Ulcers of the anorectal region are very variable in etiology and morphology. Often differential diagnosis is even possible by (excisional-)biopsy. As they are symptoms of any kind of disease, therapy has to be derived from this disease. Therefore surgery often only plays an additional part besides clearing the diagnosis, although the surgeon is mostly the first consultant, so being responsible for ongoing therapy. Some special diseases, their problems and therapeutical options, are described in detail.

In a study of 159 duodenal ulcer patients the authors studied the effect of H2-blockers, antibacterial and sucralfate monotherapy on the frequency of the ulcer healing, Helicobacter pylori (HP) infection and acid-proteolytic aggression in the body and antropyloric portion of the stomach. OBJECTIVE: To study whether factors such as age, duration of pre-perforation symptoms, size of perforation and operative evidence of chronicity could predict recurrence of ulcer after simple closure of perforated duodenal ulcers, thus enabling patients at high risk of recurrence to be subjected to definitive surgery instead of simple closure of perforation.
DESIGN: Retrospective. One hundred and fifty-two patients who had undergone simple closure of duodenal ulcer perforation in the last 10 years were included in the study.

An instrument has been developed which creates an experimental model of an acute bleeding gastric ulcer. The diameter and depth of these gastric ulcers are reproducible. The instrument can be used endoscopically or at laparotomy. Using this ulcer model nonsurgical modalities for the treatment of upper gastrointestinal bleeding can be compared in a controlled manner. This standard experimental model may also facilitate comparison of results among different research groups.
