AIM: Chronic hypotension is not uncommon in uremic patients on regular hemodialysis. This subset of patients often requires multiple operations to maintain their vascular access due to frequent thrombosis and occlusion of the arteriovenous fistula. Our aims was to assess whether surgical intervention with the brachial artery-transposed basilic vein fistula is effective in chronic hypotensive hemodialysis patients.
MATERIALS AND METHODS: Fifty-four hemodialysis patients with chronic hypotension were enrolled in this study. Most ofthem were referred from local hospitals. They were 23 men and 31

BACKGROUND: Inframalleolar bypass still preserves its role in the modern endovascular era. Aim of this study was to evaluate the mid-term outcomes of "short" inframalleolar vein bypasses in patients with chronic limb-threatening ischemia (CLTI) after the previously failed tibial endovascular recanalization.
METHODS: Between January 2015 and December 2021,107 CLTI patients in 3 Italian Departments of Vascular Surgery underwent "short" inframalleolar vein bypass after failed tibial endovascular recanalization. Early (30-day) and 3-year outcomes were evaluated in terms of survival, primary paten

Vascular endothelium has multiple functions including regulating of vascular tone, preventing platelet aggregation, anti-proliferation, etc. An intact endothelial function is essential to the maintenance of an adequate vascular tone, to prevent platelet aggregation in the intimal surface of blood vessels, to prevent smooth muscle proliferation, and to prevent atherosclerosis. This review focuses on endothelial function related to the vascular tone in cardiac surgery. The review is composed by three sections. In the first section, normal endothelial function related to vascular tone is describe

Patients with diabetes mellitus (DM) are at increased risk of developing lower extremity peripheral arterial disease (PAD). The effect of DM on restenosis and patency rates in patients with PAD undergoing surgical revascularization or percutaneous interventions has not been fully clarified. We therefore critically reviewed the role of DM in restenosis, as well as primary and secondary patency rates in these patients. We searched Medline for studies investigating the effect of DM on restenosis (primary and secondary patency) rates in patients undergoing surgical/percutaneous interventions for t

RATIONALE & OBJECTIVE: Percutaneous arteriovenous fistulas (AVF) are created by establishing a proximal forearm anastomosis and offer a safe and reliable vascular access. This study compares the Ellipsys percutaneous AVF with a proximal forearm Gracz-type surgical AVF, chosen for comparison as it is constructed at the same anatomical site.
STUDY DESIGN: Retrospective study of prospectively collected clinical data.
SETTING & PARTICIPANTS: All vascular access procedures conducted during a 34-month period were reviewed. The study groups comprised 89 percutaneous AVFs and 69 surgical AVFs.

Stent grafts are widely used to treat dysfunctional hemodialysis vascular access, yet restenosis remains common and its predictors are incompletely understood. We analyzed 203 unique patients who underwent stent graft implantation between 2018 and 2022 from a prospectively maintained database. Geographic miss was defined as inadequate lesion coverage or suboptimal stent sizing; longitudinal geographic miss (LGM) was defined as residual stenosis >30% within 5 mm of either stent edge. The outcomes were loss of stent graft patency and access primary patency, assessed using Kaplan-Meier analysis a

BACKGROUND: A significant proportion of arteriovenous fistulas (AVFs) and grafts for hemodialysis fail due to early thrombosis, necessitating reintervention. The role of systemic heparinization in improving perioperative patency remains uncertain. This meta-analysis aims to evaluate the efficacy and safety of perioperative systemic heparin on the patency of upper-limb vascular access creations (fistulas or grafts placement).
METHODS: We searched PubMed, Embase, and Cochrane databases for randomized trials comparing heparinization versus standard care in adults undergoing arm AVF or graft plac

BACKGROUND: American Indians (AI) or Alaska Natives, or in combination with another race, comprised 6.8 million individuals in 2010 and the population is expected to exceed 10 million in the current census. Diabetes is more common in AIs than in other races in the United States and is responsible for 69% of new onset end stage renal disease in AI patients. The incidence of obesity is also higher among AIs. As both diabetes and obesity make creating a successful autogenous vascular access more challenging, we reviewed our experience creating arteriovenous fistulas in AI patients.
METHODS: Our

BACKGROUND: Ultrasound-guided percutaneous transluminal angioplasty (UG-PTA) has become the preferred endovascular intervention for arteriovenous fistula dysfunction. However, long-term patency rates remain variable, and identifying key factors affecting outcomes is crucial for clinical decision-making.
METHODS: This retrospective study analyzed 173 patients who underwent UG-PTA for arteriovenous fistula dysfunction at Xuzhou Central Hospital between August 2018 and August 2022. Kaplan-Meier survival analysis was used to assess primary patency rates, and Cox proportional hazards modeling iden

The purpose of this study was to determine whether tourniquet occlusion could be safely used on the upper extremity for vascular control during hemodialysis access surgery. The hospital and outpatient records of 44 patients undergoing 105 hemodialysis access procedures were retrospectively reviewed. In 48 procedures tourniquet occlusion was used for vascular control, whereas in 57 procedures vascular clamps were used. In those procedures in which the tourniquet was used, the mean tourniquet time was 30 minutes and the mean tourniquet pressure was 242 mm Hg. The operative time was significantly

Small-diameter vascular grafts (≤ 6 mm) remain a critical unmet need in cardiovascular surgery, as autologous vessels are unavailable in up to 30% of patients requiring coronary or peripheral bypass. Induced pluripotent stem cells (iPSCs) have emerged as a transformative cell source for tissue-engineered vascular grafts (TEVGs), offering unlimited self-renewal, patient-specific or universal donor potential, and the capacity to generate all vascular cell lineages. Recent breakthroughs-including iPSC-derived grafts achieving 100% patency in allogeneic primate models and the first United States F

This early clinical experience of members of the New England Society for Vascular Surgery with expanded polytetrafluoroethylene (PTFE) grafts was evaluated. Questionnaires were distributed to 52 active members of the Society and 32 answers were received. A total of 186 graft insertions were evaluated, of which 112 were positioned in the lower limb; 106 grafts were inserted for libm salvage. Patency rates for femoral-popliteal bypass grafts were determined by life-table analysis. Our results show a 6 to 9 month cumulative patency rate of 91% for PTFE grafts with a popliteal anastomosis above th

OBJECTIVE: Adverse iliofemoral anatomy may preclude complex endovascular aortic aneurysm repair (EVAR). In our practice, staged iliofemoral endoconduits (ECs) are planned prior to complex EVAR to improve vascular access and decrease operative time while allowing the stented vessel to heal. This study describes the long-term results of iliofemoral ECs prior to complex EVAR.
METHODS: Between 2012 and 2023, 59 patients (44% male; median age, 75 ± 6 years) underwent ECs before complex EVAR using self-expanding covered stents (Viabahn). For common femoral artery (CFA) disease, ECs were delivered p

Background and Objectives: Peripheral artery disease is one of the most common vascular pathologies. There is an ongoing debate among specialists on whether open or endovascular revascularization is preferred in cases of complex superficial femoral artery (SFA) lesions. The purpose of this study was to assess patency results of a relatively new transvenous endovascular bypass device. This could add to existing evidence and aid in comparison between open and endovascular bypass. Materials and Methods: Patients with complex TASC-C and D SFA lesions who had indications for revascularization were
