Pharmacokinetics of verapamil and its major metabolite, norverapamil from oral administration of verapamil in rabbits with hepatic failure induced by carbon tetrachloride.

The aim of this study was to investigate the pharmacokinetic changes of verapamil and its major metabolite, norverapamil, after oral administration of verapamil (10 mg/kg) in rabbits with slight, moderate and severe hepatic failure induced by carbon tetrachloride. The plasma verapamil concentrations in all groups of hepatic failure were significantly higher (p < 0.01) than the control. However, the plasma norverapamil concentrations in severe hepatic failure were significantly higher (p < 0.05) than the control. The peak concentrations (Cmax) and the areas under the plasma concentration-time c…

The Verapamil in Hypertension and Atherosclerosis Study (VHAS): results of long-term randomized treatment with either verapamil or chlorthalidone on carotid intima-media thickness.

BACKGROUND: It is unclear whether the carotid intima-media thickness can be influenced by antihypertensive treatment and whether some antihypertensive agents, such as calcium antagonists, may have a greater effect on this parameter than others, such as diuretics. The present paper reports the principal results of the ultrasound substudy of the randomized, prospective, controlled, Verapamil in Hypertension and Atherosclerosis Study (VHAS). DESIGN AND METHODS: In 498 hypertensive patients in eight Italian centres, randomized to either verapamil (240 mg once a day) or chlorthalidone (25 mg once …

Complete Heart Block Triggered by Nirmatrelvir-Ritonavir and Verapamil.

Ritonavir-boosted nirmatrelvir (Paxlovid) is a treatment for COVID-19. However, ritonavir, a cytochrome P450 (CYP) CYP3A4 inhibitor, poses a risk of significant drug-drug interactions. In our case, coprescribed nirmatrelvir-ritonavir and verapamil, with concomitant acute kidney injury, resulted in complete heart block and shock. Following fluid resuscitation, blood pressure support with norepinephrine, and calcium therapy for verapamil toxicity, the patient recovered and was discharged home in normal sinus rhythm. To our knowledge, only 2 cases with this drug-drug interaction have been documen…

Effect of Verapamil on Kidney Function Using Radionuclide Imaging.

BACKGROUND AND OBJECTIVE: Calcium channel blockers (CCBs) are among the most widely used prescribed drugs for the treatment of cardiovascular diseases. The present study investigates the effect of verapamil, which is most commonly used as a CCB, on kidney function using radionuclide imaging. METHODS: Ten New Zealand white rabbits were used in vitro (4) and in vivo (6) studies. Isometric tensions were recorded for isolated renal artery ring segments, while renographic studies were performed using Technetium-99m mercaptoacetyltriglycine and Gamma camera. Time to peak activity (Tmax) and time fr…

Clozapine-induced Supraventricular Tachycardia and its Treatment with Verapamil.

Clozapine is known to be an effective antipsychotic for schizophrenia. Cardiovascular complications associated with initiation of clozapine include tachycardia, postural hypotension, and myocarditis. Factors associated with development of cardiovascular adverse events are not clearly known, and dose titration has been described to be one among the associated risk factors. We report a case of a 45-year-old man with treatment resistant schizophrenia who developed supraventricular tachycardia during initiation of clozapine and discuss the role of verapamil in its successful management.

Lipid rescue of massive verapamil overdose: a case report.

INTRODUCTION: Massive intentional verapamil overdose is a toxic ingestion which can cause multiorgan system failure and has no currently known antidote. CASE PRESENTATION: The patient is a 41-year-old Caucasian woman who ingested 19.2 g of sustained release verapamil in a suicide attempt. Our patient became hypotensive requiring three high-dose vasopressors to maintain arterial pressure. She also developed acute respiratory failure, bradycardic ventricular rhythm necessitating continuous transvenous pacing, and anuric renal failure. Our patient was treated with intravenous calcium, bicarbonat…

Hemiplegic migraine: hyperperfusion and abortive therapy with intravenous verapamil.

A 20-year-old female with hemiplegic migraine was treated during an acute attack with intravenous verapamil, which reproducibly resolved the headache within 20 min but did not affect her hemiplegia. Magnetic resonance (MR) and computed tomographic (CT) angiography and perfusion performed during the attack showed vasodilation and hyperperfusion. Cerebral hyperperfusion concurrent with hemiplegia suggests a dissociation between cerebral perfusion and neuronal function in hemiplegic migraine. The beneficial effect of verapamil on headache but not hemiplegia suggests a distinct mechanism for pain …

Verapamil (VP) improves the outcome after renal transplantation (CRT).

Calcium antagonists (CATs) have a role in the management of certain types of renal insufficiency. These include prophylaxis against post-transplant-associated acute renal failure and cyclosporine A (CsA)-induced renal dysfunction. For the transplanted kidney, CATs may be beneficial in several settings. First, a CAT during organ procurement protects the kidney during ischemic periods. Second, CATs given perioperatively protect the kidney during reperfusion and early after transplantation. Third, CATs also offer protection against CsA nephrotoxicity.

Verapamil: an effective calcium blocking agent for pediatric patients.

Verapamil is a slow-channel calcium-blocking agent that has been released recently by the Food and Drug Administration for treatment of cardiac dysrhythmias in all age groups. Its primary action is to slow conduction in the atrioventricular node, thereby abolishing those types of supraventricular tachycardia using the atrioventricular node as a part of the reentry circuit or slowing the ventricular rate in someone with atrial flutter. Other investigations have shown that it is likely to relieve left ventricular outflow obstruction in patients with hypertrophic obstructive cardiomyopathy. Becau…

Prolongation of verapamil elimination kinetics during chronic oral administration.

The elimination of verapamil and its n-demethylated metabolite, norverapamil, was studied in nine patients with chronic atrial fibrillation after the first oral verapamil dose and during chronic oral verapamil administration. Significant increases (p less than 0.01) were seen in the elimination half-lives (t 1/2's) of both verapamil (6.4 +/- 3.5 to 12 +/- 5 hours, mean +/- SD) and norverapamil (10.3 +/- 6 to 16.5 +/- 7 hours) during chronic oral verapamil administration. These pharmacokinetic observations have important clinical implications for the rational long-term administration of this ag…

Verapamil hydrochloride: pharmacological properties and role in cardiovascular therapeutics.

Verapamil hydrochloride, a prototype calcium antagonist, is now marketed in the United States for the acute treatment of supraventricular tachyarrhythmias and for chronic management of vasospastic and chronic stable angina. It inhibits the slow inward channel in in the heart and blocks calcium influx in smooth muscle. Its intrinsic negative inotropic action, which is apparent in isolated tissues, is offset in vivo by peripheral vasodilation. It has a mild, noncompetitive sympathetic antagonist effect; its most important electrophysiologic action is a depression of AV nodal conduction, accounti…

Verapamil therapy in variant angina pectoris refractory to nitrates.

Conventional therapy including nitrates, beta blockers, and surgical bypass has proved to be generally unsatisfactory in patients with variant angina of suspected vasospastic etiology. Recent evidence regarding the role of calcium in the pathogenesis of vascular smooth muscle spasm prompted an open study of the clinical effects of verapamil in seven patients with recurrent rest angina refractory to nitrates. The patients studied met strict clinical ECG and angiographic criteria for vasospastic angina. All patients had rapid (within 24 hours) and complete (without recurrence of symptoms) chroni…

Verapamil in prophylaxis of paroxysmal atrioventricular nodal reentrant tachycardia.

We examined by electrophysiologic study and clinical follow-up the use of verapamil as a prophylactic agent in 13 patients with refractory paroxysmal supraventricular tachycardia due to atrioventricular (AV) node reentry. Electrophysiologic variables and initiation and maintenance of AV nodal reentry were studied by programmed electrical stimulation. Observations were made before and after intravenous administration of verapamil, 0.15 mg/kg. Twelve of the 13 patients had previously not been controlled by other antiarrhythmic agents. Before verapamil, AV nodal reentry was induced in all 13 pati…

Comparative pharmacology of calcium antagonists: nifedipine, verapamil and diltiazem.

Calcium antagonists (slow channel blocking agents) are a very heterogeneous group of agents with dissimilar structural, electrophysiologic and pharmacologic properties. Nifedipine is a potent, long-acting vasodilator that has proved highly efficacious in relieving anginal symptoms caused by coronary vasospasm. In vivo, it exerts no myocardial depressant effects and has no antiarrhythmic properties. Treatment with nifedipine can safely be combined with administration of a beta receptor blocking agent. VErapamil prolongs atrioventricular (A-V) conduction (A-H interval) in a dose-dependent manner…
