

BACKGROUND: Tuberculosis (TB) is one of the top ten causes of death worldwide, with approximately 10 million cases annually. Focus has been on pulmonary TB, while extrapulmonary TB (EPTB) has received little attention. Diagnosis of EPTB remains challenging due to the invasive procedures required for sample collection. Spinal TB (STB) accounts for 10% of EPTB and often leads to lifelong debilitating disease due to devastating spinal deformation and compression of neural structures. Little is known about the extent of disease, although both isolated STB and a disseminated form of STB have been d

Clinical and radiological outcomes of spinal tuberculosis: A retrospective study.

OBJECTIVES: To characterise clinical and radiological features of spinal tuberculosis (TB) at presentation, mid-treatment and treatment completion. METHODS: We conducted a retrospective study of patients diagnosed with spinal TB between 2011-2022 across three London hospitals. Demographic and clinical data were collected at presentation, mid-treatment and on treatment completion. MRI findings at each time point were classified using a standardised scoring system. Clinical symptoms were scored according to severity. RESULTS: Sixty-two cases were evaluated. Most were male (45/62, 73%) and migr

A case of spinal tuberculosis presenting with cauda equina syndrome.

Tuberculosis (TB) of the central nervous system accounts for only 1% of all cases of TB but 50% of these occur in the spine. Common clinical manifestations of spinal TB include fever, weight loss, back pain, lower limb weakness (either upper or lower motor neuron), and skeletal deformities such as kyphosis or gibbus. Infective pathology caused by Mycobacterium tuberculosis with epidural collection can present as acute cauda equina syndrome. Here, we describe a 28-year-old female patient who presented with low back pain and ascending weakness of both lower limbs (Medical Research Council grade

Challenges in the Diagnosis and Management of Spinal Tuberculosis: Case Series.

Though uncommon in developed countries, spinal tuberculosis must still be considered in patients with a suspicious clinical history, to avoid delays in treatment. This case series highlights the special considerations that need to be taken into account while tackling the diagnostic and therapeutic challenges associated with this disease. We present two interesting cases of spinal tuberculosis. The first case was a 26-year-old female who presented with chronic back pain and an initial misdiagnosis of ankylosing spondylitis. The second case was a 26-year-old male with new lower extremity weaknes

Spinal tuberculosis: a comprehensive review for the modern spine surgeon.

Nearly one-third of the human population is infected with tuberculosis. Of those with active disease, approximately 10% are impacted by skeletal tuberculosis. Though, traditionally a disease of the developing world and susceptible populations, with the rise of immigration, patients may present in developed countries. The microbe responsible is the mycobacterium tuberculosis complex bacillus. The infection begins in the anterior vertebral bodies. The natural history and presentation are notable for cold abscesses causing mass effect, early or late neurological deficit, and kyphotic deformity of

The outcomes of chemotherapy only treatment on mild spinal tuberculosis.

BACKGROUND: The treatments for spinal tuberculosis (TB) patients without absolute surgical indications have been controversial. Some people believed that most spinal TB patients were indicated for surgery, while other people believed in chemotherapy only. To help clarify the treatment over spinal TB patients without absolute surgical indications, we characterized a subtype spinal TB and then analyzed the treatment outcomes of standard chemotherapy alone. METHODS: In this retrospective study, 740 adult patients of spinal TB from January 2005 to January 2013 in our institution were reviewed. Pa

[Increasing the efficiency of surgical treatment for disseminated spinal tuberculosis].

The authors present the results of radical reconstructive operations in 80 patients with complicated spinal tuberculosis, by taking into account the duration (1.5-2 and 3-4 months) of preoperative antibacterial therapy. The efficiency of a radical reconstructive operations in the early stage (following 1.5-2 months) of hospital treatment involving intensive antibacterial and pathogenetic therapies is shown. Regress of spinal disorders is comparatively higher in the patients operated on in the early period than in the late one of antibacterial therapy. A year after surgery, there was recovery a
