Recurrent vulvar melanoma in 35-year-old pregnant women.

Vulvar melanoma represents between 3% and 10% of vulvar neoplasms. We present a case of a 34-year-old pregnant woman presenting with a pigmented lesion on the left labium majus; she reported no family history of melanoma. The histological diagnosis was malignant melanoma, superficial spreading type, with Breslow thickness of 0.9 mm; the excision was complete. Eight months before, an atypical genital nevus was completely excised from a nearby location. The pregnancy was finished by cesarean delivery at term, and 3 months later, another pigmented lesion was noticed near but not within the scars.…

Advanced vulvar adenocarcinoma of mammary gland type: a case report.

Vulvar adenocarcinoma of mammary gland type (AMGT) is a rare, locally aggressive neoplasm of anogenital mammary-like glands that mimics the histologic and immunophenotypic features of breast carcinoma. Fewer than 60 cases have been described in the literature since the first documented case in 1936. The extreme rarity and resemblance to breast carcinoma complicate diagnosis and treatment. This report presents a case of AMGT in a 61-year-old woman who presented with a palpable, subcutaneous lesion of the lateral mons pubis and labia minora. Initial biopsy identified the lesion as metastatic car

Clinicopathologic analysis of angiomyofibroblastoma of the female genital tract.

OBJECTIVE: To study the clinicopathologic features and immunoprofile of angiomyofibroblastoma (AMF) with emphasis on differential diagnosis. METHODS: Seven vulvar, two vaginal and one perineal angiomyofibroblastomas were evaluated by light microscopy and immunohistochemistry. The immunohistochemical studies were performed on paraffin-embedded sections using the standard ABC method with a panel of antibodies. RESULTS: Angiomyofibroblastoma predominantly occurred in middle-aged women in the genital region, especially in the superficial area of the vulva. Clinically, most of the tumors presente

Ewing sarcoma/primitive neuroectodermal tumor (PNET) of the vulva. Case report and review of the literature.

Ewing sarcoma is a rare highly malignant neoplasm that is mostly encountered as a primary bone tumor of childhood. Extra osseous occurrence is less frequent and cases involving the vulva are very rare with only twelve reported to date. We report the case of a Ewing sarcoma that presented as a vulvar mass in a 28-year-old Lebanese lady and briefly review the differential diagnosis as well as the published literature regarding the clinical presentation and prognosis.

Loss of MHC Class I Expression in HPV-associated Cervical and Vulvar Neoplasia: A Potential Mechanism of Resistance to Checkpoint Inhibition.

Tumor cell expression of major histocompatibility complex (MHC) class I is required for antigen presentation and adaptive immune recognition. Absent or diminished MHC class I expression is thought to contribute to immunotherapeutic resistance in some epithelial tumors but has not been previously studied in cervical and vulvar carcinoma. Given that anti-programmed cell death 1 (PD-1) checkpoint inhibition is deployed for programmed cell death ligand 1 (PD-L1)-positive recurrent and metastatic cervical squamous carcinomas, identifying tumors with loss of MHC class I is of clinical interest to op

Sentinel Node Mapping in Gynecologic Cancers: A Comprehensive Review.

Gynecologic cancers are one of the most important causes of women death worldwide. The sentinel lymph node concept was introduced by Cabanas in 1977 for the penile cancer. This technique was proven safe and feasible in selected cancers such as breast cancer, melanoma, or some gynecologic cancers. Sentinel lymph node mapping is increasingly used in early stages of cervical or vulvar cancer in particular due to the safety, high detection rate, and sensitivity of the technique. In this review, we will discuss in depth the most recent evidence of nuclear medicine and other techniques used to deter

Early vulvar squamous neoplasia: advances in classification, diagnosis, and differential diagnosis.

The recognition and classification of preinvasive vulvar neoplasia are complicated by the facts that (a) their respective carcinomas have a diverse (human papillomavirus [HPV]- and non-HPV-related) pathogenesis; (b) not all vulvar squamous carcinomas are associated with precursors with strictly defined morphologic features; (c) many carcinomas have epithelial changes that are abnormal but lack sufficient nuclear atypia to warrant classification as an intraepithelial neoplasm; and (d) even lesions associated with a common etiologic agent (HPV) present a diverse morphologic spectrum. In this rev

[Dermatofibrosarcoma protuberans of the vulva].

Dermatofibrosarcoma protuberans (DFSP) is a locally malignant neoplasm derived from fibrous tissue. Similar picture of such lesions to malignant tumors as well as to benign changes creates a risk of too gentle treatment and consequently of recurrence and malignant transformation of the lesions with possible remote metastases. Therefore accurate diagnosis and aggressive surgical treatment can prevent the recurrence of the tumor. Authors of the study present case report of 49-year-old woman with DFSP of vulvar area.

Leiomyosarcoma of the Vulva Mimicking as Chronic Bartholin Cyst: A Case Report.

Smooth muscle neoplasms of the vulva are rare, and when localized in the Bartholin's gland, it can be mistaken as a benign lesion leading to a delay in diagnosis. We report a case of leiomyosarcoma of the Bartholin's gland in a 63-year-old postmenopausal woman, which clinically mimicked a chronic Bartholin's gland cyst. This case report emphasizes the importance of suspecting malignancy in any vulvar lesion in women of postmenopausal age, despite its location and asymptomatic appearance. A prompt, careful evaluation with adequate clinical judgment will help to manage such cases efficiently.

Vaginal foreign body: Successful management with vaginoscopy.

The etiology of chronic vaginal discharge in children varies and can be seen as infection, sexual abuse, congenital malformations, vulvar skin disease, vaginal neoplasms and a foreign body. A vaginal foreign body is not a common problem in childhood but it should always be considered when a little girl consults a physician with a chronic vaginal discharge problem. We present the diagnosis and treatment management via vaginoscopy applied to a 6 year old girl who complained of a foul smelling vaginal discharge that had been resistant to medical treatment for the last two years.

Sonographic findings of a large vulvar lipoma.

Lipomas of the vulva are rare benign tumors that consist of mature fat cells often interspersed with strands of fibrous connective tissue. They arise from the vulvar fatty pads and present as soft, multilobulated subcutaneous neoplasms. Histological examination reveals a thin capsule surrounding a lobular proliferation of lipocytes. Liposarcomas of the vulva have been described rarely. We present the sonographic findings of a large lipoma of the vulva, and demonstrate the contribution of topical application of a high-frequency transvaginal transducer in depicting lobular structural features, c

Angiomyofibroblastoma of the cervix uteri: a case report.

BACKGROUNDS: Angiomyofibroblastoma (AMFB) is a rare histopathologic finding of the female lower genital tract. This tumor belongs to the group of mesenchymal tumors. Mesenchymal neoplasms of the modified genital skin and mucosa are uncommon. The majority of these lesions are seen in females and, collectively, they form a family of vulvovaginal soft tissue tumors. This family includes fibroepithelial stromal polyps, angiomyofibroblastoma, cellular angiofibroma, aggressive angiomyxoma, vaginocervical myofibroblastoma, vulvar leiomyomatosis, and other smooth muscle tumors. Angiomyofibroblastoma i

Primary solitary fibrous tumor of the vulva: a case report.

BACKGROUND: Solitary fibrous tumor (SFT) of the female genital tract is an extremely rare neoplasm of mesenchymal origin. CASE: A case of primary SFT originated from the vulva of a 39-year-old woman. The tumor presented as a 10-cm, well-circumscribed lump and was composed of bland-looking cells admixed with thin and thick collagen fibers with the appearance of hemangiopericytoma. Immunohistochemically, the spindle cells were strongly positive for CD99 and vimentin, with focal positivity for CD34, and were negative for S100, SMA, desmin, ER, PR, etc. Microscopic evaluation and immunohistochemi

Vulvar Hemangioma: Case Report.

Hemangioma is a benign neoplasm that may affect the vulva, and it can cause functional or emotional disability. This article reports the case of a 52-year-old female patient with a history of a genital ulcer for the past 3 years and who had undergone various treatments with creams and ointments. The patient was biopsied and diagnosed with vulvar hemangioma and was subsequently submitted to surgical excision of the lesion. We emphasize the importance of following the steps of the differential diagnosis and proceeding with a surgical approach only if necessary.

Allelic imbalance in lichen sclerosus, hyperplasia, and intraepithelial neoplasia of the vulva.

BACKGROUND: Few studies have addressed in detail the genetic alterations that occur in vulvar squamous carcinomas (VSCC) and their precursor lesions. In a previous study, we determined the most common chromosomal loci for allelic imbalance (AI) in HPV-positive and -negative VSCCs. The present study was designed to determine whether AI and the microsatellite instability phenotype (MIN) were present in epithelial lesions known to be associated with VSCC. DESIGN: Fifty-seven epithelial loci were analyzed, including HPV-positive (classic) and -negative (differentiated) vulvar intraepithelial neop

Diagnosing Basal Cell Carcinoma of the Vulva: A Case Report and Review of the Literature.

Basal cell carcinoma (BCC) is a highly prevalent epidermal neoplasm that most commonly occurs in regions of sun-exposed skin, though rare cases arise in sun-protected areas. BCCs of the vulva account for a small fraction of cases and can be mistaken for other cutaneous genital pathologies on clinical examination. Here we report a case of vulvar BCC that presented as a firm, tender bilateral lesion of the mons pubis and was diagnosed by histopathology and immunostaining for classical BCC markers.
