Predictors of ventricular tachycardia induction in syncopal patients with mild to moderate left ventricular dysfunction.

BACKGROUND: In patients with mild to moderate left ventricular dysfunction (LVD) (35% pound LVEF pound 50%) who present with syncope, demonstration of tachy and/or brady-arrhythmia has prognostic value. In this group of patients electrophysiological study (EPS) is often necessary. METHODS: A total of 53 consecutive patients with mild to moderate LVD and history of undetermined syncope underwent EPS. Sinus node function, His-Purkinje system conduction and ventricular electrical stability were evaluated. RESULTS: Twenty eight patients (52.8%) had induction of sustained monomorphic ventricular

Left atrial appendage thrombus in a term neonate with ventricular dysfunction.

Left atrial appendage thrombosis is exceedingly rare in neonates and may present with nonspecific findings. We report a term neonate presenting with poor feeding and a gallop rhythm, who was found to have a large left atrial appendage thrombus with ventricular dysfunction. Surgical excision resulted in rapid recovery, highlighting the importance of early echocardiographic evaluation in subtle neonatal heart failure.

Characteristics of pulmonary hypertension in adults with left ventricular diastolic dysfunction.

BACKGROUND AND OBJECTIVES: Left ventricular diastolic dysfunction (LVDD) is commonly associated with pulmonary hypertension (PHT); however, the factors associated with the presence and severity of PHT in patients with LVDD have not been well characterised. METHODS: We analysed the profiles and echo characteristics of 16 058 adults with LVDD and preserved left ventricular ejection fraction (LVEF, >50%) from the National Echocardiography Database of Australia. Peak tricuspid regurgitation velocity (TRV) was used to determine the presence of PHT. Univariate and multivariate analyses were perform

[The role of epicardial obesity in the development of left ventricular diastolic dysfunction].

AIM: To study the effect of epicardial adipose tissue on risk of left ventricular (LV) diastolic dysfunction (DD) in patients with visceral obesity. MATERIAL AND METHODS: Obesity leads to the development of LV DD and is a major cause of heart failure with preserved LV ejection fraction (HFpEF). However, the contribution of epicardial adipose tissue to DD is understudied. This study included 101 men with general obesity (body weight index, 32.9±3.6 kg /m2). Based on severity of epicardial obesity (EO), two groups were formed: group 1, patients with an epicardial adipose tissue thickness (EATt)

Determinants of Inadequate Cardioprotection in Adult Patients with Left Ventricular Dysfunction.

BACKGROUND: Perioperative cardioprotection is essential for achieving satisfactory clinical outcomes in heart failure patients. It is important to understand the factors affecting perioperative cardioprotection. METHODS: The institutional database was searched for patients with reduced ejection fraction (EF, < 40%) who underwent surgery with cardioplegia-induced arrest. Patients were divided into del Nido cardioplegia (DN) and cold blood cardioplegia (CB) groups. The relationships between age, preoperative blood parameters, creatinine, cross-clamp time (CCT), extracorporeal circulation time (

Left ventricular dysfunction and arrhythmias in asymptomatic patients with systemic sclerosis.

INTRODUCTION AND AIMS: Cardiac involvement in systemic sclerosis (SS) is frequently silent and a major cause of mortality in these patients. This work aims to study the prevalence and associations of left ventricular dysfunction (LVD) and arrhythmias in SS. METHODS AND RESULTS: Prospective study of SS patients (n=36), excluding those with symptoms of (or) cardiac disease, pulmonary arterial hypertension or cardiovascular risk factors (CVRF). A clinical, analytical, electrocardiogram (EKG), Holter, and echocardiogram with global longitudinal strain (GLS) assessment were performed. Arrhythmias

Prevalence and Outcomes of Primary Left Ventricular Dysfunction in Marfan Syndrome.

Even in the absence of significant valvular disease, patients with Marfan syndrome (MFS) have evidence of impaired left ventricular (LV) performance, suggestive of a primary cardiomyopathy. However, the true prevalence and long-term outcomes of this disease process remain largely unknown. We performed a retrospective analysis of all adult patients with confirmed MFS followed at Stanford Health Care. Those with significant valvular regurgitation, coronary artery disease, or previous cardiac surgery were excluded. LV systolic dysfunction was defined as a LV ejection fraction (LVEF) <55% on trans

Management of aortic valve disease in the presence of left ventricular dysfunction.

The development of heart failure in patients with aortic valve disease is associated with increased mortality, unless aortic valve replacement is performed. There is an especially high risk of death among patients with low ejection fraction. The heart failure must be treated and the patient's condition stabilized before surgery is performed. A delay in surgery, on the other hand, may lead to irreversible cardiac dysfunction and culminate in heart failure. Initial management must include an evaluation of the severity of the lesion and the functional state of the left ventricle. It is possible t

Fragmented QRS and mortality risk in patients with left ventricular dysfunction.

BACKGROUND: Fragmented QRS (fQRS) has been shown to predict cardiac events in select patient populations. Whether fQRS improves patient selection for primary prevention patients eligible for implantable cardioverter-defibrillator (ICD) therapy remains unknown. METHODS AND RESULTS: In a prospective, multisite cohort of 842 patients with left ventricular dysfunction (ejection fraction < or =35%) representing both ischemic and nonischemic etiology, the presence of fQRS on ECG was assessed using standardized criteria. The association between fQRS and all-cause and arrhythmic mortality was evaluat

Clinical diagnosis of left ventricular dilatation and dysfunction in the age of technology.

BACKGROUND: The diagnostic process has become increasingly dependent on instrumental and laboratory investigation. AIM: To evaluate the accuracy of symptoms and signs in identifying left ventricular (LV) dilatation and/or systolic dysfunction. METHODS: A group of 100 patients in stable clinical condition and scheduled for cardiac magnetic resonance imaging was prospectively examined by two cardiologists, who were unaware of the individual patient's condition. Patients were interviewed and underwent physical examination. RESULTS: Several symptoms and signs were associated with LV dilatation

MgSO4 prevents left ventricular dysfunction in an animal model of preeclampsia.

OBJECTIVE: We hypothesized that cardiac function would be reduced in a pregnant rat model of preeclampsia induced by L-NAME, a NOS inhibitor, and be reversed with magnesium sulfate prophylaxis. STUDY DESIGN: Female Sprague-Dawley rats were bred in-house. On gestational day 17, rats were anesthetized and osmotic minipumps were implanted to continuously deliver saline, L-NAME, or L-NAME and MgSO4. On gestational day 21, hearts were isolated and perfused in the working mode using Krebs Henseleit buffer. RESULTS: Pregnant rats treated with L-NAME displayed significant hypertension compared to th

Furosemide and the progression of left ventricular dysfunction in experimental heart failure.

OBJECTIVES: We tested the hypothesis that furosemide accelerates the progression of left ventricular systolic dysfunction in a tachycardia-induced porcine model of heart failure. BACKGROUND: Furosemide activates the renin-angiotensin-aldosterone system in patients with congestive heart failure (CHF). Such activation may contribute to CHF progression, but prospective data are lacking. METHODS: Thirty-two Yorkshire pigs were randomized to furosemide (1 mg/kg intramuscularly daily, mean 16.1 +/- 0.9 mg) or placebo. Thereafter, a pacing model of heart failure was utilized to produce systolic dys

[Tamponade in patients with systolic left ventricular dysfunction. An atypical presentation].

BACKGROUND: Left ventricular failure has been described following surgery due to localized compression of the left ventricle and in case of diastolic left ventricular dysfunction after pericardiotomy or pericardiocentesis. CASE REPORTS: Global heart failure was observed in 3 patients with dilated cardiopathy who developed tamponade. Systolic left ventricular dysfunction was caused by ischemic heart disease in one patient and secondary to anthracyclin chemotherapy in the two others. The effusion was successfully removed with pericardiocentesis in all three cases. No specific complications were

[Dynamic obstruction of the left ventricular outflow tract by prosthesis dysfunction].

We report on a case of dynamic left ventricular (LV) outflow tract obstruction combined with a prosthetic valve dysfunction 13 years following mitral valve replacement with a Hancock bioprosthesis in a 46-year-old patient. Previously, repeated echocardiographic controls had been performed at regular intervals and the prosthesis had been found to be oversized and seated in abnormal position, with projection of the struts into the LV outflow tract. Moreover, a mild chronic LV outflow tract obstruction had been diagnosed upon intermittent findings of relatively high velocity in the outflow tract.

Tachycardial disorders of cardiac rhythm as a cause of left ventricular dysfunction.

The study was designed to assess the effect of chronic and paroxysmal supraventricular tachycardia (SVT) on left ventricular (LV) function as well as to study changes in LV function following surgical correction of tachycardia. A total of 59 patients with SVT were examined (mean age 22.3 +/- 4.1 years), of these 29 had chronic SVT (Group I) and 30 had paroxysmal SVT (Group II). LV function was assessed using M-mode and Doppler echocardiography. Group I patients were found to have LV dysfunction the signs of which diminished considerably following surgical correction of tachycardia. In the peri

Diastolic filling in acute left ventricular dysfunction: role of the pericardium.

Patients with congestive heart failure and elevated left ventricular filling pressures demonstrate an abnormal pattern of diastolic filling that is characterized by a redistribution of diastolic filling to early diastole with reduced reliance on late diastolic filling. The diastolic filling pattern superficially resembles that which is seen with constrictive pericarditis. To examine potential mechanisms for these clinical findings, a model of ischemic left ventricular dysfunction was produced in seven dogs by repeated coronary microsphere embolization, producing a dilated left ventricle with r
