Psychometric Properties of Vestibular and Ocular Measures Used for Concussion Assessments: A Scoping Review.

BACKGROUND: Concussions most commonly affect the vestibular and ocular systems. Clinical measures used in the assessment of vestibular and ocular deficits should contain strong psychometric properties so that clinicians can accurately detect abnormality to guide treatment interventions. OBJECTIVES: The aim of this scoping review was: (1) to identify the measures used to evaluate the vestibular and ocular domains postconcussion and (2) to document the psychometric properties of the measures. METHODS: Two databases (Medline (Ovid) and Embase) were searched from inception to May 2023. An update

The tilting room and posturography.

The tilting room examination technique is detailed. The advantages of posturography with the tilting room as an additional tool in the clinical vestibular examination are described for patients with uni- and bilateral loss of vestibular function, for patients with an acoustic neurinoma and for patients suffering from cervical ataxia. Some illustrations are also given of other applications of the tilting room.

[Vestibular ataxia caused by meningeal carcinomatosis].

A 74-year-old man developed progressive deafness and unsteady gait two years after gastrectomy for a gastric cancer. Neurological examination revealed an alert and intelligent Japanese male in no acute distress. The optic fundi were normal. The pupils and the extraocular muscles were normal, however, horizontal nystagmus was noted in right and left gaze. He showed marked bilateral deafness, and loss of caloric response bilaterally. No muscle atrophy nor weakness was noted. His gait was wide-based and ataxic. Tandem gait was impossible. Romberg sign was present. No cerebellar ataxia was noted i

Using HINTS in acute vestibular syndrome: A practical guide for the acute care physician.

Acute vestibular syndrome (AVS) is a common clinical presentation. Common causes include peripheral, self-limiting labyrinthine disorders, but a significant proportion are caused by posterior circulation stroke (PoCS). Delineating between a peripheral versus central cause in AVS is challenging for acute care physicians, but a simple, three-step bedside test known collectively as the HINTS examination may be useful. Substantial evidence supports the use of the HINTS examination in detection of PoCS in AVS, but it is important that performer is trained and experienced to ensure reliability of th

[Inferior vestibular neuritis: diagnosis using VEMP].

Vestibular evoked myogenic potentials (VEMP) are a new method to establish the functional status of the otolith organs. The sacculocollic reflex of the cervical VEMP to air conduction (AC) reflects predominantly saccular function due to saccular afferents to the inferior vestibular nerve. We describe a case of inferior vestibular neuritis as a rare differential diagnosis of vestibular neuritis. Clinical signs were a normal caloric response, unilaterally absent AC cVEMPs and bilaterally preserved ocular VEMPs (AC oVEMPs).

Characterization of Vestibular Phenotypes in Patients with Genetic Hearing Loss.

Background: The vestibular phenotypes of patients with genetic hearing loss are poorly understood. Methods: we performed genetic testing including exome sequencing and vestibular function tests to investigate vestibular phenotypes and functions in patients with genetic hearing loss. Results: Among 627 patients, 143 (22.8%) had vestibular symptoms. Genetic variations were confirmed in 45 (31.5%) of the 143 patients. Nineteen deafness genes were linked with vestibular symptoms; the most frequent genes in autosomal dominant and recessive individuals were COCH and SLC26A4, respectively. Vestibular…

Physiologic effects on the vestibular system in Meniere's disease.

Ménière syndrome is an inner ear disorder characterized by spontaneous attacks of vertigo, fluctuating low-frequency sensorineural hearing loss, aural fullness and tinnitus. When the syndrome is idiopathic and cannot be attributed to any other cause (eg, syphilis, immune-mediated inner ear disease, surgical trauma), it is referred to as Ménière disease. This article reviews the physiologic effects of Ménière disease on vestibular function, as measured by caloric, head impulse, and vestibular-evoked myogenic potential testing.

Vestibular Function and Clinical Presentation of Dizziness: Are They Similar in Patients With Different Types of Migraine?

OBJECTIVE: To compare the vestibular function and clinical aspects (vestibular and migraine symptoms) of patients divided into three groups-migraine without aura, migraine with aura, and chronic migraine-and a control group by using electronystagmography and a design questionnaire. STUDY DESIGN: Case-control study. SETTING: Tertiary referral center. PATIENTS: Women aged between 18 and 55 years diagnosed with migraine with aura, migraine without aura, or chronic migraine according to the International Classification of Headache Disorders ICHD-third edition; diagnosis was made by a headache s

A proposed method to comprehensively define outcomes in acoustic tumor patients undergoing CyberKnife management.

BACKGROUND: Vestibular assessment in patients with acoustic tumor (so-called vestibular schwannoma, VS) via ocular vestibular-evoked myogenic potential (oVEMP) and cervical VEMP (cVEMP) tests are not often discussed in the neurosurgical literature. OBJECTIVES: This study conducted physiological and morphological assessments for VS patients before and after CyberKnife radiosurgery. METHODS: Twenty patients with unilateral VS underwent a battery of tests comprising facial nerve function test, audiometry, and caloric, oVEMP and cVEMP tests before and 2 years after CyberKnife treatment at a mean

[Acute vestibular syndrome].

Acute vestibular syndrome (AVS) requires a systematic approach to differentiate between central and peripheral causes, as conditions like stroke and vestibular neuritis can present similarly. The head impulse, nystagmus, skew deviation, and hearing test examination (HINTS+) is effective in distinguishing strokes with high sensitivity. Most important for treatment of peripheral AVS includes vestibular rehabilitation, while central causes require targeted therapies. A multidisciplinary approach is essential for effective diagnosis and management in order to improve outcomes and quality of life,

The use of vestibular tests for the selection of anti-vertigo drugs.

Vertigo is not a disease, but a symptom. Therefore, the therapy of vertigo is primarily directed at eliminating the underlying cause. In about half of the cases the cause will not become apparent, however, and the therapy is doomed to be symptomatic. Several types of drugs have been found to have an anti-vertigo activity, e.g. antihistamines, calcium antagonists, anti-emetics. For the selection of vestibular depressants, the vestibular caloric test and the rotation test can be used. The duration of the nystagmus induced by rotation is the most useful parameter. The lag-period, the extent and d

[Analysis of vestibular function characteristics in vestibular schwannomas at different stages: two case reports and literature review].

Vestibular schwannoma(VS) grows slowly, with a striking contrast in clinical manifestations at different stages. During the growth phase of VS, it often presents as an acute vestibular syndrome similar to vestibular neuritis, with prominent vestibular symptoms such as vertigo and balance disorders. However, with the slow growth of the tumor, central vestibular compensation develops promptly, resulting in the absence of typical vertigo indicative of unilateral vestibular injury, which may be inconsistent with vestibular test findings. This paper reports two cases of VS at different disease stag

Vestibular evoked myogenic potentials: an overview.

UNLABELLED: The vestibular evoked myogenic potential (VEMP) test is a relatively new diagnostic tool that is in the process of being investigated in patients with specific vestibular disorders. Briefly, the VEMP is a biphasic response elicited by loud clicks or tone bursts recorded from the tonically contracted sternocleidomastoid muscle, being the only resource available to assess the function of the saccule and the lower portion of the vestibular nerve. AIM: In this review, we shall highlight the history, methods, current VEMP status, and discuss its specific application in the diagnosis of
