PURPOSE: Achievement of urinary continence in patients with the exstrophy-epispadias complex remains a challenge. We reviewed our experience with the modified Young-Dees-Leadbetter bladder neck repair in patients with bladder exstrophy who underwent primary bladder closure elsewhere.
MATERIALS AND METHODS: We retrospectively reviewed exstrophy charts and database of 57 male and 11 female with classic bladder exstrophy who underwent bladder neck repair at our institute and successful primary bladder closure elsewhere during the last 2 decades. Osteotomy was performed at primary closure in 14 (

Decreased muscle contractility has been observed in association with intercellular collagen deposition after acute urinary obstruction. Chronically induced (2 months) bladder neck obstruction in rabbits produced similar marked changes in the ultrastructure of the bladder wall, with widening of the intercellular spaces and formation of collagen. Resting intravesical pressure was greater in animals with bladder neck obstruction than in the controls, yet response to stimulation was not impaired. Bacterial cystitis, in contrast, produced no consistent change in ultrastructure but the response of i

A 54-year-old man visited our clinic for dysuria. Transabdominal ultrasonography revealed a multilocular cyst at the neck of the bladder, and a cystoscopy revealed obstruction of the neck of the bladder. Dysuria improved after tamsulosin treatment was initiated, and abdominal magnetic resonance imaging (MRI) showed disappearance of the cyst. The patient had urinary difficulty again after 2 years. An MRI and cystoscopy revealed recurrence of the cyst. Tamsulosin administration was reinitiated, but his urinary difficulty did not improve. Transurethral resection of the cystic wall was performed.

The Bladder Neck Support Prosthesis is a Silastic vaginal device designed to elevate and support the bladder neck and proximal urethra during stressful physical activities. It mimics the effects of a urethropexy on urethral function without resulting in urinary outflow obstruction. Recent clinical studies in women with stress and mixed incontinence have demonstrated significant objective reductions in involuntary urine loss and high subjective satisfaction ratings.

A case of outlet obstruction caused by a V-shaped bar at the bladder neck in a woman was treated successfully by transurethral resection of the bar. The poor urinary flow in our patient was believed to be due to impedance during voiding caused by retraction of the bladder neck in the anteroposterior direction by the bar. Our case is different from previously reported cases.

A review is presented of the adrenoreceptors in the lower urinary tract, and their relationship to disorders of micturition. It is suggested that they may be of importance of neurogenic bladder disease, bladder neck obstruction, benign enlargement of the prostate, and certain drug and hormonal effects on the lower urinary tract, and that modification of their activity can be of benefit in the management of these conditions.

Hem-o-lok clips are widely used in robotic-assisted radical prostatectomy (RARP). However, clips-related complications have been reported, including intravesical migration. Here, we share a 60-year-old male case with newly diagnosed prostatic adenocarcinoma. With an unfavorable intermediate risk, he was admitted for RARP. He was discharged from hospital without any immediate complications. However, he reported progressive dysuria and slow urine stream 6 months after surgery. Cystoscopy showed severe bladder neck contracture (BNC), and 2 Hem-o-lok clips were found intravesically and removed dur

Common urologic complaints in the midlife man include bladder outlet obstruction, bladder hyperactivity, and large urinary output. Obstruction can result from benign prostate hypertrophy or some other problem distal to the bladder neck, such as urethral stricture. Hyperactivity can be induced by stress and caffeine or can suggest neurologic disease or bladder neoplasia. Large urinary output suggests excessive fluid intake, diabetes insipidus or mellitus, or mobilization of fluid from the use of diuretics or reclining at night. Sexual dysfunction may be caused by stress, but it is more often li

A case of giant vesical diverticulum is reported. A 31-year-old man was admitted with intermittent self-catheterization for 6 months duration due to urinary retention. A cystogram demonstrated a giant solitary diverticulum extended left-posteriorly, which compressed the bladder outlet and caused obstruction. On cystoscopy, the neck of the diverticulum was seen postero-lateral to left ureteral orifice. There was no vesical trabeculation, but slight obstruction of the bladder neck was suspected secondary to intermittent self-catheterization. A diverticulectomy was carried out by combined approac

Neurogenic urinary tract dysfunction is characterized by inadequate voiding and urinary incontinence. The aim of therapy nowadays is adequate bladder emptying and control of urinary incontinence. Neurogenic urinary incontinence can be caused by (a) detrusor hyperreflexia, (b) sphincter hypo- or areflexia, (c) a combination of both, or also (d) detrusor hyporeflexia with consequent overlow incontinence. Based on a simple urodynamic classification the current treatment strategies are presented. (a) Detrusor hyperreflexia can be transformed into hypo- or are-flexia pharmacologically with potent d

Endoscopic incision of the bladder neck, performed to relieve outflow obstruction, was evaluated as regards effects on ejaculation and orgasm in 61 men (mean age 48 years) by analyses of seminal fluid and a questionnaire. Relief of obstruction was confirmed by post-operative normalisation of urinary flow. In 47 cases there was unchanged antegrade ejaculation, while reduced semen volume was reported by 11 men and retrograde ejaculation by only 3. The quality of orgasm and sexual satisfaction were not permanently changed by the operation. Post-operative analysis of seminal fluid was performed in

Bladder outlet obstruction following treatment of pelvic cancer, predominantly prostate cancer, occurs in 1-8% of patients. The high incidence of prostate cancer combined with the long-life expectancy after treatment has increased concerns with cancer survivorship care. However, despite increased oncological cure rates, these adverse events do occur, compromising patients' quality of life. Non-traumatic obstruction of the posterior urethra and bladder neck include membranous and prostatic urethral stenosis and bladder neck stenosis (also known as contracture). The devastated bladder outlet can

CASE DESCRIPTION: A 19-year-old neutered male domestic shorthair cat was evaluated because of signs of urinary tract obstruction.
CLINICAL FINDINGS: Physical examination findings were consistent with urethral obstruction, and a mass could be palpated in the region of the bladder neck. Abdominal ultrasonography and thoracic radiography revealed a mass in the trigone of the urinary bladder and a solitary mass in the left caudal lung lobe. Cytologic examination of the urine sediment, samples obtained by means of traumatic urethral catheterization, and fine-needle aspirates of the bladder mass di

This paper reviews 165 cases of lower urinary obstruction in infancy. The commonest lesions were posterior urethral valves (91) and ectopic ureterocele (34), stenosis or atresia of the urethra (8), and bladder neck obstruction (6). There were 12 different diagnoses in the remaining 26 cases. The commonest presenting features were general in nature (110), and of these, failure to thrive and vomiting (87) were most frequent. The commonest urinary signs and symptoms were palpable bladder (93) and urinary infection (90). An accurate diagnosis was obtained by intravenous pyelography or cystourethro

Cystoscopy has been a well established procedure in adult urology for years and more recently for the endoscopic evaluation of the lower urinary tract of infants and children. Utilizing small caliber cold light endoscopes and resectoscopes, therapeutic manipulations are readily possible. Among these are extraction of ureteral stones, lithotripsy of bladder calculi, transurethral resection of bladder tumors, as well as transurethral incision and resection of bladder-neck obstructions, urethral valves and urethral stenoses. The excellent results obtained in the treatment of infravesical urinary

A prospective series of 104 patients underwent bladder neck incision (44 unilateral and 60 bilateral) for urinary obstruction caused by a small benign prostate enlargement. The preoperative mean peak flow value improved significantly from 11.4 ml/s to 16.2 ml/s. There were no significant differences in peak flow values between the unilateral and bilateral incision groups. Subjective results seemed to be similar but transurethral resection of the prostate was needed more often after unilateral incision than after bilateral incision. Postoperative complications were recorded only in the bilatera
