[Vertigo in the elderly patient].

Among the 52 patients, aged 70 and older, who came at office for vertigo, one or more specific diagnoses were made in 36 patients (69.23 percent). The most common diagnoses being benign positional vertigo (29 percent) and Ménière's disease (15.4 percent). Other associated pathologies were identified in 38 patients (73.1 percent). The mechanisms involved are discussed.

Histamine in the treatment of vertigo.

The vasodilating properties of histamine were the basis for histamine treatment of episodic vertigo and other inner ear dysfunctions. The successes obtained led to the development of betahistine: an orally active histamine analogue; its general pharmacology resembles that of histamine. Animal pharmacology experiments proved that betahistine increases cerebral blood flow and probably also affects vestibular neurons. From clinical studies, it appears that betahistine is an effective agent for the symptomatic treatment of Meniere's syndrome. Efficacy has also been shown in the treatment of patien…

Cluster headache and vertigo.

The term "cluster vertigo" was originally used by Gilbert to describe episodes of vertigo in patients with Ménière's syndrome. Since these patients also had co-existing cluster headache, he suggested that both disorders could have had a common pathophysiology. There is no evidence in the literature for an increased incidence of Ménière's syndrome in cluster headache patients, so the argument that cluster headache and Ménière's syndrome may have a common pathogenesis cannot be supported. Subsequent authors have used the term "cluster vertigo" to denote a variant form of cluster headache and hav…

Surgical treatment of vertigo.

Surgical therapy for disabling vertigo usually comes in at a late stage in the course of treatment for this patient group. The surgical procedures can be divided into two categories: non-destructive and destructive surgery. The former constitutes primarily procedures directed against the endolymphatic sac such as endolymphatic sac shunt procedures or decompression procedures. Peri-endolymphatic shunt procedures have also been proposed by some surgeons. None of these latter procedures have, however, gained widespread clinical acceptance. Destructive surgery is aimed against the end-organ itself…

Pharmacological treatment of vertigo.

A review of the treatment of vestibular disorders with drugs is presented. In animal experiments, administration of certain drugs, e.g. alcohol, barbiturates and chlorpromazine, retards the vestibular compensation, while others, e.g. caffeine, amphetamine and ACTH, accelerate the compensation. In compensated animals, some drugs produce overcompensation and some decompensation. A double-blind, randomized trial of the effect of scopolamine and dimenhydrinate in patients with vertigo of varying origin showed, as compared with placebo, that these drugs did not significantly alleviate the vestibula…

Diagnosis and management of vertigo.

Patients with vertigo may be best evaluated when the physician has the various entities that may be responsible clearly in mind. Those diseases listed in group II are more familiar to the primary care physician than those listed in group I, which belong more in the realm of neuro-otology than in general medicine and neurology. The proper use of history, physical findings, and laboratory evaluations will allow the clinician to classify the patient's problem and to initiate treatment and consider appropriate referral when indicated.

Perspectives on the pharmacotherapy of vertigo.

Recent evidence from experiments in animals suggests a rationale for more effective pharmacologic and physical therapy in patients with vertigo. In particular, one must consider the possible effects of medications on the adaptive processes that naturally mediate recovery from vestibular lesions. The challenge now is for clinicians to devise medication and exercise regimens for patients with vestibular disturbances and critically (and quantitatively) evaluate their effects.
